ЦЕЛЬ: оценить значение метаболических параметров, ассоциированных с развитием отдаленных не- благоприятных сердечно-сосудистых событий (НССС) у больных сахарным диабетом 2 типа (СД2). МАТЕРИАЛЫ И МЕТОДЫ: обследовано 94 больных СД2 (средний возраст 55,3±5,5 лет; 65% женщин). У всех больных СД2 была гипертоническая болезнь, избыток массы тела или ожирение, у 37 пациентов (39,4%) - стабильная ишемическая болезнь сердца. Критерии исключения: наличие клинически выра- женной хронической сердечной недостаточности (ХСН), фракции выброса левого желудочка <50%, хро- нической болезни почек 3-5 стадий, дыхательной недостаточности, анемии. Больным СД2 исследовали уровень гликированного гемоглобина (HbA1c), С-пептида, рассчитывали индекс триглицериды (ТГ)/глюкоза плазмы натощак. Вариабельность гликемии определяли на основании измерения глюкозы крови 3-4 раза в сутки на протяжении не менее 3-х дней, рассчитывая коэффициент вариации (CV) гликемии. С помощью эхокардиографии оценивали структурные и функциональные изменения сердца. Толерантность к физиче- ской нагрузке (ТФН) у больных СД2 определяли на основании жалоб на одышку, усталость. НССС (смерть от любых причин, инфаркт миокарда, экстренную реваскуляризацию миокарда, госпитализации по пово- ду декомпенсации ХСН, ишемический инсульт) анализировали через 8,8±0,7 лет. Методом логистической регрессии выявляли наиболее значимую комбинацию исходных факторов, взаимосвязанных с наличием отдаленных НССС у больных СД2. РЕЗУЛЬТАТЫ: НССС выявлены у 34 (38,6%) больных СД2 (n=88) через 8,8±0,7 лет: 12 летальных исходов (10 – от сердечно-сосудистых заболеваний), 5 случаев инфаркта миокарда, 9 – экстренной реваскуляри- зации миокарда, 4 – госпитализации в связи с декомпенсацией ХСН, 4 - ишемического инсульта. Больные СД2 с НССС (n=34) составили 1-ую группу, больные СД2 без НССС (n=54) – 2-ую группу. Исходно у больных 1-ой группы по сравнению с больными 2-ой группы был выше CV гликемии (28,2% (20,7;32,3) vs 18,8% (13,8;22,7), p<0,01), больше – размер левого предсердия [ЛП] (4,5±0,6 см vs 4,2±0,5 см, р=0,04), чаще встре- чалось снижение ТФН (61,8% vs 27,8%, р<0,01). Не выявлено различий между больными 1-ой и 2-ой груп- пы по исходному уровню HbA1c (10,4% (8,6;11,7) vs 9,2% (7,9;11,1), р=0,1), С-пептида (1,9 нг/мл (1,5;2,9) vs 2,5 нг/мл (1,6;3,6), р=0,3), индексу ТГ/глюкоза (5,1 (4,9;5,4) vs 5,1 (4,8;5,3), р=0,4), а также по частоте терапии инсулином (67,6% vs 53,7%, р=0,3), препаратами сульфонилмочевины [СМ] (17,6% vs 31,5%, р=0,1), метфор- мином [МФ] (79,4% vs 85,2%, р=0,5) и по суточной дозе инсулина (0,4 ЕД/кг массы тела (0,2;0,5) vs 0,4 ЕД/кг массы тела (0,4;0,5), р=0,8). Но исходно CV гликемии был выше у больных СД2 на инсулинотерапии - 22,7% (19,3;31,7), чем у больных СД2 на терапии препаратами СМ - 18,3% (14,8;24,6) и монотерапии МФ - 14,8% (9,8;16,9), р<0,01. По данным логистической регрессии, выявлена ассоциация отдаленных НССС у больных СД2 с исходным увеличением CV гликемии (SE=-0,62, p<0,01), увеличением размера ЛП (SE=-0,42, p=0,02) и наличием жалоб на снижение ТФН (SE=-0,36, p=0,03) с процентом конкордации 87,8%. ВЫВОДЫ: метаболическим фактором, взаимосвязанным с развитием отдаленных НССС у больных СД2, является исходное повышение CV гликемии, особенно в сочетании с увеличением размера ЛП и наличием жалоб на снижение ТФН.
