Исследование посвящено анализу динамики волемического статуса и параметров системы кровообращения у больных с острой кишечной непроходимостью в предоперационном периоде. Указанные параметры определялись с помощью метода импедансометрии посредством тетраполярной реовазографии. Обследованы 100 больных с явлениями кишечной непроходимости. Длительность заболевания колебалось от 6 до 96 часов. Возраст пациентов варьировал от 28 до 90 лет. Пациенты были разделены на 2 группы в зависимости от исходного дефицита объема циркулирующей крови. В первой группе он составил менее 15 %, во второй — свыше 15 %. В обеих группах проводилось мониторирование основных гемодинамических показателей. Время от начала болезни в первой группе — 11,28 ± 6,66 ч, во второй — 46,56 ± 30,63 ч. Оценка по шкале APACHE II составила 12,24 ± 1,96 балла в первой группе и 23,52 ± 1,82— во второй. Регидратация проводилась сбалансированными коллоидными и кристаллоидными препаратами стерофундин и тетраспан. В первой группе для регидратации применялись только кристаллоиды, во второй группе к кристаллоидам добавлялась инфузия коллоидных растворов. Основные гемодинамические показатели оценивали до проведения инфузионной терапии и по достижении центрального венозного давления (ЦВД) 100 мм вод.ст. или по истечении 180 минут после ее начала. Объем инфузионной терапии стерофундином в первой группе составил в среднем 1025,00 ± 435,22 мл, во второй группе объем инфузии стерофундина составил 1585,00 ± 359,31 мл, тетраспана — 615,00 ± 203,35 мл. Для статистического анализа использовались непараметрические критерии Манна — Уитни и Вилкоксона.У больных первой группы до начала инфузионной терапии определялось снижение ЦВД на 20 %. Нормальный уровень сердечного индекса (СИ) в этой группе не был достигнут даже за счет тахикардии; отмечалось снижение ударного индекса (УИ) почти в два раза. Поддержание артериального давления (АД) достигалось повышением удельного периферического сосудистого сопротивления (УПСС). Во второй группе сдвиги основных гемодинамических показателей носили более грубый характер, чем в первой. Вероятно, это связано с более продолжительным временем болезни в сравнении с первой группой (46,56 ± 30,63 и 11,28 ± 6,66 ч соответственно) и исходно более тяжелым состоянием пациентов (12,24 ± 1,96 балла по шкале в первой группе APACHE II и 23,52 ± 1,82 — во второй). ЦВД у 82 % исследуемых пациентов имело отрицательную величину, а у оставшихся 18 % не превышало 20 мм вод.ст. Частота сердечных сокращений (ЧСС) была увеличена на 74 %. УИ, СИ и среднее АД были снижены на 58, 28 и 20 % соответственно, а УПСС — повышено на 23 %. Увеличение ЧСС и УПСС, снижение среднего АД, СИ и УИ соответствовали признакам гиповолемического шока.После регидратации в первой группе ЦВД увеличилось на 50 % и не отличалось от контроля. УИ и СИ выросли на 51 и 26 % соответственно. Показатели ЧСС и УПСС снизились соответственно на 16 и 18 %. Уровень среднего АД нормализовался. Однако сохранялись достоверные различия параметров ЦВД, ЧСС и УИ в сравнении с контрольными величинами. Во второй группе ЦВД увеличилось до 90,74 ± 8,50 мм вод.ст. ЧСС и УПСС снизилось на 22 и 15 % соответственно (p < 0,05). Показатели УИ и СИ также в среднем повысились на 85 и 44 % (p < 0,05). Обнаружена достоверная разница ЦВД, ЧСС, УИ и срАД с показателями контрольной группы. Параметры СИ и УПСС статистически достоверных различий с контрольными показателями не имели. Большинство больных было успешно оперировано, и послеоперационный период протекал без осложнений. Пять пациентов второй группы умерли от присоединившихся хирургических осложнений.Проведенные исследования показали, что рационально проведенная предоперационная инфузионная терапия современными растворами у больных с острой кишечной непроходимостью способна нормализовать основные параметры центральной гемодинамики в большинстве случаев.
Исследована эффективность и безопасность препарата BCD-085 при бляшечном псориазе с псориатической ониходистрофией, применяя различные режимы введения. Лечение больных бляшечным псориазом с псориатической ониходистрофией не всегда позволяет достичь выраженного клинического эффекта. Учитывая это, продолжается изучение препаратов, селективно действующих не только на кожу, но и на ногти. Российской биотехнологической компанией «БИОКАД» разработан оригинальный генно-инженерный биологический препарат — BCD-085 — моноклональные антитела к интерлейкину-17А. Целью клинического исследования была оценка эффективности и безопасности различных режимов введения препарата BCD-085 у больных бляшечным псориазом с псориатической ониходистрофией. Работа является частью международного многоцентрового сравнительного рандомизированного двойного слепого плацебо-контролируемого клинического исследования эффективности и безопасности препарата BCD-085 при подкожном введении в разных дозах больным среднетяжелым и тяжёлым бляшечным псориазом. Пациентов рандомизировали в соотношении 1:1:1:1 в 4 группы: группа 1 получала препарат BCD-085 в дозе 40 мг, группа 2 — в дозе 80 мг, группа 3 — в дозе 120 мг и группа 4 получала плацебо. Введение BCD-085/плацебо проводили подкожно в день 1 на неделях 0, 1, 2, а затем в день 1 на неделях 4, 6, 8, 10. Эффективность терапии оценивали на 12 неделе по динамике индексов PASI и NAPSI. Доля пациентов, достигших PASI75 на 12-й неделе лечения, составила 92,7 % в группе 3 (120 мг BCD-085), 83,3 % в группе 2 (80 мг BCD-085), 80,0 % в группе 1 (40 мг BCD-085) и 23,08 % в группе 4 (плацебо) (p < 0,001). Улучшение состояния ногтевых пластинок по индексу NAPSI составило 73 % у пациентов в группе 3 (120 мг BCD-085), 43 % в группе 2 (80 мг BCD-085), 25 % в группе 1 (40 мг BCD-085) и 0 % в группе 4 (плацебо) (p < 0,05). Препарат BCD-085 показал высокую эффективность в лечении больных бляшечным псориазом с псориатической ониходистрофией.
