The article represents literature review and provides evidence for psychological stress to play essential role in the development of arterial hypertension. The pathogenesis of hypertension is complex with a significant diversity and variability of the mechanisms involved in individual patient. In this regard, the determination of specific pathogenic pathways underlying sustained blood pressure elevation in each patient would substantially individualize therapeutic approaches, and hence increase the effectiveness of treatment. Psychological stress is proposed as a significant factor contributing to the development of hypertension. Global urbanization, sedentary lifestyle, daily stress at workplace, lack of physical activity and social support lead to increased anxiety, uncertainty, and finally to chronic mental and emotional stress. This review provides information about alterations in neuroendocrine and immune systems as the main pathogenic pathways linking psychological stress and hypertension. Endothelial dysfunction is considered not only as a consequence but also a primary factor causing prohypertensive state. Moreover, physical inactivity is discussed as one of the plausible mechanisms playing a key role in the development of hypertension in modern lifestyle conditions. Particularly the loss of connection between psychosocial strain and physical activity may underlie the deleterious effect of stress on cardiovascular and metabolic health.
The article represents literature review dedicated to molecular and cellular mechanisms underlying clinical manifestations and outcomes of acute myocardial infarction. Extracellular matrix adaptive changes are described in detail as one of the most important factors contributing to healing of damaged myocardium and post-infarction cardiac remodeling. Extracellular matrix is reviewed as dynamic constantly remodeling structure that plays a pivotal role in myocardial repair. The role of matrix metalloproteinases and their tissue inhibitors in fragmentation and degradation of extracellular matrix as well as in myocardium healing is discussed. This review provides current information about fibroblasts activity, the role of growth factors, particularly transforming growth factor β and cardiotrophin-1, colony-stimulating factors, adipokines and gastrointestinal hormones, various matricellular proteins. In conclusion considering the fact that dynamic transformation of extracellular matrix after myocardial ischemic damage plays a pivotal role in myocardial infarction outcomes and prognosis, we suggest a high importance of further investigation of mechanisms underlying extracellular matrix remodeling and cell-matrix interactions in cardiovascular diseases.
Цель. Изучить особенности суточного профиля артериального давления (АД) и вариабельности сердечного ритма (ВСР) в зависимости от уровня физиче-ской активности и психоэмоционального напряжения у пациентов, страдаю-щих артериальной гипертензией (АГ). Материал и методы. Обследовано 76 пациентов мужского пола, страдающих АГ I и II стадии (средний возраст 46,75±0,56 лет). Группу 1 составили 36 паци-ентов, страдающих АГ и ожирением, группу 2 -40 больных АГ без ожире-ния. Всем пациентам проведено суточное мониторирование АД и электрокар-диограммы (ЭКГ) с анализом ВСР. Показатель психоэмоционального напряже-ния пациентов определяли с помощью шкалы PSM-25. Уровень реактивной и личностной тревожности определяли с помощью опросника Спилбергера-Ханина. Для оценки физической активности использовали краткий междуна-родный опросник по физической активности (IPAQ). Результаты. При анализе данных суточного мониторирования АД наблюдалось значимое увеличение вариабельности систолического АД в дневное время у паци-ентов 1 группы на фоне гиподинамии (17,97±0,64 мм рт.ст.) по сравнению с пациен-тами, имеющими достаточный уровень физической активности (15,43±0,73 мм рт.ст., р=0,003). У пациентов с гиподинамией более часто встречался суточный ритм АД по типу non-dipper (68,8%) по сравнению с пациентами, имеющими доста-точную физическую нагрузку (51,6%). Также на фоне гиподинамии выявлено досто-верное снижение показателей временного анализа ВСР -SDNNi и RMSSD, отра-жающих дисбаланс автономной нервной регуляции сердечно-сосудистой сис-темы, кроме того, установлена обратная корреляционная взаимосвязь SDNNi и RMSSD с уровнем психоэмоционального напряжения. Во всех обследуемых группах отмечалась прямая корреляционная связь вариабельности систоличе-ского и диастолического АД с уровнем реактивной и личностной тревожности и с показателем психоэмоционального напряжения. Выявлена недостаточная сте-пень ночного снижения систолического и диастолического АД у пациентов с высо-ким уровнем личностной (r=-0,67; p<0,00001 и r=-0,60; p=0,0001) и реактивной тревожности (r=-0,54; р=0,0006 и r=-0,43; р=0,009). Заключение. У больных с АГ как гиподинамия, так и хроническое психоэмо-циональное перенапряжение являются самостоятельными факторами, приво-дящими к повышению вариабельности и уменьшению степени ночного сниже-ния АД, а также снижению вариабельности сердечного ритма независимо от наличия либо отсутствия ожирения.
