Лечение рецидива рака гортани остается клинически сложной задачей. Перистомальный рецидив-одно из тяжелых осложнений рака гортани, встречается у 2-15 % пациентов после ларингэктомии, имеет неблагоприятный прогноз: 2-летняя выживаемость составляет 45 % у пациентов с I и II стадиями рецидива (по G. A. Sisson) и 9,0 % у пациентов с III и IV стадиями. Развитие стомального рецидива происходит в течение 2 лет после оперативного вмешательства. На его развитие влияют такие главные факторы, как расположение опухоли в подскладочном отделе гортани, размер первичной опухоли, наличие регионарных метастазов, наложенная до ларингэктомии трахеостома, подслизистое распространение опухоли на трахею. Это необходимо учитывать при назначении адъювантной терапии и выборе лечения у первичных пациентов с учетом патогенетических механизмов развития стомального рецидива. При многообразии оценок клинической картины и факторов формирования перистомального рецидива при раке гортани требуется современная оценка с учетом комплексного подхода к лечению основного заболевания с целью достижения низкого уровня развития стомального рецидива.
Aim of the study — to provide for medical oncologists and surgeon, who specialize in treatment of head and neck oncology patients, the most recent information about incidence and feature of oral squamous cell carcinoma (OSCC) in young patients.Material and мethods. Pubmed, ScienceDirect are used for literature search. Data of world literature published since 1983 till 2021 and contain information about incidence, epidemiology, clinic, treatment and prognosis in OSCC in young patients. The data of retrospective and prospective clinical trials were studied.Results. Determined the relevance of this problem and described incidence and epidemiology. Based on the analysis of the most recent publications, the authors provided data on clinical picture and modern approaches to the treatment and prognosis of OSCC in young patients by comparison with old patients.Conclusion. Over the past 30 years, the incidence of oral squamous cell carcinoma in young patients has been increasing worldwide while the incidence of classical oral cancer has been decreasing and no clear etiology has been identified. In this review we analyzed and synthesized the literature regarding early onset OSCC. Since 1983, the first assumptions about the uniqueness of the etiology and clinical picture of OSCC in young patients have appeared; till this day, this topic remains relevant for head and neck oncologists. Understanding the biology of the OSCC in young patients can be crucial in determining the prevention of disease, treatment and prognosis for patients.
Злокачественные опухоли головы и шеи в структу-ре общей онкологической заболеваемости занимают менее 5 %. При этом на III-IV стадии плоскоклеточно-го рака головы и шеи приходится более 60 % случаев [1,2]. Из-за сложного анатомического строения аэро-дигестивного тракта рост опухоли зачастую приводит к нарушению приёма пищи и развитию дисфагии, что требует дополнительного лечения и коррекции. Хро-ническое недоедание наблюдается приблизительно у 20-57 % пациентов с плоскоклеточным раком головы и шеи [3]. Помимо этого, нарушение приёма пищи возникает в результате комбинированного лечения, и уровень нутритивного статуса прямо влияет на переносимость лечения и, как следствие, на его эффективность. Адекватное энтеральное питание является одним из важных компонентов терапии сопровождения при проведении лечения больных с плоскоклеточным раком головы и шеи. Одним из воз-можных вариантов современного лечения дисфагии и коррекции нутритивного статуса у онкологических пациентов является чрескожное эндоскопическое наложение гастростомы (ЧЭГ). This review analyzes the research data concerning the problem of antibiotic prophylaxis (ABP) of wound infections after various surgeries for head and neck tumors. In patients with clean wounds, ABP should be used in exceptional cases only (for example, if the patient has any risk factors), otherwise it should be avoided. A short ABP course is recommended for patients with clean-contaminated wounds; however highrisk patients may require a prolonged course. There is some evidence of ABP efficacy in patients with non-contaminated wounds after cervical lymphadenectomy. When choosing a drug for ABP, a doctor should consider the site of surgery and the risk of wound contamination. The optimal drugs after head and neck surgeries include first- and second-generation cephalosporins, ampicillin in combination with sulbactam, metronidazole, and clindamycin. First- and second-generation cephalosporins in combination with metronidazole are preferable, but if the wound is infected with gram-positive bacteria, it is necessary to use clindamycin monotherapy. Reconstructive surgeries with a free flap require a short course of ABP with one of the following combinations: cefazolin + metronidazole, cefuroxime + metronidazole, or ampicillin + sulbactam; if the patient is allergic to beta-lactams, clindamycin can be used. Despite the availability of standard ABP regimens, a surgeon must apply a tailored approach when choosing an ABP regimen for each patient, taking into account risk factors and the volume of surgery.
