Макулярный отёк рассматривается как фактор риска значительного снижения остроты зрения и качества жизни пациента независимо от его причины. Поиск причин макулярного отёка в некоторых случаях должен включать междисциплинарный подход и лабораторные исследования для правильного определения его этиологии и тактики ведения таких пациентов. Поэтому дифференциация этиологии и патогенеза макулярного отёка на этапе диагностики определяет возможность сохранения не только зрительных функций, но в некоторых случаях и жизни пациента. Цель исследования: проанализировать клинические случаи макулярного отёка, ассоциированного с тромбозом центральной вены сетчатки различной этиологии и выявить дифференциально-диагностические критерии и индивидуальные подходы к лечению. Материалы и методы. Проведено проспективное исследование отдельных пациентов с макулярным отёком, ассоциированным с тромбозом центральной вены сетчатки или её ветвей, в условиях реальной клинической практики. Всем пациентам проведено стандартное офтальмологическое обследование, а также оптическая когерентная томография, и пациентке с подозрением на макроглобулинемию проведён биохимический анализ крови. Результаты. В первом клиническом случае макулярный отёк был ассоциирован с экссудативной формой ретинальной артериальной макроаневризмы. Пациентке проведена прямая лазеркоагуляция ретинальной артериальной макроаневризмы. Через 6 месяцев после прямой лазеркоагуляции макроаневризмы острота зрения левого глаза с коррекцией составляет 1,0. Глазное дно -макроаневризма, отёк сетчатки, твёрдые экссудаты не офтальмоскопируются. Во втором клиническом случае макулярный отёк, тромбоз центральной вены сетчатки ассоциирован с болезнью Вальденстрема. Рекомендовано наблюдение и лечение основного заболевания у врача-гематолога. Заключение. Таким образом, макулярный отёк, протекающий на фоне тромбоза центральной вены сетчатки и её ветвей, имеющий атипичную симптоматику в виде кольцевидного отложения твёрдых экссудатов, экссудативной отслойки сетчатки, наличие двухстороннего процесса, а также резистентность к анти-VEGF терапии требуют тщательного сбора анамнеза с учётом соматического состояния пациента и наличия иной офтальмологической сосудистой патологии.
