На сегодняшний день одной из актуальных проблем здравоохранения остается пандемия COVID-19, негативно влияющая на здоровье населения во всем мире, в связи с чем необходима координация международных ответных мер с учетом рекомендуемых стратегий выживания и осознанной осведомленности, что обеспечит высокую сопротивляемость неблагоприятным воздействиям пандемии и снижению риска развития психопатологических симптомов. Во всех странах постояннопроводятся исследования воздействия пандемии на психическое здоровье населения. Выявлено, что социальная изоляция, смерть родственников, неизвестность будущего, страх перед болезнью в совокупности являются факторами риска развития психогенных расстройств, а также ухудшения психического состояния при уже существующих расстройствах. Ограничительные меры, связанные с COVID-19, повлияли на модель потребления психоактивных веществ в большинстве стран мира, что может привести к дополнительной угрозе психическому и физическому здоровью населения. Цель: изучить связь влияния стрессовых факторов на формирование психогенных расстройств, таких как тревога, депрессия, посттравматическое стрессовое расстройство, у лиц с синдромом зависимости от психоактивных веществ в период пандемии.Материал. Результаты анкетирования пациентов (n=86) мужского и женского пола в возрасте от 20 до 60 лет, проходивших стационарное лечение в Алтайском краевом наркологическом диспансере (Барнаул) в феврале – сентябре 2021 г. Методы. Миссисипская шкала посттравматического стрессового расстройства, шкалы тревоги и депрессии Бека. Статистические расчеты производились с использованием программ Microsoft Office Excel 2013, EpiInfo7.2. Заключение. Около трети (29,7%) исследуемых испытывали негативное психологическое воздействие пандемии COVID-19. Травматический стресс вызван высокой интенсивностью переживаний из-за угрозы заражения, страхом тяжелых последствий и смертельного исхода, деструктивным воздействием средств массовой информации на общественное сознание без доказательных аргументов. У обследуемых, испытывающих страх летального исхода и убежденных в разрушительном воздействии COVID-19 на физическое и психическое здоровье, чаще встречаются проявления тревоги, депрессии, психогенных расстройств. Так как лица с ПТСР более других склонны к разным дезадаптивным формам поведения, они в первую очередь нуждаются в социальной и медико-психологической поддержке. Наркологические больные относятся к группе высокого риска по развитию психогенных психических расстройств в период пандемии COVID-19, что необходимо учитывать при проведении комплексных лечебно-реабилитационных мероприятий.
После 20 лет снижения эпидемиологических показателей, отражающих распространение туберкулеза, в 90-е гг. XX века в России произошло ухудшение эпидемической ситуации. Важной проблемой является лечение больных с сочетанием туберкулеза легких и психического заболевания. Такие больные, как правило, уклоняются от лечения туберкулеза, нарушают режим противотуберкулезных учреждений. Вопросы комплексной терапии сочетанной патологии до сих пор окончательно не разработаны, являются предметом исследований. Исследование выполнено на базе наркологического туберкулезного отделения, функционирующего в Алтайском крае с 2015 г. Показан рост удельного веса лиц с синдромом зависимости от алкоголя в выборке пациентов, страдающих туберкулезом легких. В ходе исследования выявлены основные клинико-динамические закономерности сочетания психических заболеваний и туберкулеза легких.
