Relevance. Vaccine prophylaxis is the most cost-effective and affordable means of controlling infectious diseases. At the same time, there is a great regional diversity in the number of people who refuse vaccination. In our country, according to several large studies, there is a relatively low adherence to vaccination compared to other European countries. It is common to have doubts and questions about immunization in adult patients or parents who vaccinate their children. A decrease in vaccination coverage of the population can lead to an increase in the incidence of infections preventable by immunization. At the same time, measures to promote vaccination used by preventive health care systems in various countries are insufficient. This increases the likelihood for doctors of various specialties to meet in their daily activities with patients' questions and concerns about vaccination.The purpose of this work was to highlight the practical aspects of building a dialogue with patients who have doubts about vaccination. Conclusions. Successful communication is based on the doctor's ability to build a confidential dialogue based on confidence in the decency and goodwill of all its participants. Based on the study, the following conclusions can be drawn. An alarming trend in recent years is the increasing number of patients who doubt the effectiveness of vaccination. For hesitant patients, the doctor is one of the most important sources of information about vaccines. The doctor's ability to clearly and confidently build a dialogue about vaccination helps to dispel the patient's doubts and is the most effective means of increasing adherence to immunization of the population.
Клинико-иммунологическая эффективность отечественного иммунотропного препарата у детей при острых респираторных инфекциях с бронхообструктивным синдромомФедеральное государственное бюджетное учреждение «Детский научно-клинический центр инфекционных болезней» Федерального медико-биологического агентства, 197022, г. Санкт-Петербург, Российская Федерация Резюме Введение. Частота развития бронхообструктивного синдрома (БОС) на фоне острых респираторных инфекций (ОРИ) у детей раннего возраста остается высокой. У этой группы детей, по данным ранее проведенных исследований, выявлен сниженный иммунный ответ на инфекцию. Таким образом, целесообразно использование иммуномодулирующей терапии у детей с бронхообструктивным синдромом при ОРИ.Цель исследования -оценить клиническую и иммунологическую эф фективность применения препарата альфа-глутамил-триптофан спрей у детей с БОС при ОРИ.Материал и методы. В статье представлены результаты рандомизированного двойного слепого плацебо-контролируемого исследования применения препарата альфа-глутамил-триптофан спрей у детей с БОС при ОРИ, проведенного в инфекционно-пульмонологическом отделении СПБ ГБУЗ «ДГБ Св. Ольги» с разрешения этического комитета ДГБ. В исследовании приняли участие дети в возрасте до 6 лет с наличием 2 и более эпизодов БОС на фоне ОРИ. Альфа-глутамил-триптофан назначали после купирования острой картины заболевания в течение 5 дней в виде спрея в дозе 25 мкг детям до 3 лет и 50 мкг -старше 3 лет. Клиническую эффективность оценивали на основании сравнения исходных клинико-анамнестических данных с данными катамнеза через 6 мес после терапии. Критериями оценки были частота и особенности течения ОРИ и БОС, количество госпитализаций. Иммунологическое исследование проводили дважды: в острый период заболевания и не ранее 18-го дня после приема препарата или плацебо. Определялось абсолютное и относительное количество лимфоцитов и их субпопуляций CD3 + , CD4 + , CD8 + (Т-лимфоциты); CD19 + (В-лимфоциты). Также оценивались функциональная активность полиморфно-ядерных нейтрофилов, фагоцитарный показатель и фагоцитарное число, показатель завершенности фагоцитоза.Результаты. Использование альфа-глутамил-триптофана интраназально в виде спрея в комплексной терапии детей с БОС приводило к сокращению частоты и длительности заболеваний ОРИ, способствовало уменьшению эпизодов БОС на фоне ОРИ и улучшению иммунологических параметров, заключающихся в нормализации уровня CD4 + -Тклеток, нормализации соотношения CD4 + /СD8 + , снижению уровня провоспалительного цитокина ФНОα.Заключение. Выявлено положительное влияние препарата на частоту, тяжесть и длительность ОРИ у детей с БОС. Благоприятные изменения иммунологических показателей позволяют использовать альфа-глутамил-триптофан спрей в комплексной терапии ОРИ у детей с БОС и для профилактики их рецидивирующего течения. The analysis of safety and efficiency of vaccination of the children born before the term (61 children of 1 – 4 degree of prematurity) and children born in time (21 children – group of comparison). For vaccination and revaccination was used: DTP (20 and 15 children respectively), DTaP (22 and 4 children), pentavalentny vaccine (DTaP/IPV/Hib – 9 and 2 children) and DT (10 children of the studied group with convulsion in the anamnesis). Against measles, mumps and a rubella was immunized 47 and 20 children respectively. After immunization all clinical symptoms was analyzed and after 2 months diphtheria measles and mumps antibodies was detected.Prematurely born children has the 1 vaccination for 3 – 4 months later, then infants, with born at time. After vaccination 75.4% and 74.5% infants in both groups hadn’t any clinical symptoms. Acute respiratory infection was registered with the same frequency (25,5 – 25.0% and 24.6 – 23.8%). The antibodies titer to diphtheria, measles, mumps didn't depend on degree of prematurity.
