По данным ВОЗ, туберкулез (ТБ) и ВИЧ-инфекция являются одними из ведущих причин смертности населения в мире. На фоне значительного уменьшения числа больных ТБ в период 2004-2014 гг. (с 83,3 до 59,5 на 100 тыс. населения) в России наблюдается рост числа инфицированных ВИЧ (с 160,1 до 335,3 на 100 тыс. населения) и вновь выявленных больных ТБ, сочетанным с ВИЧ-инфекцией (ТБ/ВИЧ) за этот же период-с 0,9 до 9,9 на 100 тыс. населения [2]. Цель данного обзора-анализ эпидемиологии, системы организации контроля, профилактики и оказания медицинской помощи больным ТБ/ВИЧ в странах Восточной Европы и РФ на основе анализа опубликованных данных, включая аналитические обзоры, сборники статистических показателей по ТБ и ВИЧ, используемых в РФ. Рост заболеваемости ВИЧ-ассоциируемым ТБ сопровождается ростом распространенности ТБ с множественной лекарственной устойчивостью (МЛУ-ТБ)-с 14,2 до 24,8 на 100 тыс. населения. Удельный вес случаев МЛУ-ТБ среди больных ТБ, отбывающих наказание в Федеральной службе исполнения наказания, остается очень высоким (42% в 2005 г., почти 49% в 2009 г. и 42% в 2014 г.). В свою очередь рост числа лиц с ВИЧ-инфекцией, выявленной на 4б-5-й стадиях, затрудняет диагностику и снижает эффективность лечения ТБ, что ведет к тяжелому течению последнего и наступлению смерти в среднем в течение года после постановки диагноза. В период 2004-2012 гг. доля поздних стадий ВИЧ-инфекции среди больных ТБ составляла около 60%. Эта доля выросла до 63,7% в 2013 г. и достигла 70% в 2014 г. В связи с этим необходимо совершенствовать систему контроля, профилактики и организации медицинской помощи как больным ТБ, так и инфицированным ВИЧ в контексте ТБ/ВИЧ и МЛУ-ТБ с учетом лучших медицинских практик, накопленных в мире, и рекомендаций ВОЗ. Особое внимание следует уделять своевременному выявлению случаев ВИЧинфекции и МЛУ-ТБ у больных ТБ с целью оказания эффективной медицинской помощи больным и ухода за ними. Также необходимо обеспечить универсальный доступ к антиретровирусной терапии среди лиц, инфицированных ВИЧ. Необходимо проводить дополнительные исследования, направленные на получение данных для информирования политики и практики оказания медицинской помощи по ТБ/ВИЧ в уязвимых группах населения.
