Exercise is a stress stimulus for the human organism affecting the homeostatic mechanisms of the body, depending on the type, duration, intensity and frequency of exercise. The aim of this study was to determine the effects of a moderate aerobic exercise bout on the Hypothalamo-Pituitary-Adrenal (HPA) axis acute hormonal responses in healthy adult humans. Twelve healthy male and female volunteers (age: 30.6 ± 4.4 years), performed a single bout of a 30-minute aerobic exercise at 70% of VO 2max on a treadmill, following standard diet. Blood samples were collected before (t 0), at the end of the exercise bout (t30), and 30 min after the completion of exercise (t 60). Serum adrenocorticotropic hormone (ACTH), cortisol (COR), aldosterone (ALDO) and renin (REN) were measured. One-way ANOVA was used for statistics. ACTH and COR decreased after exercise, reaching significance (p < 0.01) 30 min after the completion of the exercise bout. ALDO increased at the end of exercise and remained elevated 30 min after its completion. REN significantly increased at the end of exercise (p < 0.05) and remained elevated. The exercise regimen used in this study had beneficial effects on the stress axis, suggesting that specific exercise protocols can be characterised by mild physiological stress-inducing effects hence be prescribed for special diseased populations.
Η σωματική άσκηση αποτελεί μια μορφή φυσιολογικού στρες για τον οργανισμό, ωστόσο η σχέση μεταξύ των ιδιαίτερων χαρακτηριστικών (έντασης και διάρκειας) της άσκησης και των ορμονικών αποκρίσεων που σχετίζονται με το στρεσογόνο αυτό ερέθισμα είναι συχνά διαφορετική, εξαιτίας τόσο των πολλών παραγόντων που εμπλέκονται στη ρύθμιση αυτών των αποκρίσεων όσο και των ιδιαίτερων χαρακτηριστικών των πρωτόκολλων άσκησης. Σκοπός αυτής της μελέτης ήταν να διερευνηθούν ειδικότερα οι ορμονικές απαντήσεις που σχετίζονται με τον υποθαλαμο-υποφυσιο-επινεφριδιακό (HPA) άξονα (ο οποίος συνδέεται με το στρες) σε υγιείς εθελοντές μετά από ένα πρωτόκολλο άσκησης μέτριας έντασης και μέσης διάρκειας. ΜΕΘΟΔΟΛΟΓΙΑ: Δώδεκα υγιείς εθελοντές, 8 άνδρες and 4 γυναίκες (ηλικία: 30.6±4.4 έτη, σωματική μάζα: 77.3±12.3 kg και ανάστημα: 1.77±0.07 m) εκτέλεσαν άπαξ ένα πρωτόκολλο αερόβιας άσκησης διάρκειας 30΄, με ένταση 70% VO2max σε κυλιόμενο τάπητα, μετά από καθορισμένη δίαιτα. Δείγματα αίματος συλλέχτηκαν πριν την άσκηση (tπ), κατά την επίτευξη σταθερής έντασης άσκησης 70% VO2max (t0), στο τέλος της τριαντάλεπτης άσκησης (t30) και 30΄ μετά το τέλος της άσκησης (t60). Μετρήθηκαν οι ορμόνες αδρενοκορτικοτρόπος (ACTH), κορτιζόλη ορού (COR ser), κορτιζόλη σιέλου (COR sal), αλδοστερόνη (ALDO), ρενίνη (REN), ινσουλίνη (INS), αυξητική ορμόνη (GH), ινσουλινομιμητικός αυξητικός παράγοντας-1 (IGF-1), προλακτίνη (PRL), καθώς και η ιντερλευκίνη (IL-6) και η C-αντιδρώσα πρωτεΐνη (CRP). Το ίδιο πρωτόκολλο άσκησης εφαρμόστηκε μετά από χορήγηση δεξαμεθαζόνης για πρόκληση καταστολής του υποθαλαμο-υποφυσιο-επινεφριδιακού άξονα και μετά από χορήγηση τετρακοσακτίδης (Synacthen) για πρόκληση διέγερσης του υποθαλαμο-υποφυσιο-επινεφριδιακού άξονα και μετρήθηκαν οι ίδιες ορμόνες και παράγοντες στα ίδια χρονικά σημεία. Για τη στατιστική ανάλυση χρησιμοποιήθηκαν μη παραμετρικά