Aim. Atherosclerosis is one of the most important complications of type 2 diabetes mellitus (T2DM). In the present study, we aimed to assess the carotid intima-media thickness in type 2 diabetes patients with the arterial hypertension (AH) comorbidity. Materials and Methods. 96 patients with T2DM and AH (30M/66F, aged 53.4?5.4 years, duration of diabetes 7.7?7.5 years) and 30 hypertensive patients without diabetes (12M/18F, aged 53.1?6.0 years) were enrolled in this study. Stiffness indices for right and left common carotid arteries were assessed with noninvasive ultrasound method in both groups. Results. Как следствие, биопсия предстательной железы выполняется часто лишь на основании повышенного уровня простат-специфического антигена и данных пальцевого ректального исследования, что далеко не всегда позволяет дифференцировать суспициозные участки, и забор материала производится из стандартных зон. Существует альтернатива - биопсия под контролем МРТ, однако присущие временные и финансовые затраты накладывают существенные ограничения на широкое использование данной методики. Внедрение новых биомаркёров позволит создать персонализированный подход в диагностике, лечении и прогнозировании течения рака предстательной железы у каждого конкретного пациента. Fast development of laboratory diagnostic methods has allowed introducing various biomarkers for detection of prostate cancer to clinical practice. However, difficulties in visualization of tumor foci remain one of the most urgent problems. As a result, prostatic biopsy is often performed only on a basis of elevated levels of prostate-specific antigen and data of rectal examination. These tests do not always allow to differentiate suspicious areas, and the material is taken from standard zones. An existing alternative is MRI-guided biopsy, but the related time and financial costs impose significant limitations on extensive use of this technique. Implementation of new biomarkers will create an individualized approach to diagnostics, treatment and prognosis of prostate cancer for each patient.
Background and Aims Lupus nephritis is a common part of systemic lupus erythematosus (SLE), severe autoimmune disease that affects multiple organ systems and associated with pure prognosis. Patients with lupus nephritis who experience persisted disease activity despite conventional immunosuppression are at high risk of early death. Re-setting of the immune system and self-tolerance by high-dose immunosuppressive therapy with autologous stem cell transplantation (ASCT) is a new approach in the treatment of refractory SLE. Remote outcomes of this method and effectiveness relapse treatment are still unclear and were the aims of this study. Method We report a sick woman 39 years old, with refractory severe lupus nephritis. Standard therapy was ineffective, SLEDAI score remains 22 so patient was underwent high dose immunosuppressive therapy with ASCT and included in European Group for Blood and Marrow Transplantation European League against Rheumatism (EBMT/EULAR) registry on 53 pts who received ASCT for SLE between 1996 and 2005. Ethical approval was obtained for this study from Cambridge University Hospital Ethics Committee. Results The ASCT induced complete clinical and serological remission (SLEDAI score was 0) for 3 years. Than relapse with nephritic syndrome and anti-dsDNA positivity occurred and caused renal failure, creatinine clearance decreased to 21. Despite therapy including prednisolone, 45 mg daily, and mycophenolate mofetil, 2000 mg daily, disease activity persisted, creatinine clearance remained decreasing and 3 years later became 15 ml/min. During 1 year patient was on regular hemodialysis, then renal transplantation performed. Now, follow up is 15 years after ASCT and 8 years after kidney transplantation. Patient received a standard post-transplant immunosuppression with prednisolone, 5 mg daily, tacrolimus, 2.5 mg daily, and azathioprine, 100 mg daily, and her conditions remains stable, she has functioning renal allograft, SLEDAI score is 2 (anti-dsDNA positivity in low titer). Conclusion We present the successful remote outcomes of first case of immunoablation and double transplantation of autologous stem cells and allogeneic kidney in severe refractory to conventional immunosuppression SLE with lupus nephritis.
