ΣΗΜΑΣΙΑ: Ο τραυματισμός του παλίνδρομου λαρυγγικού νεύρου (ΠΛΝ) είναι μία από τις πιο σοβαρές επιπλοκές της επέμβασης του θυρεοειδούς. Η διεγχειρητική νευροπαρακολούθηση (IONM) έχει εισαχθεί για την επαλήθευση της ακεραιότητας λειτουργίας του ΠΛΝ και μπορεί να είναι ένα χρήσιμο βοήθημα στη χειρουργική παρασκευή του παλίνδρομου λαρυγγικού νεύρου. ΣΤΟΧΟΣ: Για να προσδιορίσετε αν η χρήση του IONM μπορεί να μειώσει τη συχνότητα εμφάνισης των ΠΛΝ τραυματισμό σε ασθενείς που υποβάλλονται σε ολική θυρεοειδεκτομή. ΣΧΕΔΙΑΣΜΟΣ, ΡΥΘΜΙΣΗ ΚΑΙ ΣΥΜΜΕΤΕΧΟΝΤΕΣ: Η μελέτη περιελάμβανε 2556 ασθενείς που υποβλήθηκαν σε ολική θυρεοειδεκτομή μεταξύ Ιανουαρίου 2002 και Δεκεμβρίου 2012, στην Ωτορινολαρυγγολογική κλινική του Βενιζέλειου - Πανάνειου Γενικού Νοσοκομείου Ηρακλείου. Οι ασθενείς στους οποίους χρησιμοποιήθηκε η συσκευή IONM κατά τη διάρκεια της θυρεοειδεκτομής (n = 1481) συγκρίθηκαν με τους ασθενείς που υποβλήθηκαν σε χειρουργική επέμβαση με οπτική αναγνώριση των νεύρων μόνο (n = 1075). Όλοι οι ασθενείς υποβλήθηκαν σε έμμεση λαρυγγοσκόπηση τόσο προεγχειρητικά και όσο και τη δεύτερη μετεγχειρητική ημέρα για την αξιολόγηση της κινητικότητας των φωνητικών χορδών. ΚΥΡΙΑ ΑΠΟΤΕΛΕΣΜΑΤΑ ΚΑΙ ΜΕΤΡΑ: Η χρήση της συσκευής IONM και η επίπτωση τραυματισμού των παλίνδρομων λαρυγγικών νεύρων. ΑΠΟΤΕΛΕΣΜΑΤΑ: Συνολικά συμπεριλήφθηκαν 2556 ασθενείς (2028 γυναίκες και 528 άνδρες [5112 ΠΛΝ σε κίνδυνο] με μέση[SD] ηλικία, 51.35 [14.18] έτη και εύρος ηλικίας από 18 έως 89 ετών) που υποβλήθηκαν σε ολική θυρεοειδεκτομή. Η μονοπαραγοντική ανάλυση έδειξε ότι η χρήση της συσκευής IONM οδήγησε σε σημαντική μείωση της συχνότητας εμφάνισης τραυματισμού του ΠΛΝ (3,3% έναντι 0,7%) με μείωση του σχετικού κινδύνου κατά 2,6% (odds ratio [OR] = 5.15, 95% CI = 3.12 – 8.49). Η πολυπαραγοντική λογιστική παλινδρόμηση έδειξε ότι η μη χρήση της συσκευής IONM ήταν ένας ανεξάρτητος παράγοντας κινδύνου για τραυματισμό του ΠΛΝ σε ασθενείς που υποβλήθηκαν σε ολική θυρεοειδεκτομή (p <0.001, adjusted odds ratio [AOR] = 5.44, 95% CI = 03.26 - 09.09). Πρόσθετοι παράγοντες κινδύνου για τον τραυματισμό του ΠΛΝ ήταν ο χρόνος της χειρουργικής επέμβασης (p <0.001, AOR = 12,91, 95% CI = 6.66 – 25.06), η μέγιστη διάμετρος του δεξιού λοβού μεγαλύτερη των 45 mm (p <0.001, AOR = 4.91, 95% CI = 3.12 - 8.56) και του αριστερού λοβού του θυρεοειδούς (p = 0.027, AOR = 2.24, 95% CI = 1.39 – 4.32), η εξωθυρεοειδική επέκταση (p = 0.021, AOR = 3.26, 95% CI = 1.62 – 6.59), η τυχαία παραθυρεοειδεκτομή ( p = 0.001, AOR = 3,30, 95% CI = 2.13 - 5.09), και το μέγεθος του όγκου μεγαλύτερο από 10 mm (p = 0.019, AOR = 3.24, 95% CI = 1.59 – 6.62). ΣΥΜΠΕΡΑΣΜΑΤΑ ΚΑΙ ΣΗΜΑΣΙΑ: Τα ευρήματά μας έδειξαν ότι η χρήση της συσκευής IONM μείωσε σημαντικά την επίπτωση τόσο για του προσωρινού όσο και μόνιμου τραυματισμού του παλίνδρομου λαρυγγικού νεύρου. Η τεχνολογία της διεγχειρητικής παρακολούθησης του ΠΛΝ είναι ασφαλής και αξιόπιστη, και αυτή η τεχνική είναι μια σημαντική προσθήκη στη χειρουργική παρασκευή αλλά και στη διαπίστωση της λειτουργικής ακεραιότητας του ΠΛΝ. Η τακτική χρήση της συσκευής IONM μειώνει τις παγίδες και βοήθησε στην καθοδήγηση των χειρουργών μας σε δύσκολες περιπτώσεις, επανεγχειρήσεις αλλά και σε ασθενείς υψηλού κινδύνου.
