Актуальность Трофические язвы на фоне злокачественных новообразований кожи, мягких тканей и костей обнаруживают приблизительно в 1-1,5% случаев. В виду редкой встречаемости и разнообразного течения, ранняя диагностика затруднительна и возможна только с применением цитологического исследования отделяемого и мазков-отпечатков, а также биопсии различных подозрительных участков краев и дна язвы с гистологическим исследованием. Методы лечения и прогноз выздоровления зависят от стадии онкологического процесса, определяющей возможность проведения радикального вмешательства.Цель исследования Описание случая наблюдения пациента с трофической язвой на фоне меланомы кожи.Материалы и методы Представлен пациент 74 лет с направляющим диагнозом: сахарный диабет, тип 2, компенсация, синдром диабетической стопы, гнойно-некротическая рана правой стопы. Язва существует более 3 лет. Лечился амбулаторно в поликлинике по месту жительства, неоднократно проходил стационарное лечение в хирургическом отделении. За 3 года рана увеличилась в несколько раз. В ходе обследования произведена биопсия из края язвы, в результате которой диагностирована злокачественная меланома кожи с изъязвлением. Пациенту проведено хирургическое лечение в онкологи-ческом диспансере.Выводы Отсутствие онкологической настороженности врачей может приводить к длительному неадекватному лечению больных с трофическими язвами на фоне злокачественного заболевания, способствуя распространению процесса и делая невозможными радикальные вмешательства. Прогноз онкологического заболевания зависит от стадии процесса и зачастую определяется возможностью проведения радикального вмешательства.