τεστ Two-way ANOVA και One-way ANOVA καθώς και o συντελεστής συσχέτισης Pearson (r). ΑΠΟΤΕΛΕΣΜΑΤΑ: Οι ACTH και COR ser μειώθηκαν μετά την άσκηση κι έφτασαν σε στατιστική σημαντικότητα (p<0.01) 30΄ μετά την ολοκλήρωση της άσκησης. Η ALDO αυξήθηκε στο τέλος της άσκησης και παρέμεινε αυξημένη 30΄ μετά τη συμπλήρωση της άσκησης. Αυτές οι αλλαγές στις πρωτογενείς τιμές δεν ήταν στατιστικά σημαντικές, ωστόσο η περαιτέρω ανάλυση των ποσοστιαίων μεταβολών σε σύγκριση με τον χρόνο tπ ανέδειξε στατιστική σημαντικότητα αυτών των μεταβολών. Η ALDO παρουσίασε σημαντική αύξηση στο τέλος της άσκησης (p<0.001) και αυξήθηκε περαιτέρω 30΄ μετά το τέλος της άσκησης (p<0.001). Η REN αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.05) και παρέμεινε αυξημένη 30΄ μετά το τέλος της άσκησης. Η INS μειώθηκε στο τέλος της άσκησης και έφτασε σε στατιστική σημαντικότητα (p<0.05) 30΄ μετά το τέλος της άσκησης. Η περαιτέρω ανάλυση των ποσοστιαίων μεταβολών σε σύγκριση με το χρόνο tπ, ανέδειξε σημαντική πτώση της INS και στο τέλος της άσκησης (p<0.05). Η GH αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.001), ενώ 30΄ μετά το τέλος της παρουσίασε μείωση χωρίς να φτάσει όμως στα προασκησιακά επίπεδα (tπ). Η μείωση αυτή ήταν στατιστικά σημαντική μόνο σε σύγκριση με τον χρόνο t30 (p<0.001). Ο IGF-1 και η PRL δεν παρουσίασαν σημαντικές μεταβολές. Η IL-6 αυξήθηκε μη στατιστικώς σημαντικά στο τέλος της άσκησης και παρέμεινε αυξημένη 30΄ μετά το τέλος της άσκησης. Όταν αναλύθηκαν οι ποσοστιαίες μεταβολές σε σύγκριση με τον χρόνο tπ, η IL-6 αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.01) και παρέμεινε αυξημένη 30΄ μετά το τέλος της άσκησης. Η CRP μειώθηκε 30΄ μετά το τέλος της άσκησης, χωρίς να εμφανίσει στατιστική σημαντικότητα. Σε συνθήκες καταστολής του υποθαλαμο-υποφυσιο-επινεφριδιακού άξονα, οι ACTH και COR παρέμειναν σε σταθερά και σε χαμηλά επίπεδα, αντίστοιχα, όπως ήταν αναμενόμενο. Όταν αναλύθηκαν οι ποσοστιαίες μεταβολές της COR sal, βρέθηκε ότι αυτή αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.05) και 30΄ μετά το τέλος της άσκησης (p<0.05). Η ALDO αυξήθηκε, όχι σημαντικά, στο τέλος της άσκησης και μειώθηκε 30΄ μετά την ολοκλήρωση της άσκησης, φτάνοντας σε στατιστικά σημαντικά επίπεδα 30΄ μετά την ολοκλήρωση της άσκησης, σε σύγκριση με τον χρόνο t30 (p<0.05). Η INS μειώθηκε σημαντικά στο τέλος της άσκησης (p<0.05) και μειώθηκε περαιτέρω 30΄ μετά το τέλος της άσκησης (p<0.05). Η GH αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.001) ενώ 30΄ μετά το τέλος της άσκησης μειώθηκε σημαντικά χωρίς, ωστόσο, να φτάσει στα προασκησιακά επίπεδα (p<0.001). Ο IGF-1 δεν επηρεάστηκε από την άσκηση. Η PRL αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.05) και στη συνέχεια μειώθηκε χωρίς πλήρη επαναφορά της στα προασκησιακά επίπεδα. Η IL-6 αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.05) και αυξήθηκε σημαντικά περαιτέρω 30΄ μετά το τέλος της άσκησης (p<0.001). Επίσης, όταν αναλύθηκαν οι ποσοστιαίες μεταβολές της σε σύγκριση με τον χρόνο tπ, η IL-6 παρουσίασε σημαντική αύξηση τόσο στο τέλος της άσκησης (p<0.01) όσο και 30΄ μετά το τέλος της άσκησης (p<0.01). Η IL-6 παρουσίασε σημαντική αύξηση 30΄ μετά το τέλος της άσκησης και σε σύγκριση με τον