Ο ομφάλιος λώρος αποτελεί σημαντική πηγή βλαστοκυττάρων, για τη θεραπεία αιματολογικών και μη αιματολογικών ασθενειών. Παγκοσμίως πραγματοποιούνται χιλιάδες μεταμοσχεύσεις με τη χρήση βλαστοκυττάρων από την ομφαλοπλακουντιακή μονάδα, ενώ παρατηρείται ραγδαία αύξηση των κλινικών μελετών για την εφαρμογή τους σε μεγαλύτερο αριθμό ασθενειών. Στην παρούσα διατριβή μελετήθηκαν όλοι οι παράγοντες που θα οδηγήσουν στην ανάκτηση και απομόνωση μεγάλου αριθμού βλαστοκυττάρων από την ομφαλοπλακουντιακή μονάδα αλλά και η ύπαρξη πολυδύναμων βλαστικών κυττάρων με ικανότητα διαφοροποίησης σε άλλους κυτταρικούς πληθυσμούς.Η χρήση των ομφαλοπλακουντιακών μονάδων έναντι άλλων πηγών μεταμοσχεύσεων πλεονεκτεί καθώς τα κύτταρα αυτά είναι πιο νεανικά βλαστικά κύτταρα του οργανισμού και επιπλέον πιο ανώριμα ανοσολογικά. Βασικό μειονέκτημα αποτελεί ο μικρός αριθμός βλαστικών κυττάρων που απομονώνονται από κάθε ομφαλοπλακουντιακή μονάδα. Βασικός στόχος της μελέτης ήταν η απομόνωση μεγαλύτερου αριθμού βλαστοκυττάρων τόσο με τις καθιερωμένες μεθόδους λήψης όσο και με νέες μεθόδους. Μελετήθηκαν οι παράγοντες και οι συνθήκες στις οποίες πρέπει το ομφαλοπλακουντιακό αίμα να μεταφέρεται μέχρι την επεξεργασία. Διαπιστώθηκε ότι διατήρηση στους 40C αλλά και η άμεση επεξεργασία για την απομόνωση ομφαλοπλακουντιακών μοσχευμάτων αποτελούν σημαντικούς παράγοντες της ανάκτησης του μεγαλύτερου αριθμού βλαστοκυττάρων. Εφαρμόστηκε διαδικασία επεξεργασίας των βλαστοκυττάρων από το ομφαλοπλακουντιακό αίμα που οδηγεί σε ποσοστά ανάκτησης βλαστοκυττάρων μικρού τελικού όγκου και υψηλής απομάκρυνσης των ερυθρών αιμοσφαιρίων χρησιμοποιώντας υλικά κατάλληλα για κλινικές μελέτες. Παράλληλα εφαρμόστηκε πρωτοποριακή μέθοδος απομόνωσης βλαστοκυττάρων από το εσωτερικό του πλακούντα. Έγινε αποστράγγιση του αίματος του πλακούντα που οδηγεί σε μεγαλύτερο αριθμό αιμοποιητικών βλαστικών κυττάρων αλλά και πολυδύναμων κυττάρων ικανών για διαφοροποίηση σε άλλες κυτταρικές σειρές. Με τη νέα τεχνική λαμβάνονται τουλάχιστον δυο διαφορετικοί πληθυσμοί βλαστικών κυττάρων. Ένας CD34+, CD45+αιμοποιηιτικός πληθυσμός με δυνατότητες δημιουργίας όλων των αιμοποιητικών σειρών και ένας CD34-, CD45- πληθυσμός που εκφράζει μεσεγχυματικούς δείκτες, όπως CD90, CD105, CD29, CD44, και δυνατότητα διαφοροποίησης προς άλλους ιστούς του μεσοδέρματος. Η νέα ποσότητα ομφαλοπλακουντιακού αίματος που λαμβάνεται είναι εμβρυϊκής προέλευσης καθώς δε διαπιστώθηκε επιμόλυνση από μητρικό αίμα. Παράλληλα απομονώθηκαν πολυδύναμα βλαστοκύτταρα από τον ομφάλιο λώρο με τη προσθήκη βλαστικών κυττάρων από το λιπώδη ιστό. Τα κύτταρα που απομονώθηκαν μελετήθηκαν με τη βοήθεια κυτταρομετρίας ροής και ήταν θετικά σε δείκτες που εκφράζουν τα πολυδύναμα βλαστοκύτταρα (USSCs). Απομονώθηκαν βλαστοκύτταρα από τη γέλη του Wharton και εφαρμόστηκε μια νέα τεχνική απομόνωσης μεσεγχυματικών βλαστικών κυττάρων από ολόκληρο το μήκος του ομφαλίου λώρου. Με τη νέα τεχνική απομονώνονται περισσότερα μεσεγχυματικά κύτταρα από τον ιστό του ομφάλιου λώρου. Κύτταρα που απομονώθηκαν διαφοροποιήθηκαν σε οστικά κύτταρα και λιποκύτταρα. Τα βλαστικά κύτταρα διατήρησαν τα χαρακτηριστικά τους κατά τη διάρκεια των ανακαλλιεργειών και μετά την απόψυξη. Κύτταρα που απομονώθηκαν από ολόκληρο το μήκος του ομφαλίου λώρου ελέχθησαν για την ύπαρξη πολυδύναμων εμβρυϊκών δεικτών. Διαπιστώθηκε η παρουσία μικρών σε μέγεθος κυττάρων στον ιστό του ομφαλίου λώρου που παρουσιάζουν μορφολογικά και φαινοτυπικά χαρακτηριστικά παρόμοια με αυτά των εμβρυϊκών κυττάρων (VSELs). Τα κύτταρα που απομονώθηκαν ήταν θετικά για τους εμβρυϊκούς δείκτες Oct CXCR4 και Nanog ενώ χρωματίζονται έντονα με την αλκαλική φωσφατάση, χρωστική που χαρακτηρίζει και τα εμβρυϊκά πολυδύναμα κύτταρα. Συνοψίζοντας μπορούμε να πούμε ότι τα αποτελέσματα της παρούσας διατριβής ενισχύουν τη σημασία των ομφαλοπλακουντιακών μονάδων στο πεδίο των μεταμοσχεύσεων αλλά και στις εφαρμογές της αναγεννητικής ιατρικής. Νέες μέθοδοι απομόνωσης αλλά και νέες πηγές αιμοποιητικών, μεσεγχυματικών και πολυδύναμων βλαστικών κυττάρων προτείνονται οι οποίες θα αυξήσουν τη χρήση των ομφαλοπλακουντιακών μονάδων για θεραπευτικές εφαρμογές.
