Вступ. Варикозне розширення вен нижніх кінцівок (ВРВНК) є однією з найпоширеніших судинних патологій серед населення планети, що вражає до 50% популяції розвинених і країн, що розвиваються. Спектр медичної допомоги при варикозній хворобі обмежений компресійною терапією і варіантами оперативного лікування, спрямованими на оклюзію неспроможних сегментів підшкірної венозної системи, що включають термодеструкцію, склерооблітерація, скарифікацію, лігування і, найчастіше, комбінацію різних методів). Необхідність поєднання різних методів диктується варіативністю калібру і конфігурації судинного русла і неефективністю кожного з методів окремо щодо комплексного порушення венозної гемодинаміки. Двома принципово відмінними сценаріями пошкодження венозної макроциркуляції є висхідний і нисхідний варіанти, за даними літератури, з приблизно рівним розподілом і частим поєднанням між собою. Традиційні методи хірургічного лікування варикозної розширення вен ніг мають ряд принципових особливостей в умовах перфорантного типу хвороби. Мета дослідження. Порівняння ефективності та дослідження результатів використання мініінвазивих методик лікування у пацієнтів з хронічною венозною недостатністю, обумовленою варикозною хворобою нижніх кінцівок висхідного типу С2-6EpsAрPr класів. Дослідження динаміки якості життя пацієнтів з варикозною хворобою після проведеного комплексного лікування. Матеріали та методи. Дослідження базується на результатах аналізу 214 клінічних випадків комбінованого хірургічного лікування пацієнтів з висхідною формою варикозної хвороби на базі Університетської клініки (УК) ОНМедУ за період 2015–2019. Загальна вибірка була розділена на три групи: 1 – перша: група, в якій виконана транслюмінальна оклюзія перфорантів із застосуванням торцевого світловоду – 158 пацієнтів; 2 – друга: група, в якій в використано пінну склерооблітерація – 20 пацієнтів; 3 – третя: група з 36 осіб, яким виконано 60 лігувань перфорантів. Динаміка якості життя досліджувалася за допомогою стандартних протоколів шкали клінічної тяжкості венозної недостатності (Venous Clinical Severity Scope – VCSS). Результати досліджень та їх обговорення. Контроль лікування проводився за допомогою опитування та УЗ контролю апаратом інтраопераційно і через 7 днів, 1, 2, 6 місяців після операції. Як критерій якості проведеного втручання був обраний параметр повноти облітерації неспроможної перфорантної вени. Для результатів 1 і 2 групи визначено статистично достовірну різницю (p <0,001) на користь результатів 2 групи. Для результатів 1 і 3 групи підтверджена статистична значущість різниці (p <0,001) на користь результатів 3 групи. Для результатів 2 і 3 групи розрахована статистична перевага для 3 групи (р <0,05). За результатами загоєння трофічних виразок знайдена співставна ефективність досліджуваних способів лікування варикозної хвороби. Висновки. Транслюмінальна оклюзія перфорантів є найбільш ефективним малоінвазивним методом лікування горизонтального венозного рефлюксу у пацієнтів із висхідним варіантом варикозного розширення вен ніг. Комплексний підхід з індивідуальною оцінкою морфологічних особливостей організму пацієнта і анатомічного варіанту конфігурації венозної мережі є запорукою успішної оклюзії джерел венозного рефлюксу. Вступ. На сьогоднішній день варикозне розширення вен нижніх кінцівок є принципово важливим питанням в системі охорони здоров’я більшості країн світу через свою надзвичайну поширеність, латентному прогресуючого перебігу з важкими ускладненнями і життєво небезпечними наслідками. При розробці лікувальної тактики на сьогоднішній день перевага віддається транскутанним втручанням. Еталонним методом малоінвазивного лікування є ендовенозна лазерна коагуляція (ЕВЛК) з використанням діодних світловодів, яка характеризується високою результативністю при діаметрі неспроможного сегмента менше 20 мм і дозозалежним спектром ускладнень. Це обумовлює актуальність пошуку нового методу термальної абляції з меншим нагріванням тканин і більш досконалим алгоритмом управління. Мета дослідження. Порівняти ефективність методик ендовенозного електрозварювання та лазерної абляції в лікуванні хронічної венозної недостатності нижніх кінцівок. Матеріали та методи. Дослідження проведено на вибірці з 279 пацієнтів (178 жінок, 106 чоловіків у віковому діапазоні від 29 до 76 років) з хронічною венозною недостатністю нижніх кінцівок С2-6ЕрsАsрPr, яким була виконана ендовенозна абляція стовбурів підшкірних вен з діаметрами неспроможних сегментів від 14 до 28 мм. Контрольну групу склали 192 пацієнта, яким була проведена лазерна коагуляція стовбура великої підшкірної вени за допомогою діодного світловоду 1470 нм. У основній групі (67 пацієнтів) операція проводилася за допомогою ендовенозного електрозварювання. Ефективність лікування оцінювали з урахуванням об’єктивних та суб’єктивних даних. Результати досліджень та їх обговорення. Усі пацієнти проходили контрольну перевірку через 7 днів, 1 та 6 місяців, і потім кожні півроку. Фізикальне обстеження і кольорове дуплексне сканування оперованої кінцівки виконувалося для оцінки якості оклюзії БПВ і можливого рефлюксу і реканалізації. З метою контролю якості життя пацієнтів просили заповнити анкету CIVIQ через 1 і 6 місяців після операції. У результаті проведеного лікування через 6 місяців частота випадків часткової реканалізації стовбура великої підшкірної вени в 1 групі становила 4%, у другій – 0% (р> 0.05 за параметром χ2). У всіх випадках рецидиви в першій групі спостерігалися при діаметрі пригирлової ділянки судини вище 21 мм. У ранньому післяопераційному періоді з однаковою частотою спостерігалися випадки больового синдрому низької інтенсивності за ходом ВПВ. При аналізі якості життя пацієнтів за шкалою CIVIQ обидві методики є однаково ефективними. Метод ендовенозного електрозварювання (ЕВЕЗ) має більш простий принцип трансформації електричної енергії в теплову, а також можливість саморегулювання тривалості експозиції робочої поверхні електрода в просвіті вени, що дозволяє досягати оклюзії стовбурів підшкірних вен діаметром до 30 мм без появи ознак рецидиву варикозної хвороби протягом 6 місяців післяопераційного спостереження. Висновки. ЕВЕЗ є ефективним аналогом ЕВЛК з більш високим профілем безпеки і досконалим алгоритмом управління. Методика становить інтерес для подальшого наукового дослідження і практичного застосування. ЕВЕЗ дозволяє ефективно проводити транскутанну венектомію при варикозній хворобі нижніх кінцівок з діаметром неспроможного сегмента підшкірної вени від 20 до 30 мм при мінімальному спектрі ускладнень.
