In Ukraine, the problem of stroke, like other cardiovascular diseases, is extremely acute: it leads to 67 % of all deaths. The Ministry of Health of Ukraine notes that more than 140,000 of our compatriots are diagnosed with stroke every year; 30–40 % of patients die in the first 3 days and up to 50 % — within a year; 20–40 % of survivors become dependent on external assistance (12.5 % of primary disability). All over the world, measures that can prevent the occurrence of a stroke, and methods for rehabilitation of people who have already developed it are of great importance in terms of solving the problem of vascular disasters. This article provides scientifically based etiopathogenetic mechanisms of cerebral stroke (ischemic, hemorrhagic), its diagnosis (clinical, instrumental, laboratory), diagnostic criteria for early detection and early emergency (urgent) medical care. Examples of providing care in the pre-hospital period, inpatient (stroke centers) stage — primary, secondary and tertiary care are given. The importance of early rehabilitation and preventive measures is emphasized. It is indicated that it is important to act urgently and to focus efforts where the greatest effectiveness in stroke prevention can be achieved.
The aim: Research of the ways of adequate solving of problems with understuffed healthcare system and problems with continuous professional development of doctors. Materials and methods: We have used a official statistical data for the period from 2009-2020 years. This data was analyzed with usage of systemic approach and statistical epidemiological analysis. Results: According to the data from Ministry of Health of Ukraine during the first year of the pandemic (2020) the quantity of our doctors has diminished by 6.9 thousands, and quantity of nurses – by 22,5 thousands which is much bigger deficit then in period of 2018-2019 years. The insufficiency of pediatric doctors in Ukraine has reached 1000 vacancies, general physicians – 1700, family doctors – more than 3000 vacancies, surgeons – more than 950 vacancies. It is also important to involve medical experts and medical associations and unions in the process of accreditation and licensing of providers of continuous medical education. Conclusions: Existing deficit of medical personnel should be replenished by increasing of medical education of students in medical universities and colleges funded by government. Existing system of continuous professional development of doctors requires a lot of changes (additional activities, new technologies) which must be implied with the help of professional medical associations and unions.
Расстройства психики и поведения у детей являются актуальной проблемой современной педиатрической науки и практики. Высокие уровни распространенности этой патологиисвоеобразный маркер неблагоприятного морально-психологического состояния общества и влияния стресса в семье на неустойчивую психику ребенка. Нельзя также исключать и отрицательное влияние информационной среды, а также окружающей среды, в том числе радиационных факторов на организм ребенка. Целью исследования было изучение 25-летних трендов заболеваемости расстройствами психики и поведения детского населения страны Украины с целью определения влияния на их развитие факторов окружающей среды, в частности радиационного фактора, после аварии на ЧАЭС в 1986 году. Проведен анализ заболеваемости и распространенности расстройств психики и поведения (РПП) у детей из разных областей Украины, в том числе загрязненных в результате аварии на ЧАЭС. Применялись методы статистического оценивания, эпидемиологического анализа данных Центра медицинской статистики МЗ Украины по состоянию психического здоровья детского населения страны. Установлено, что в течение последних 25 лет наблюдалась достоверная динамика по уменьшению распространенности и заболеваемости расстройствами психики и поведения у детей Украины до 3,81 случаев на 1000 детей (p<0,01) с преимущественным выявлением этой патологии у детей из крупных промышленных и аграрных областей страны, где наблюдается значительное экотоксичное давление факторов внешней среды на детский организм. У детей, постоянно проживающих в областях страны, которые были загрязнены в результате аварии на ЧАЭС в 1986 году, особенно в Черниговской и Житомирской областях, показатели заболеваемости расстройствами психики и поведения в течение последних 25 лет превышали общегосударственные показатели и показатели заболеваемости РПП у детей из областей, не имевших территорий радиологического контроля. У детей, имеющих статус пострадавших вследствие аварии на ЧАЭС, показатель заболеваемости расстройствами психики и поведения был высоким в начале 90-х годов, но в течение последних 25 лет уровень заболеваемости РПП стал снижаться, и сейчас он меньше аналогичного показателя у детей, постоянно проживающих в областях страны, которые были загрязнены в результате аварии на ЧАЭС в 1986 году, и
The presented research is devoted to the definition of the interconnection of genetic and social risk factors depending on the time of the onset of acute ischemic stroke. The analysis of 150 patients with acute ischemic stroke confirms that patients with different incidence of stroke symptoms have a characteristic difference in the spectrum of social predictors. Sufficiently high concentration of pathogenic mutation of the TTH gene of MTHFR C7661T was observed only in the group of patients with the daytime period of stroke symptoms, it can be concluded that the social risk pattern of the first group of patients served as a characteristic trigger for activating the pathological effect of the mutated locus 7661 of the MTHFR gene, which is has led to the occurrence of ischemic stroke in 20.5 % of patients in this subgroup.
