Syncope, defined as a transient loss of consciousness caused by transient global cerebral hypoperfusion, affects 30–40% of humans during their lifetime. Vasovagal syncope (VVS) is the most common cause of syncope, the etiology of which is still unclear. This review summarizes data on the genetics of VVS, describing the inheritance pattern of the disorder, candidate gene association studies and genome-wide studies. According to this evidence, VVS is a complex disorder, which can be caused by the interplay between genetic factors, whose contribution varies from monogenic Mendelian inheritance to polygenic inherited predisposition, and external factors affecting the monogenic (resulting in incomplete penetrance) and polygenic syncope types.
Vasovagal syncope (VVS) is the most common cause of sudden loss of consciousness. VVS results from cerebral hypoperfusion, due to abnormal autonomic control of blood circulation, leading to arterial hypotension. It is a complex disease, and its development is largely associated with genetic susceptibility. Since abnormal neurohumoral regulation plays an important role in VVS development, we analyzed the association of VVS with polymorphic variants of ADRA1A , ADRB1 , HTR1A , ADORA2A , COMT , and NOS3 genes, the products of which are involved in neurohumoral signaling, in patients with a confirmed VVS diagnosis (157 subjects) and individuals without a history of syncope (161 subjects). We were able to identify the associations between VVS and alleles/genotypes ADRA1A rs1048101, ADRB1 rs1801253, ADORA2A rs5751876, and COMT rs4680, as well as NOS3 rs2070744 in biallelic combination with COMT rs4680. Thus, we are the first to observe, within a single study, the role of the genes that encode α- and β-adrenergic receptors, catechol-O-methyltransferase, adenosine receptors and nitric oxide synthase in VVS development. These findings demonstrate that the genes involved in neurohumoral signaling pathways contribute to the formation of a genetic susceptibility to VVS.
Цель. Оценка результатов использования CPAP-терапии (создание постоян-ного положительного давления воздуха в верхних дыхательных путях) у боль-ных с синдромом обструктивного апноэ во время сна (СОАС) и фибрилляцией предсердий (ФП) в сопоставлении с эффективностью их лекарственного анти-аритмического лечения. Материал и методы. В исследование включены 36 больных (58% мужчин, средний возраст 61±11 лет) с ФП и диагностированным СОАС согласно результатам кардиореспираторного мониторирования. У 6 (17%) пациентов выявлена среднетяжелая степень дыхательных нарушений и у 30 (83%) -тяжелая степень. 24 (67%) больных страдали пароксизмальной и 12 (33%) -персистирующей формой ФП. Всем больным с целью поддержания синусо-вого ритма и профилактики рецидивов ФП была подобрана лекарственная антиаритмическая терапия (ААТ). CPAP-терапия была рекомендована всем пациентам, но после пробного (в течение 1-3 ночей) курса лечения 18 больных согласились на продолжение этого вида терапии в домашних условиях (пер-вая группа), другие 18 пациентов (вторая группа) не смогли пользоваться CPAP-аппаратами из-за плохой переносимости. По результатам 12-ти месяч-ного наблюдения был проведен сравнительный анализ эффективности ААТ в двух группах больных. Группы статистически значимо не различались по при-менявшимся антиаритмическим препаратам. Результаты. На фоне CPAP-терапии в первой группе у большинства пациен-тов -12 (67%) в течение 12 месяцев не наблюдалось рецидивов ФП, в то время как, во второй группе только 6 (33%) больных, не имели ФП. Сле-дует отметить, что больные первой группы потенциально имели меньше шан-сов на успех антиаритмической терапии, исходя из традиционного анализа факторов риска развития ФП, поскольку у них наблюдались более выраженное ожирение (индекс массы тела 39,5 кг/м 2 против 34, р=0,07), более тяжелые дыхательные нарушения во время сна (индекс апноэ-гипопноэ сна 50,8 соб./час против 37,4, р=0,09) и больший объем левого предсердия (82 мл против 60,5, р=0,002) по сравнению с пациентами из второй группы. На фоне 3-х месячного применения CPAP-терапии у больных первой группы отмечено достоверное уменьшение дисперсии Р-волны на 30,8% (р=0,05). Заключение. Проведение CPAP-терапии у больных со среднетяжелой и тяже-лой степенью СОАС позволяет более чем в 2 раза (с 33% до 67%) повысить эффективность антиаритмического лекарственного лечения, направленного на профилактику рецидивов ФП. Одним из механизмов антиаритмического действия CPAP-терапии у больных с ФП может являться уменьшение диспер-сии времени предсердного проведения.
