ель исследования. Определить ультразвуковые критерии строения и функ-ции здоровых височно-нижнечелюстных суставов. Материалы и методы. В настоящее исследование были включены результаты ультразвуковых исследований ВНЧС 9 студентов-волонтѐров (16 суставов; 5 женщин -55,55% и 4 мужчин -44,45%; средний возраст -22,9 лет). Всем волонтерам была проведена магнитно-резонансная томография для исключения наличия патологи-ческих дислокаций суставных дисков. Ультразвуковое исследование проводилось c ис-пользованием высокочастотного 12 МГц линейного датчика на портативном аппарате Samsung SONO ACE R3.Результаты. По данным внешнего осмотра волонтеров, степень открывания рта варьировала от 4.4 см до 5.4 см, среднее значение составило 4.94 ± 0,39 см. Были опре-делены средние показатели толщины суставного диска ВНЧС в области его передней, средней и задней частей, которые составили 0,42 ± 0,07 см, 0,35 ± 0,07 см и 0,41 ± 0,05 см соответственно. Среднее значение высоты нижней суставной щели равнялось 0,12 ± 0,03 см. Среднее значение степени подвижности головки нижней челюсти в параса-гиттальной плоскости составило 14,7 ± 1,38 мм.Заключение. Полученные результаты ультразвуковых критериев строения и функции здоровых ВНЧС соответствуют положениям нормальной анатомии человека и показателям нормы, установленным для МРТ ВНЧС, и могут быть использованы для ди-агностики внутренних нарушений ВНЧС.Ключевые слова: височно-нижнечелюстной сустав, ВНЧС, МРТ, норма, УЗИ, уль-трасонография.Контактный автор: Юркевич Р.И.,
The paper presents the results of examination of patients with articulation disorders of the lower jaw caused by internal pathology of the TMJ. The purpose of the presented work: to study the effectiveness of the use of mechanical and virtual articulators in the functional diagnosis of patients with internal TMJ disorders. All patients underwent comprehensive clinical and instrumental examination including cone-beam computed tomography (CT) and axiographic examination (optical axiograph Dentograf Prosystom, Russia). CBCT was used to assess the state of the TMJ and determine the individual ratio of jaw and joint models. When axiography was recorded and analyzed articular trajectories of the lower jaw. In the first group of patients dynamic occlusion was evaluated using a mechanical articulator, in the second group a virtual articulator was used. It was revealed that the use of mechanical articulators in functional diagnostics to assess dynamic occlusion is limited and does not allow to obtain individualized patient data, their efficiency was 75%. The use of virtual articulators allows to evaluate the dynamic occlusion during opening and closing of the mouth, protrusion and laterotrusion, as well as the continuous movement of the lower jaw with the registration of all possible dental contacts. Due to the combination of CT data of the patient’s head and virtual models, the highest accuracy of placing models in the virtual articulator in accordance with the individual characteristics of patients was achieved.
В работе представлен анализ исследований ряда отечественных и зарубежных авторов, посвященных изучению различных окклюзионных капп, в частности, механизмам их действия, классификации, их эффективности при различных формах поражения височно-нижнечелюстного сустава (ВНЧС), преимуществах и недостатках отдельных капп, а также материалов, используемых при их изготовлении. Как показал анализ представленных научных работ, использование окклюзионных капп в комплексном лечении нарушений функции ВНЧС позволяет достичь нормализации пространственных взаимоотношений внутрисуставных структур и устранить хроническую механическую травму тканей ВНЧС. Выбор оптимального метода лечения внутренних нарушений ВНЧС следует проводить с учетом положения головок нижней челюсти, характера смещения суставного диска, наличия или отсутствия болевого синдрома, возможных причин, его вызвавших, и существования местных и общих отягощающих факторов. Применение разобщающих капп позволяет установить суставной диск в физиологически правильное положение относительно головки нижней челюсти и тем самым устраняет предпосылки развития дисфункции ВНЧС. По мнению как российских, так и зарубежных исследователей, использование разобщающих капп способствует повышению эффективности консервативного лечения дисфункции ВНЧС, а также профилактике таких осложнений, как вторичный остеоартроз и не вправляемое смещение суставного диска. AIM: The aim of this work is to increase the quality and accuracy of the prostheses manufacture on implants in patients with completely absent dentition, to reduce the number of doctors’ visits to creation of temporary crowns, and more quickly adapt to new prostheses. MATERIALS AND METHODS: A survey and comprehensive treatment of 55 people with complete edentulousness were carried out. Group I consisted of 30 patients who underwent treatment using the developed prototyping method, which took into account the shape of the complete removable dentures and the ratio of the jaws during the manufacture of the prosthesis on implants. The treatment of Group II, consisting of 25 patients, was carried out using a standard technique - after opening the implants, the central ratio of the jaws and the setting of the teeth were re-determined. RESULTS: The treatment of patients with completely absent dentition using the developed prototyping methodology allowed to increase the accuracy of manufacturing temporary dentures based on dental implants, reduce the time of adaptation to the prosthesis and achieve high aesthetic results. CONCLUSION: A prosthesis made using the proposed methodology for transferring the parameters of a temporary dentures to a permanent ones based on dental implants meets all functional and aesthetic requirements, improves the quality of prosthetics, reduces the manufacturing time of the prosthesis, the patient’s adaptation time to it, and is the basis for improving the quality of patient’s life.
