Εισαγωγή: Η χρήση χειρουργικών πλεγμάτων στην αποκατάσταση κοιλιοκήλης έχει μειώσει σημαντικά τα ποσοστά υποτροπής. Ωστόσο, η ενδοπεριτοναïκή τοποθέτηση προσθετικών υλικών μπορεί να προκαλέσει την ανάπτυξη ενδοπεριτοναïκών συμφύσεων. Η παρούσα πειραματική μελέτη αξιολόγησε τη συνδυασμένη χορήγηση 4% ικοδεξτρίνης και μηλεϊνικής διμεθινδένης για την πρόληψη σχηματισμού ενδοπεριτοναïκών συμφύσεων σε πλέγμα πολυπροπυλενίου και σε επικαλυμμένο πλέγμα πολυπροπυλενίου με τιτάνιο.Υλικά και μέθοδοι: Εξήντα θηλυκά λευκά κουνέλια χωρίστηκαν σε τέσσερις ομάδες. Ένα τεμάχιο πλέγματος 2x2 cm στερεώθηκε σε άθικτο περιτόναιο σε όλα τα ζώα μέσω μέσης τομής. Στις ομάδες 1 και 2 τοποθετήθηκε ελαφρύ πλέγμα πολυπροπυλενίου ενώ στις ομάδες 3 και 4 πλέγμα πολυπροπυλενίου επικαλυμμένο με τιτάνιο. Οι ομάδες 2 και 4 υπεβλήθησαν σε διεγχειρητική αγωγή με ενδοφλέβια χορήγηση μηλεϊνικής διμεθινδένης (0,1 mg / kg) και ενδοπεριτοναϊκού διαλύματος 4% icodextrin (20 ml / kg) και για τις επόμενες έξι ημέρες με μηλεϊνική διμεθινδένη ενδομυϊκά. Μετά τις 15 ημέρες τα πειραματόζωα θανατώθηκαν και προσδιορίστηκαν η παρουσία και το είδος των συμφύσεων, το ποσοστό κάλυψης της επιφάνειας του πλέγματος από συμφύσεις, τα επίπεδα της υδροξυπρολίνης των ιστών και παράλληλα έγινε ιστοπαθολογική ανάλυση των δειγμάτων.Αποτελέσματα: Όλα τα πειραματόζωα στην ομάδα 1 και το 57% των πειραματόζωων στην ομάδα 3 παρουσίασαν μετεγχειρητικές συμφύσεις. Ο συνδυασμός των αντισυμφυτικών μείωσε σημαντικά την έκταση και τη σοβαρότητα των συμφύσεων και τα επίπεδα υδροξυπρολίνης στις ομάδες 2 και 4 σε σύγκριση με τις ομάδες 1 και 3. Κατά τη μικροσκοπική αξιολόγηση τα πειραματόζωα στην ομάδα 1 εμφάνισαν υψηλότερες βαθμολογίες φλεγμονής συγκριτικά με την ομάδα 2, ενώ οι ομάδες 2 και 4 είχαν καλύτερη μεσοθηλιοποίηση σε σχέση με τις ομάδες 1 και 3.Συμπεράσματα: Η συνδυασμένη χορήγηση 4% ικοδεξτρίνης και μηλεïνικής διμεθινδένης μειώνει την έκταση και τη σοβαρότητα των συμφύσεων και μπορεί να χρησιμοποιηθεί με επιτυχία για την πρόληψη συμφύσεων μετά από ενδοπεριτοναïκή τοποθέτηση πλέγματος.
