Εισαγωγή/Σκοπός: Αυξημένα επίπεδα λεπτίνης, αντιπονεκτίνης και ρεζιστίνης έχουν αναφερθεί στην κίρρωση αλλά διαβητικοί ασθενείς δεν έχουν εξαιρεθεί, παρόλο που ο διαβήτης αλλάζει τις συγκεντώσεις των αντιποκινών. Δεν είναι ξεκάθαρο εάν η σοβαρότητα της κίρρωσης συσχετίζεται με τις συγκεντρώσεις των αντιποκινών. Η αλκοολική κίρρωση είναι η λιγότερο μελετημένη μορφή ηπατοπάθειας. Εκτιμήσαμε τα επίπεδα στον ορό έξι αντιποκινών (λεπτίνη, αντιπονεκτίνη, ρεζιστίνη, βισφατίνη, RBP4, απελίνη) σε ασθενείς με αλκοολική κίρρωση σε διάφορα στάδια σοβαρότητας της νόσου. Μέθοδοι: Σαράντα μη διαβητικοί ασθενείς με αλκοολική κίρρωση (μέση ηλικία: 59 έτη, άνδρες: 87.5%, Child-Pugh (CP) score: μέση τιμή 7 (5-12), CP A/B/C: 18/10/12, Μοντέλο ηπατικής νόσου τελικού σταδίου (MELD): μέση τιμή 10 (6-25), χρόνος παρακολούθησης: μέση τιμή 32.5 μήνες (10-43)) συμπεριλήφθηκαν προοπτικά. Τα επίπεδα των αντιποκινών εκτιμήθηκαν με ELISA δυο φορές. Τα σωματομετρικά χαρακτηριστικά εκτιμήθηκαν μέσω βιοηλεκτρικής εμπέδησης.Αποτελέσματα: ΒΜΙ: 25.9 (20.1-39.3), λίπος: 23.4% (7.6-42.1), λιπώδης μάζα: 17.8 (5.49-45.4), ελέυθερη λίπους μάζα (FFM): 56.1 (39.6-74.4), ολικό νερό σώματος (TBW): 40.6 (29.8-58.8). Τα επίπεδα λεπτίνης και βισφατίνης συσχετίστηκαν θετικά με τη λιπώδη μάζα (p<0.001/p=0.027, αντίστοιχα) και η RBP4 με το TBW (p=0.025). Τα επίπεδα της αντιπονεκτίνης ήταν σημαντικά υψηλότερα σε ασθενείς με CP C συγκριτικά με τους ασθενείς με CP A (CPA: 7.99±14.07, CPB: 7.66±3.48, CPC: 25.73±26.8, p=0.04), ενώ τα επίπεδα RBP4 και απελίνης μειώθηκαν αναλογικά με τη σοβαρότητα της ηπατικής νόσου (p=0.006/p=0.034, αντίστοιχα). Κατόπιν προσαρμογής ανάλογα με τη λιπώδη μάζα, η βισφατίνη και η αντιπονεκτίνη αυξήθηκαν σημαντικά από το CP A στο CP C (p<0.001), ενώ μια αντίστροφη συσχέτιση παρατηρήθηκε για τη RBP4 και την απελίνη (p<0.001). Στην πολυπαραγοντική ανάλυση, μόνο το MELD εμφάνισε ανεξάρτητη συσχέτιση με την ολική επιβίωση (HR:1.53, 95%CI:1.05-2.32, p=0.029).Συμπέρασμα: Οι αντιποκίνες συσχετίζονται με την επιδεινούμενη ηπατική λειτουργία με έναν περίπλοκο τρόπο σε ασθενείς με αλκοολική κίρρωση.
