The endothelial dysfunction (ED) in diabetic foot syndrome (DFS) is manifested in the imbalance between the formation of vasodilating and vasoconstrictive substances, not only in the affected area (lower limb), but in the other areas of the peripheral circulation (skin of dorsum of the hand). The aim of this study was the comparative evaluation of endothelial function in patients with neuropathic and neuroischemic forms of DFS in the remote skin tissue, that is, skin. Laser Doppler flowmetry (LDF) was used to assess skin perfusion. The study was conducted in patients with neuropathic and neuroischemic forms of diabetic foot receiving standard hospital treatment at the surgical ward of complicated forms of diabetic foot syndrome in the St. Petersburg's City Hospital 14. Control group comprised patients with proven absence of manifestations of diabetes mellitus and cardiovascular disease. In patients with neuropathic and neuroischemic forms of DFS the significant decrease in microcirculation in the skin was observed. Assessment of the functional state of endothelium in patients with neuropathic and neuroischemic forms of DFS was conducted using functional tests (cold test and iontophoresis of acetylcholine or nitroglycerine). We found significant decrease in endothelium-dependent and endothelium-independent vasodilatation, which was more prominent in neuroischemic forms of DFS.
АННОТАЦИЯ Цель. Повысить эффективность лечения пациентов с хроническим генерализованным пародонтитом путем применения бактериотоксической светотерапии с последующим хирургическим вмешательством с остеопластическим материалом Био-Ген. Материалы и методы. Под наблюдением находилось 30 человек-основная группа, а также 30 человек-контрольная группа. Всем пациентам основной группы до оперативного вмешательства была проведена бактериотоксическая светотерапия с фотосенсибизизатором "Фотолон", разведенным по инструкции. Далее было проведено хирургическое лечение хронического генерализованного пародонтита с применением остеопластического материала Био-Ген. Пациентам контрольной группы было проведено лечение заболевания общеизвестным консервативным методом. У всех пациентов было изучено состояние микроциркуляции тканей пародонта методом лазерной допплеровской флоуметрии, определен состав микрофлоры содержимого пародонтальных карманов до и после проведенного лечения. Результаты. В результате проведенного нами комплексного лечения хронического генерализованного пародонтита (основная группа) с применением бактериотоксической светотерапии с фотолоном с последующим хирургическим вмешательством с применением остеопластического материала Био-Ген спустя 12 месяцев данные лазерной допплеровской флоуметрии показали улучшение состояния микроциркуляции тканей пародонта. После проведенного лечения отмечено значительное уменьшение числа выделяемых микроорганизмов. В контрольной группе пациентов не было выявлено значительного улучшения состояния микроциркуляции тканей пародонта, а также снижения количества основных пародонтопатогенных видов микроорганизмов. Заключение. Сочетанное применение бактериотоксической светотерапии и хирургического лечения с применением остеопластического препарата Био-Ген при лечении хронического генерализованного пародонтита демонстрирует выраженный терапевтический эффект, происходит улучшение микроциркуляции тканей пародонта. Микробиологическое исследование у всех пациентов после проведения лечения показало высокую эффективность данного метода в отношении основных видов пародонтопатогенных микроорганизмов.