χρόνο t30 (p<0.01). Η CRP δεν παρουσίασε σημαντικές μεταβολές. Σε συνθήκες διέγερσης του ΗΡΑ άξονα οι COR ser και COR sal αυξήθηκαν σημαντικά, όπως ήταν αναμενόμενο, στο τέλος της άσκησης (p<0.001) και 30΄ μετά το τέλος της άσκησης (p<0.001). Η COR sal αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.001) και 30΄ μετά το τέλος της άσκησης (p<0.001). Η ALDO αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.05). Η περαιτέρω ανάλυση των ποσοστιαίων μεταβολών της ALDO έδειξε πως παρέμεινε σημαντικά αυξημένη 30΄ μετά την ολοκλήρωση της άσκησης (p<0.01). Η INS μειώθηκε και έφτασε σε στατιστικά σημαντική μείωση 30΄ μετά το τέλος της άσκησης (p<0.05). Η GH αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.001) ενώ 30΄ μετά το τέλος της άσκησης μειώθηκε σημαντικά χωρίς, ωστόσο, να φτάσει στα προασκησιακά επίπεδα (p<0.001). Ο IGF-1 δεν άλλαξε μετά την άσκηση. Η PRL αυξήθηκε σημαντικά στο τέλος της άσκησης (p<0.01) μόνο για τις αναλύσεις των ποσοστιαίων μεταβολών. Η IL-6 αυξήθηκε στο τέλος της άσκησης φτάνοντας σε στατιστική σημαντικότητα 30΄ μετά το τέλος της άσκησης (p<0.01). Επίσης, όταν αναλύθηκαν οι ποσοστιαίες μεταβολές της σε σύγκριση με τον χρόνο tπ, η IL-6 παρουσίασε σημαντική αύξηση τόσο στο τέλος της άσκησης (p<0.001) όσο και 30΄ μετά το τέλος της άσκησης (p<0.05). Η IL-6 παρουσίασε σημαντική αύξηση 30΄ μετά το τέλος της άσκησης και σε σύγκριση με τον χρόνο t30 (p<0.01). Η CRP δεν παρουσίασε σημαντικές μεταβολές. ΣΥΖΗΤΗΣΗ: Τα κύρια ευρήματα της μελέτης ήταν η μείωση της ACTH και COR και η αύξηση της ALDO και της REN μετά από το συγκεκριμένο πρωτόκολλο αερόβιας άσκησης που εφαρμόστηκε άπαξ. Η αύξηση της GH και η μείωση της COR ser υποδηλώνει ένα τελικό θετικό (αναβολικό) αποτέλεσμα της άσκησης για υγιείς και ενδεχομένως και για ασθενείς. Επιπλέον, η IL-6 αυξάνεται σημαντικά με την άσκηση χωρίς να ενεργοποιείται η έκκριση της COR. Άρα ενδεχομένως αποφεύγονται οι καταβολικές δράσεις με ταυτόχρονη διέγερση των αναβολικών οδών. Επίσης η ALDO αυξάνεται με την άσκηση και παράλληλα παρατηρείται αύξηση της REN γεγονός που καταδεικνύει ότι ενεργοποιείται κάποιος αγγειοκινητικός μηχανισμός για την έκκριση της REN, στη Φάση Α. Η απάντηση της GH στην άσκηση υπό συνθήκες καταστολής της ACTH εξασθενεί (φάση Β). Η απάντηση της GH στην άσκηση υπό συνθήκες διέγερσης της ACTH αυξάνεται (φάση Γ). Η επιλεγείσα αερόβια άσκηση (έντασης 70%VO2max και διάρκειας 30΄) αυξάνει την GH και μειώνει την COR. Η χορήγηση δεξαμεθαζόνης δεν επηρεάζει την ασκησιογενή αύξηση της GH. Οι αλλαγές της GH μετά από άσκηση δεν επηρεάζονται από τον άξονα ΗΡΑ. Tα αποτελέσματα αυτά δείχνουν πως το συγκεκριμένο πρωτόκολλο είναι χρήσιμο για αυτούς που κάνουν καθιστική ζωή αλλά και ενδεχομένως για ειδικούς πληθυσμούς ασθενών που χρειάζεται να κάνουν άσκηση χωρίς όμως να αυξήσουν ιδιαίτερα το φυσιολογικό στρες στον οργανισμό τους. Είναι ενδιαφέρον και σημαντικό να μελετηθούν και να καθοριστούν τα βέλτιστα πρωτόκολλα άσκησης που προσφέρουν τα ευεργετήματα της άσκησης χωρίς τις στρεσογόνες επιδράσεις αυτής. Περισσότερες ερευνητικές μελέτες απαιτούνται για να οριστούν τα πρωτόκολλα άσκησης που θα μπορούν να συνταγογραφούνται σε διάφορους κλινικούς πληθυσμούς και διαφορετικές ηλικιακές ομάδες.
