Satisfaction with quality of life and self-attitude in patients operated by «traditional» (conventional surgical methods) and endoscopically assisted methods are considered in the article. Differences in the quality of life in patients, self-attitude to ones appearance are described. 65 patients were surgically operated and examined. Control group – patients operated by «traditional» techniques (35 patients), the average age of patients was 38 ± 11,1 years. The comparison group – patients operated using endoscopically assisted methods (30 patients), the average age of patients was 44 ± 17,7 years. Psychodiagnostic methods: 1. N. E. Vodopyanova`s scale of life quality; 2. The Short Form-36; 3. S.R. Panteleev`s Assessment of self-relationship. Complaints of paresthesia and pain experienced by patients who underwent a “traditional” operation were revealed as a result of the examination. Operated on with an endoscopically assisted method patients did not present any complaints. Differences between the assessment of the life quality and self-relationship in examined groups were found. Operated on with an endoscopically assisted method patients evaluated life quality, satisfaction with their appearance and physical condition higher than patients operated on with a “traditional” method.
The purpose of the research. To study prognostic significance of indicators of systemic inflammation of peripheral blood and relative indicators: neutrophil‑lymphocytic and platelet‑lymphocytic ratio, the course of squamous cell carcinoma of the oral mucosa and gastric adenocarcinoma.Materials and methods. Prospective analysis of patients with squamous cell carcinoma of the oral mucosa and disseminated gastric adenocarcinoma was performed. Patients with verified diagnosis, without signs of inflammatory diseases in anamnesis, not receiving antibacterial and immunomodulatory therapy were selected. Overall survival and survival without progression were considered as the main estimated parameters.Results. The selection criteria were met by 32 patients with disseminated gastric adenocarcinoma and 60 patients with squamous cell carcinoma of the oral mucosa. The prognostic value of relative indicators is determined: overall survival of patients with gastric adenocarcinoma with a low value of the neutrophil‑lymphocytic index is significantly higher than that of the rest of the cohort of patients: 16 months vs. 8 and 7 months (95 % CI (confidence interval) from 12 to 23 months, p=0.0382). Overall survival of patients with low platelet‑lymphocytic index was also higher: 16 months vs. 8 months (95 % CI from 11 to 24 months, р=0,0026). Different relapse‑free survival was noted in the group of patients with squamous cell head and neck cancer: patients with low index value it is 7 months vs. 2 months (95 % CI from 5 to 9 months, p=0.0499).Conclusions. The results show the possibility of using immunological microenvironment of the tumor and indices, characterizing the systemic inflammation, for prognosis of gastric adenocarcinoma and squamous cell carcinoma of the oral mucosa.
Relevance. Currently, there are several basic techniques for the dental implant surface structuring. Laser treatment is an extremely promising technique for the surface structuring. This technology allows creating regular implant surface without using chemicals and in just one technological step. The purpose was to present study aimed to compare and evaluate in vivo the stability and osseointegration of dental implants with 2 different surfaces structured by ytterbium-doped pulsed fiber laser operating at 1064 nm.Materials and methods. 60 dental implants were placed in the study. 2 types of dental implant surfaces, namely holes and parallel grooves, were created by the ytterbium laser operating at 1064 nm. A polished dental implant (without laser surface structuring) was also included in the experiment for comparison. The study was carried out on 15 laboratory animals (male rabbits, weight 3.5-4 kg). The implants were placed in the tibia. 4 implants with different surface types but of the same diameter and length were placed in each rabbit.Results. Laboratory animals were sacrificed 1.5 and 3 months after the surgery. The stability of the implants was assessed by RFA (Resonance Frequency Analysis), based on the registration of resonance electromagnetic oscillations of the implant and the surrounding bone when they are exposed to the electromagnetic field (Osstell ISQ). Also, nondecalcified bone blocks were histologically examined using a confocal laser scanning microscope (Carl Zeiss LSM 780) and histomorphometry was performed (BIC-index: Bone-to-implant contact). Bone blocks were prepared according to a special technique: they were soaked and embedded into the plastic and synthetic resin. The obtained blocks were cut into sections, 40-50 µm thick, and stained with toluidine blue.Conclusion. Laser surface structuring of the dental titanium implants is a promising technique. 59 in 60 (98.3%) implants were osseointegrated, there were no signs of inflammation in the bone tissue. The present results allow further studying of dental implants with various surface designs, structured by ytterbium laser.
