Цель исследования определить значимость параметров центральной гемодинамики для коллате- ральной компенсации нарушенного магистрального кровотока головного мозга при выполнении опе- рации каротидной эндартерэктомии (КЭАЭ). Материал и методы. Были обследованы 100 больных, которым выполнена операция КЭАЭ. Для за- щиты головного мозга от ишемии применяли системную артериальную гипертензию. Для оценки це- ребральной перфузии во время пережатия внутренней сонной артерии (ВСА) использовали измере- ние ретроградного давления в ВСА. В период пережатия прецеребральных сосудов осуществляли параллельный инвазивный контроль артериального и ретроградного давления. Рассчитывали индекс ретроградного давления. По величине индекса больные были разделены на три группы. Проведен анализ корреляции системного и ретроградного давления. Результаты. У 61 (61) пациента отмечена высокая связь между показателями ретроградного и си- стемного давления (степень корреляции 0,9830,995). У остальных 39 (39) больных была выявлена сниженная корреляция, тем не менее остававшаяся в пределах высокой степени зависимости (сте- пень корреляции 0,6730,886). При этом установлено достоверное различие значений системного артериального давления в группах с различным индексом ретроградного давления (р0,05). Выде- ленные группы достоверно различались по уровню ретроградного давления в ВСА (р0,05). Выводы. 1. Ретроградное давление в ВСА зависит от уровня системного АД, зависимость эта прямо пропорциональная и может быть высокой, средней и низкой. 2. При выполнении операции КЭАЭ уро- вень систолического ретроградного давления выше 40 мм рт. ст. может быть обеспечен посредством коррекции центральной гемодинамики на этапе пережатия внутренней сонной артерии.
Aim -to develop the method of surgical treatment of V1 and V2 segments occlusions or stenoses of vertebral arteries combined with V4 segment stenoses of vertebral arteries and basilar artery. Material and methods. A hybrid method was used in 4 patients with symptoms of vertebral-basilar insufficiency who had a combination of subtotal stenosis or occlusion of V1 and V2 segments of vertebral artery (extracranial lesion) with stenosis of V4 segment of vertebral artery and basilar artery (intracranial lesion). Results. Grafts and stents were patent in all patients during follow-up. There were no signs of restenoses. Volumetric flow rate through the graft exceeded 100 mL/min in all patients. Dynamic neurological examination showed persistent clinical improvement with reduction or disappearance of coordination and statics disorders, auditory and visual disturbances in all patients.
Objectives – to evaluate the efficacy of different types of risk reducing cardioprotection preceding the open surgery on the abdominal aorta.
Material and methods. The study analyses the treatment outcomes in 262 patients with abdominal aortic aneurysm. In all the patients the operation of prosthetics of abdominal aorta was performed. The patients were divided in two groups depending on the method of preoperative cardioprotection. The first group included 158 patients who received the intensive medicamentous preparation for the surgery. The second group of 104 patients additionally to drug treatment underwent the stress EchoCG; the coronarography was performed in patients with the positive test result. If the coronarography revealed the significant impairment of the coronary artery, the preliminary cardioprotective operations were performed: myocardial revascularization in 24 patients, CABG in 5 patients and coronary stenting in 19 cases.
Results.In spite of the intensive medicamentous preoperative preparation, in the first group the rate of myocardial infarction and acute coronary syndrome outcomes was significantly higher than in the second group of patients, who received the different preliminary myocardial revascularization according to individual requirements.
Conclusion.The individual approach to the choice of cardioprotection method before abdominal aortic aneurysm surgery significantly reduces the adverse cardiac events rate.
