In recent decades, novel microscopic methods commonly referred to as super- resolution microscopy have been developed. These methods enable the visualization of a cell with a resolution of up to 10 nm. The application of these methods is of great interest in studying the structure and function of the cell nucleus. The review describes the main achievements in this field.
<p><strong>Aim:</strong> The study was designed to investigate predictors of effective valve-sparing ascending aortic replacement in patients with Stanford type A aortic dissection combined with aortic insufficiency and to analyze efficacy and safety of this kind of surgery.<br /><strong>Methods:</strong> From January 2010 to December 2015, 49 patients with Stanford type A aortic dissection combined with aortic insufficiency underwent ascending aortic replacement. All patients were divided into 3 groups: valve-sparing procedures (group 1, n = 11), combined aortic valve and supracoronary ascending aortic replacement (group 2, n = 12), and Bentall procedure (group 3, n = 26). We assessed the initial status of patients, incidence of complications and efficacy of valve-sparing ascending aortic replacement.<br /><strong>Results:</strong> The hospital mortality rate was 8.2% (4/49 patients). The amount of surgical correction correlated with the initial diameter of the aorta at the level of the sinuses of Valsalva. During the hospital period, none of patients from group 1 developed aortic insufficiency exceeding Grade 2 and the vast majority of patients had trivial aortic regurgitation. The parameters of cardiopulmonary bypass, cross-clamp time and circulatory arrest time did not correlate with the initial size of the ascending aorta and aortic valve blood flow impairment, neither did they influence significantly the incidence and severity of neurological complications. The baseline size of the ascending aorta and degree of aortic regurgitation did not impact the course of the early hospital period.<br /><strong>Conclusions:</strong> Supracoronary ascending aortic replacement combined with aortic valve repair in ascending aortic dissection and aortic regurgitation is effective and safe. The initial size of the ascending aorta and aortic arch do not influence immediate results. The diameter of the aorta at the level of the sinuses of Valsalva and the condition of aortic valve leaflets could be considered as the limiting factors. Цель исследования-анализ случаев формирования стенозов проксимальных отделов левой коронарной артерии (ЛКА) после протезирования корня аорты. Материал и методы. В период с января 2012 г. по октябрь 2017 г. в нашей клинике было выполнено 302 операции протезирования корня аорты, в 5 случаях вмешательство осложнилось формированием стеноза в бассейне ЛКА. У всех 5 пациентов была аневризма корня и восходящего отдела аорты, аортальная недостаточность, дилатация левого желудочка (ЛЖ) без нарушения его функции. Исходно стенозы коронарных артерий отсутствовали. Результаты. В госпитальном периоде стеноз ствола ЛКА был выявлен у 3 больных (у 2-после операции Bentall-DeBono, у 1-после операции David I). У 1 больного отмечена окклюзия огибающей ветви ЛКА, поводом для выполнения коронарографии (КГ) стали повышение уровня маркеров повреждения миокарда, ишемические изменения ЭКГ и появление регионарных зон нарушения сократимости ЛЖ. В последнем случае проводилась консервативная терапия, в двух других-стентирование пораженного участка ЛКА. В четвертом случае после операции David I в связи с тяжелой сердечной недостаточностью (СН) проводилась поддержка гемодинамики с помощью временного обхода ЛЖ (система PLS, «Maquet») и внутриаортальной баллонной контрпульсации (ВАБК). После восстановления сократительной способности ЛЖ на 18-е сутки после операции ВАБК и обход ЛЖ были удалены. При МСКТ-КГ у данной пациентки был выявлен стеноз ствола ЛКА 50%; от дальнейших вмешательств пациентка отказалась. Через 3 года у данной больной появилась клиническая картина стенокардии; при КГ была выявлена субокклюзия устья ствола ЛКА. Последующее стентирование зоны стеноза прошло без особенностей. У пятого пациента после операции Bentall-De Bono госпитальный период протекал без особенностей; стенокардия напряжения у него появилась через 11 мес после операции. При КГ были выявлены субокклюзия ствола ЛКА и окклюзия в зоне устья передней межжелудочковой ветви ЛКА. Последующее эндоваскулярное лечение особенностей не имело. Заключение. При проведении операций на корне аорты, включающих выделение устьев КА, нарушения коронарного кровотока могут возникнуть вследствие излишнего выделения артерий, их перегибов, а также формирования стенозов вследствие внешних причин. Требующей углубленного изучения является возможность местной реакции на компоненты биоклея. Признаки ишемии миокарда и/или острой СН являются показаниями к незамедлительному визуализирующему исследованию КА. В течение 2 мес после протезирования корня аорты целесообразно выполнение МСКТ-ангиографии грудной аорты с оценкой зоны реконструкции и проксимальных отделов КА. Применение системы поддержки кровообращения является эффективным методом лечения тяжелой сердечной недостаточности. Background Aortic valve calcification associated with either bicuspid or tricuspid aortic valve remains among the most dangerous cardiovascular diseases but cellular and molecular reasons of this pathology are not fully understood. It has been shown that NOTCH1 haploinsufficiency in endothelial cells derived from iPS and also inhibition of NOTCH1 in aortic valve endothelial cells gives rise to the activation of proosteogenic pathways. Endothelial cells regulate underlying interstitial cells and govern them to differentiation. Thus, elucidation of molecular mechanisms of endothelial dysfunction could shed light on valve calcification pathogenesis. Purpose To elucidate key genes responsible for valve endothelial dysfunction in aortic valve calcification. Methods Primary human aortic valve endothelial cells (VEC) were isolated from 45 patients (BAV, n=21; TAV, n=26) with calcific aortic valve disease and from 8 healthy donors. Expression of corresponding genes was estimated by qPCR. Results We checked expression of about 30 genes related to Notch/Wnt/Bmp pathways and shear stress response. Endothelial cells of patients with calcification had significantly lower mRNA levels of Notch component genes NOTCH2, NOTCH3, DLL4 and JAG1 as well as of Notch target genes HEY1, SNAIL and SLUG. Expression of Wnt-related genes such as DKK1, GREM1 and TCF4 was altered in VEC from calcified valves. Interestingly, mRNA level of ROBO4, recently suggested to be involved in endothelial dysfunctions, was significantly higher in VEC of patients with calcification. Moreover, endothelium from calcified valves had impaired expression of matrix gla protein (MGP), which is known to sequester calcium and inhibit soft tissue calcification in the vasculature and aortic valve. Conclusions Our data show that Notch pathway genes are downregulated in valve endothelial cells of the patients with aortic valve calcification. Our results suggest also implication of shear stress responsive genes and genes of endothelial performance in changing the properties of valve endothelial population of calcified valves and thus presumably to the pathogenesis of aortic valve calcification.
