Б. Богаткова, 175/1 Аннотация Цель: изучение взаимосвязей отношения к курению и диагностике сердечно-сосудистых заболеваний (ССЗ) в открытой городской популяции трудоспособного возраста. Материал и методы. Проведено два одномоментных эпидемиологических исследования мужской и женской открытых популяций трудоспособного возраста 25-64 лет в количестве 1553 человек: 850 лиц мужского пола и 704 лиц женского пола. Формирование репрезентативных выборок проводилось с использованием метода случайных чисел из избирательных списков лиц обоего пола Центрального административного округа Тюмени объемом 1000 человек каждая, отклик на исследование составил 77,7%: среди мужчин-85,0% и среди женщин-70,4%. Отношение респондентов к здоровью в части отношения к диагностике ССЗ и курению оценивалось по результатам опроса по анкете международного проекта МОНИКА-психосоциальная «Знание и отношение к своему здоровью», которая заполнялась в формате самозаполнения. Проведен статистический анализ с применением программ STATISTICA 12.0, значение р < 0,05 считалось статистически значимым. Результаты. Тюменская популяция 25-64 лет характеризуется высокой распространенностью курения с существенным преобладанием показателя в мужской выборке относительно женской (47,0 vs 14,8%, р < 0,001). Результаты проведенного анализа показали, что в популяции по отношению к диагностике ССЗ около ¼ респондентов преимущественно доверяются своему самочувствию, тогда как ¾ респондентов доверяют врачу, при этом практически половина из них придерживаются мнения в пользу тщательного исследования специалистами. Отмечены гендерные особенности ассоциаций отношения к диагностике ССЗ и к курению. Для мужчин, никогда не пытавшихся бросить курить, более свойственно доверяться своему самочувствию (41,8%) и, напротив, для категории никогда не куривших или бросивших курить в большей мере присуще доверие осмотру врача как достаточному методу для диагностики ССЗ (44,4 и 42,0% соответственно). В свою очередь, в женской выборке в аналогичных категориях по отношению к курению респонденты отдавали предпочтение 3-му утверждению, которое предполагало помимо осмотра врача тщательное обследование специалистами (83,3; 45,4 и 50,7% соответственно), p < 0,05. Установлена сопряженность отношения к диагностике ССЗ с распространенностью курения у мужчин и женщин (р < 0,001). Дисперсионный анализ Краскела-Уоллиса и медианный тест показали, что в обследованной популяции заинтересованы в диагностике ССЗ преимущественно некурящие респонденты. Заключение. Установленные взаимосвязи между отношением к курению и диагностике ССЗ, а также их гендерные особенности в открытой городской популяции необходимо учитывать при разработке и внедрении мер первичной профилактики. The validity of the use of a subjective-objective indicator of health as a significant indicator of its association with the prevalence of coronary artery disease is shown in both domestic and foreign scientific literature. At the same time, the subjective-objective indicator of health, and first of all, the attitude to one's health, is recognized as an indicator of a lifestyle that establishes the interpretation of conventional risk factors for coronary artery disease and interest in preventive measures at the population level. Purpose: to determine the attitude towards one's health in the age range and in groups with and without coronary artery disease in an open population of a medium-urbanized city of Western Siberia in terms of gender. Material and methods. Cross–sectional studies were carried out in the framework of two cardiac screenings on unorganized subpopulations of men and women 25–64 years old in Tyumen (with responses 85.0 and 70.3 %, respectively). The isolation of various forms of coronary heart disease was carried out on the basis of standard epidemiological methods, the comparison group was defined as the absence of coronary heart disease. The attitude of the population to their health was established using a rigidly standardized WHO questionnaire MONICApsychosocial «Knowledge and attitude to the health». Results. In the open population of a moderately urbanized city of Western Siberia, with a predominance among women, a low level of attitude towards their health was established, the prevalence of low responsibility for their health in the gender aspect took place in the third, fourth and sixth decades of life. Negative attitudes towards their health among persons of both sexes were determined mainly in groups with CHD; in women, the tendency towards a decrease in responsibility for their health was more pronounced than in men. Conclusion. When planning a preventive program for the population of a medium-urbanized city of Western Siberia, taking into account the priority impact on the subjective-objective indicator of population health, the implementation of the population strategy will probably not be enough. Taking into consideration the obtained regularities of the growth of negative attitudes towards their health among women, especially in groups with the presence of coronary heart disease, along with the population strategy, it would be optimal to use a high-risk strategy.
