The prevalence of congenital ectopia lentis is 7–10 cases per 100 000 people. The most common causes of congenital lens displacement are the FBN1 gene mutations that have been found in 25–85 % of patients with this pathology. The aim of the study is to establish the FBN1 gene mutations in patients with congenital lens displacement and in their families. The study group included three families with children and adults suffered from the congenital lens dislocation. The nucleotide sequence of the FBN1 gene was analyzed by direct sequencing. The pathogenicity of the identified mutations was assessed using the Ghent criteria revised in 2010. The mutation c.1884C> G (p.Cys628Trp) in the heterozygous state in the 16th exon of the FBN1 gene was detected in proband 1 and her brother. Proband 2 was found to be a heterozygous career of the mutation c.2461T> A (p.Cys821Ser) in the 21st exon; this mutation was absent in parents and a healthy brother. The mutation c.7851delС (p.Cys2617Trpfs*65) in the heterozygous state in the 64th exon was identified in proband 3 and her mother. In accordance with the revised Ghent classification and the clinical manifestations and molecular genetic studies, Marfan’s syndrome (MS) was diagnosed in all probands and their affected relatives. We detected three pathogenic mutations not previously described in the literature in the 16th, 21st, and 64th exons of the FBN1 gene in patients with congenital ectopia lentis caused by MS. We established the spectrum of clinical manifestations of MS characteristic for the identified mutations.
Введение. Эктопия (подвывих, вывих) хрусталика – это смещение (дислокация) хрусталика с его естественного местоположения. Случаи эктопии хрусталика, не связанные с травмой, являются, как правило, генетически обусловленными и обозначаются термином «врожденный подвывих хрусталиков». Наиболее распространенной причиной врожденного смещения хрусталиков является синдром Марфана (OMIM 154700) – наследственное заболевание соединительной ткани, обусловленное мутациями гена FBN1. Подвывих хрусталика также может быть проявлением генетически детерминированных патологических состояний, ассоциированных с мутациями в генах ADAMTSL4, ADAMTS10, LTBP2, ADAMTS17, COL18A1, ADAMTS18, SUOX, PAX6, LOXL1.Цель исследования. Установить этиологию наследственного заболевания, манифестировавшего подвывихом хрусталиков, у двух сибсов из одной семьи.Материалы и методы. Пробанды – девочка 2 лет и мальчик 4 лет, родные сибсы, направлены на консультацию к генетику в связи с врожденным смещением хрусталика. Обоим пробандам и их матери было проведено прямое секвенирование 6-го экзона гена ADAMTSL4.Результаты и обсуждение. Оба пробанда родились в срок с нормальными показателями массы и длины тела. Мать обращала внимание на дрожание радужной оболочки у обоих детей с рождения, однако патология органа зрения при офтальмологическом обследовании была выявлена лишь в возрасте 2 (у дочери) и 4 лет (у сына). У девочки был установлен подвывих хрусталиков 1-й степени и сложный миопический астигматизм обоих глаз. У мальчика констатированы подвывих хрусталика 1–2-й степени, сложный миопический астигматизм и амблиопия средней степени правого глаза, а также сложный миопический астигматизм левого глаза. Признаков системного вовлечения соединительной ткани, патологических изменений при эхокардиографии у детей выявлено не было. В результате секвенирования 6-го экзона гена ADAMTSL4 у обоих пробандов обнаружена делеция c.826_836del11 (p.Arg276SerfsX21) в гомозиготном состоянии. У матери эта мутация выявлена в гетерозиготном состоянии.Заключение. Дислокация хрусталика, ассоциированная с повреждениями гена ADAMTSL4, является второй после синдрома Марфана частой причиной врожденного смещения хрусталика. В этой работе представлен первый в Республике Беларусь опыт молекулярно-генетической диагностики ADAMTSL4-ассоциированной офтальмологической патологии и впервые описаны пациенты – гомозиготные носители редкого варианта c.826_836del11 в генеADAMTSL4. Обследование на наличие мутаций в гене ADAMTSL4 следует рекомендовать всем пациентам с врожденным смещением хрусталика. Introduction. Ectopia lentis (EL) is a displacement or malposition of the eye’s crystalline lens from its normal location. In the absence of traumatic injury, the occurrence of EL is predominantly attributed to hereditary diseases. The most common cause of congenital EL is Marfan syndrome (OMIM 154700) – hereditary connective tissue disease associated with mutations in the FBN1 gene. The lens subluxation can also be a manifestation of genetically determined pathological conditions due to mutations in the ADAMTSL4, ADAMTS10, LTBP2, ADAMTS17, COL18A1, ADAMTS18, SUOX,PAX6, LOXL1 genes.Purpose of the study: to establish the etiology of a hereditary disease manifested by EL in two children in one family.Materials and methods. Exon 6 of the ADAMTSL4 gene was analyzed by direct DNA sequencing in two probands (girl 2 years old and boy 4 years old) and their mother.Results and discussion. Both proband were born at term with normal weight and length. Mother noticed iridodonesis at birth in both children. However, eye pathology was revealed during ophthalmologic examination only in the age of 2 (in daughter) and in the age of 4 (in son). The girl was diagnosed bilateral EL and complex myopic astigmatism. In the boy the right eye showed EL, complex myopic astigmatism and amblyopia and complex myopic astigmatism was found in the left eye. There were no systemic findings, cardiac ultrasound was normal, and there was no evidence for skeletal manifestations of a connective tissue disorder. Both patients were homozygous for frameshift mutation c.826_836del11 in ADAMTSL4. The mother was heterozygous for this variant. Conclusion. ADAMTSL4-associated isolated EL due to mutations in ADAMTSL4 gene is the second most common cause of congenital EL after Marfan syndrome. We performed the molecular genetic diagnosis of ADAMTSL4-associated ophthalmic pathology and described for the first time patients homozygous carriers of the rare variant c.826_836del11 in the ADAMTSL4 gene. Screening for mutations in the ADAMTSL4 gene should be recommended to all patients with congenital EL.