Γονιδιακοί πολυμορφισμοί και θυρεοειδική λειτουργία σε ασθενείς με καρδιακή ανεπάρκεια Σκοπός:Στην παρούσα μελέτη εκτιμήσαμε τις πιθανές συσχετίσεις των γονιδιακών πολυμορφισμών του πυρηνικού παράγοντα κάπα Β{NFkB, rs28362491 [-94ins/delATTG (W/D)]} και του μετατρεπτικού ενζύμου της αγγειοτασίνη {ACE; rs1799752 [Ins(I)/Del(D)]} με την λειτουργία του θυρεοειδούς αδένα σε ασθενείς με καρδιακή ανεπάρκεια.Σχεδιασμός της μελέτης:Η μέγιστη κατανάλωση οξυγόνου (VO2) όπως αυτή εκτιμάτε από την μέγιστη καρδιοαναπνευστική δοκιμασία και την σταδιοποίηση κατά Weber προσδιοριστικέ σε 194 ασθενείς με καρδιακή ανεπάρκεια. Η λειτουργία του θυρεοειδούς αδένα έγινε με τον προσδιορισμό των : Thyroid-stimulating hormone (TSH), triiodothyronine (T3), thyroxine (T4) and free (F) T3 and FT4. Οι ασθενείς ανάλογα με την ικανότητα τους για άσκηση κατατάχτηκαν σε 4 ομάδες : Στην ομάδα Α με peak VO2 >20 ml/kg/min, στην ομάδα Β με peak VO2 =16-20 ml/kg/min, στην ομάδα Γ με peak VO2 =10-16 ml/kg/min, και στην ομάδα Δ με peak VO2 =6-10 ml/kg/min. Στους παραπάνω ασθενείς επιπρόσθετα έγινε γονιδιακή ανάλυση για NFkB and ACE.Αποτελέσματα:Οι τιμές της Τ3 και της ελεύθερης Τ3 μειώνονται με κάθε αύξηση της ομάδας κατά Weber. (p=0.007 and p=0.012 αντίστοιχα). Οι ασθενείς με Del γονότυπο έχουν υψηλότερη FT3 σε σχέση με τους ασθενείς με Ins γονότυπο (p=0.021).Οι ασθενείς με γονότυπο ΙΙ είχαν αυξημένα επίπεδα Τ4 σε σχέση με τους ασθενείς με ID γονότυπο (p=0.044). Τόσο οι τιμές της Τ4 όσο και οι τιμές στης ελεύθερης Τ4 ήταν μειωμένες στην ομάδα των ασθενών με αλλήλιο D (p=0.007 and p=0.045, αντίστοιχα).Συμπεράσματα:Οι ορμόνες του θυρεοειδούς αδένα συσχετίζονται με την ικανότητα για άσκηση. Επιπρόσθετα παρατηρήθηκαν συσχετίσεις ανάμεσα στους πολυμορφισμούς του γονιδίου του NFkB και του ACE και των επιπέδων των θυρεοειδικών ορμονών. Περισσότερες και μεγαλύτερου μεγέθους μελέτες είναι απαραίτητες για να διαλευκάνουν την συνεισφορά των γονιδιακών πολυμορφισμών στην καρδιακή ανεπάρκεια.
