Τα μεσεγχυματικά βλαστικά κύτταρα (MSCs) αποτελούν μία ιδιαίτερη κατηγορία βλαστικών κυττάρων, καθώς επιπλέον της ιδιότητάς τους να αυτοανανεώνονται και να διαφοροποιούνται σε κύτταρα μεσοδερμικής, κυρίως, προέλευσης, παρουσιάζουν και ανοσορρυθμιστική δράση in vivo. Θεωρείται κατά συνέπεια ότι θα μπορούσαν να συμβάλλουν σε κλινικές εφαρμογές αναγεννητικής Ιατρικής και σε προηγμένες κυτταρικές θεραπείες. Έχει ωστόσο αποδειχθεί ότι τα βλαστικά κύτταρα που απομονώνονται από ιστούς ενηλίκων οργανισμών, είναι «γηρασμένα», παρουσιάζουν μειωμένη πολλαπλασιαστική ικανότητα και περιορισμένη λειτουργικότητα. Σκοπός της παρούσας Διδακτορικής Διατριβής ήταν να διαπιστωθεί κατά πόσο το ομφαλοπλακουντιακό αίμα και ο ιστός του ομφαλίου λώρου, θα μπορούσαν να χρησιμεύσουν ως πηγές MSCs για κλινικές εφαρμογές. Για την πραγματοποίηση της μελέτης, κύτταρα που απομονώθηκαν από το ομφαλοπλακουντιακό αίμα με επιστίβαση, αφού είχε προηγηθεί εμπλουτισμός του πληθυσμού των MSCs με τη χρήση εμπορικού σκευάσματος RosetteSep αλλά και χωρίς εμπλουτισμό, καθώς και κύτταρα που απομονώθηκαν από τον ομφάλιο λώρο με ιστοκαλλιέργειες ή ενζυματική πέψη (με κολλαγενάση και υαλουρονιδάση) καλλιεργήθηκαν εκτεταμένα σε διαφορετικές συνθήκες. Συγκεκριμένα, εξετάστηκαν δύο διαφορετικά θρεπτικά υλικά: ένα εμπορικό κιτ για την καλλιέργεια MSCs και το α-ΜΕΜ συμπληρωμένο είτε με τον ζωικό ορό εμβρύου βοός είτε με ανθρώπινο ορό ομφαλοπλακουντιακού αίματος. Για το φαινοτυπικό χαρακτηρισμό των MSCs ελέγχθηκε ο ανοσοφαινότυπός τους με κυτταρομετρία ροής καθώς και η έκφραση γονιδίων πολυδυναμικότητας των βλαστικών κυττάρων με αντίστροφη μεταγραφή και αλυσιδωτή αντίδραση πολυμεράσης Παράλληλα μελετήθηκε η ιδιότητα διαφοροποίησής τους με την καλλιέργειά τους σε θρεπτικά υλικά που επάγουν την οστεογένεση, τη λιπογένεση και τη χονδρογένεση. Τα διαφοροποιημένα κύτταρα ανιχνεύθηκαν με τη βοήθεια των κατάλληλων χρώσεων. Σε ένα δεύτερο χρόνο επιχειρήθηκε η κρυοσυντήρηση των MSCs του ομφαλίου λώρου εξετάζοντας την επίδραση της κατάψυξης, σε διαφορετικά χρονικά διαστήματα, στα χαρακτηριστικά και τις ιδιότητες των κυττάρων που είχαν απομονωθεί από τον ομφάλιο λώρο με τις προαναφερθείσες μεθόδους. Τέλος μελετήθηκε η δυνατότητα επικοισμού ικριωμάτων κολλαγόνου από MSCs του ομφαλίου λώρου με συγκαλλιέργειά τους τόσο σε στατικές όσο και δυναμικές συνθήκες. Από την ανάλυση των αποτελεσμάτων: -Προέκυψε ότι MSCs υπάρχουν στο ομφαλοπλακουντιακό αίμα αλλά η συστηματική απομόνωσή τους είναι ανέφικτη με τις μεθόδους που διαθέτουμε αυτή τη στιγμή. Θεωρούμε ότι η διαπίστωση αυτή, στο μεγαλύτερο δείγμα (268 μονάδες ομφαλοπλακουντιακού αίματος) που έχει χρησιμοποιηθεί έως σήμερα, ξεκαθάρισε οριστικά το νεφελώδες τοπίο το οποίο είχε προκύψει από δημοσιεύσεις μελετών (σε μικρά δείγματα) με αντικρουόμενα αποτελέσματα. -Διαπιστώθηκε η ευκολία απομόνωσης MSCs από τη Βαρτόνειο γέλη του ομφαλίου λώρου. Τα κύτταρα αυτά παρουσιάζουν διαφορές στη δυνατότητα διαφοροποίησής τους σε σχέση με αυτά των ενηλίκων, διαφορές οι οποίες σχετίζονται με την εμβρυική προέλευσή τους και την «νεαρή» τους ηλικία. -Αποδείχθηκε ότι τα MSCs του ομφαλίου λώρου μπορούν να αποικήσουν ικριώματα κολλαγόνου και θα μπορούσαν πιθανότατα να χρησιμοποιηθούν ως εναλλακτική λύση των κατασκευών ιστομηχανικής που έως τώρα στηρίζονται αποκλειστικά στην προμήθεια πιο «γηρασμένων» MSCs από ιστούς ενηλίκων. -Σχεδιάσθηκε μελετήθηκε και επικυρώθηκε ένα πρωτόκολλο απομόνωσης και κρυοσυντήρησης των κυττάρων αυτών με σκοπό τη μελλοντική τους χρήση για κλινικές εφαρμογές, γεγονός που επέτρεψε τη δημιουργία της πρώτης Τράπεζας Μεσεγχυματικών Κυττάρων του ομφαλίου λώρου στο Ίδρυμα Ιατροβιολογικών Ερευνών της Ακαδημίας Αθηνών. -Διαπιστώθηκε ότι η καλλιέργεια εμβρυικών μεσεγχυματικών βλαστικών κυττάρων μπορεί να γίνει χωρίς τη χρήση ζωικών ορών, χρησιμοποιώντας, για πρώτη φορά, ως υποκατάστατο τον ορό ομφαλοπλακουντιακού αίματος.
