The combination of etiotropic antiviral drugs with Kagocel enhances the efficiency of antiviral therapy. Monitoring of antiviral cytokines during the treatment of influenza A is a convenient tool to verify the efficiency of antiviral therapy and needs to be more widely introduced into medical practice.
Резюме Цель. Установить степень повреждения ДНК лимфоцитов и определить уровень содержания 8-гидрокси-2дезоксигуанозина (8-OHdG) и 8-нитрогуанина (8-NO2G), общей антиоксидантной активности (ОАА) в сыворотке крови больных хроническими вирусными гепатитами С (ХВГС), В (ХВГВ). Материалы и методы. В исследовании приняли учас тие 100 человек с ХВГС, 50 с ХВГВ и 43 добровольца. Повреждения ДНК оценивали методом ДНК-комет. Уровень 8-NO2G и 8-OHdG определяли в образцах сыворотки крови с помощью наборов для ИФА. ОАА определяли по стандартной методике с ABTS. Статистическую обработку проводили с использованием пакета STATISTICA 10.0. Результаты. Отмечалась тенденция к увеличению уровня повреждений ДНК с прогрессированием стадии фиброза печени при ХВГВ и ХВГС. Обнаружены значимые отличия от показателя нормы 8-NO2G в группах F1, F2, F3 при ХВГС и в группах F2, F3, F4 (ρ<0,05) при ХВГВ. Во вех группах при ХВГС и в F2 и F4 при ХВГВ уровень 8-OHdG выше по сравнению с образцами контрольной группы (ρ=0,0001). Показаны корреляции между стадией фиброза печени и уровнем 8-NO2G и 8-OHdG соответственно (r=0,786620 и r=0,625844; ρ<0,05) при ХВГС и ХВГВ (r=0,573933 и r=0,478849; ρ<0,05). При исследовании ОАА у пациентов отмечалась тенденция к её истощению с развитием фибротических изменений, однако достоверные различия в сравнении с контролем были только на стадии фиброза F3 и F4 при ХВГС и F2, F4 (ρ<0,05) при ХВГВ. Заключение. Выявлен генотоксический эффект влияния вирусов гепатита В и С на ДНК лимфоцитов. Отмечено увеличение степени фрагментации ДНК с прогрессированием фиброза печени. Выявлена корреляция между стадией фиброза и биохимическими маркерами повреждения ДНК, а также показано декомпенсаторное снижение ОАА сыворотки на продвинутых стадиях фиброза печени.
The neuropeptides comprise an important part in the nervous system interacting with endocrine and immune systems. Peptide regulators are responsible for the continuity of communicating elements, which support homeostasis, however, despite abundant research examining neuropeptides, not all specific mechanisms and features of interacting proteins with cells and immune components have been uncovered. Objective: to perform a comprehensive assessment of neuropeptide system in patients with herpes zoster. Materials and methods: 106 in-hospital patients were examined diagnosed with herpes zoster within 2016–2019 period. Control group consisted of 30 healthy age- and sex-matched volunteers. Blood serum was collected after verifying diagnosis on day 1. After discharge, patients were monitored for signs of pain syndrome and overall state within 3 months. It allowed to divide patients into 3 groups retrospectively. Group 1 — patients with herpes zoster, accompanied by mild or moderate pain syndrome; group 2 — patients with herpes zoster, accompanied by severe pain; group 3 — patients with herpes zoster, complicated by postherpetic neuralgia. Level of serum protein s100B, myelin basic protein, nerve growth factor, brain-derived neurotrophic factor, neuron specific enolase was measured by using specific reagents purchased from “R&D Diagnostics Inc.” (США). Results. it was found that level of serum protein S100B in all groups was significantly increased compared to control group, showing no inter-group differences. Amount of myelin basic protein in all study groups vs. control was significantly higher. Moreover, level of these parameters in group 2 vs. group 1 and 3 was significantly elevated. In addition, level of nerve growth factor was significantly increased in group 1 vs. groups 2 and 3, whereas in group 3 it was significantly lower than in control and group 2. Brain-derived neurotrophic factor was significantly decreased in all the study groups compared to control, showing no significant intergroup differences. Level of neuron-specific enolase was significantly increased in group 3 vs. control as well as group 1 and 2. The data obtained allowed to identify two parameters for assessing a risk of postherpetic neuralgia in acute herpes zoster, as well as provided deeper insights into the pathogenesis of neuroimmune disorders accompanying herpes zoster.
Data on identification and verification of the etiologically diagnosis of a acute respiratory viral infections are presented in article at sharp upper respiratory tract infections of the multiple and not specified localization and at local diseases of respiratory organs in Vladivostok. The highest average value of detectability from 2011 for 2017 was at a rinoviral infection - 22,8 cases %ооо. The detectability of flu A (H3N2) was 19,6%ooo; flu A (H1N1) - 15,1%ooo; flu B - 9,8%ooo. Detection of the acute respiratory viral infections verified on an etiology had natural seasonality. The incidence of flu, caused by representatives of the Orthomyxoviridae family, was formed in the winter and in the spring. During the summer-autumnal period rise in incidence of Paramyxoviridae family viruses was formed (paraflu, RS-viral infection, a metapneumoviral infection). The Coronaviral infection had winter seasonality. The Rinoviral infection, adenoviral and bakaviral infections were characterized by summer-autumnal seasonality. According to the nature of seasonality and structure of the verified cases on months of year the probability of confirmation of the diagnosis of a rinoviral infection from May to October was from 56,8+5,1% to 62,1+7,5%. Similar situation develops with paraflu. From May to November the specific weight of the verified paraflu was from 11,6+4,9% to 19,5+4,9% of all confirmed SARS cases. For flu A (H1N1) the probability of confirmation of the diagnosis from December to March is from 15,4+3,9% to 54,8+5,3%. And at flu A (H3N2) in data of month - from 5,2+2,4% to 47,6+4,0%. At flu B high probability of confirmation of the diagnosis in spring months, in March and April, 30,0+3,6% and 26,4+5,2%.
