6.0 Περίληψη στα Ελληνικά Εισαγωγή: Ο στόχος της συγκεκριμένης προοπτικής μελέτης ήταν να εκτιμηθεί η αρτηριακή πίεση και οι δείκτες αρτηριακής σκληρίας πριν και μετά την χημειοθεραπεία σε τρεις υποομάδες ασθενών με μεταστατικό καρκίνο παχέος εντέρου, μεταστατικό καρκίνο νεφρού και στρωματικό καρκίνο γαστρεντερικού καθώς και να διερευνηθεί η πιθανή επίδραση της χημειοθεραπείας, των βιολογικών παραγόντων και πιθανής καρδιοτοξικότητας.Υλικό και Μέθοδοι: 171 ασθενείς συμπεριελήφθησαν στην μελέτη; 60 με μεταστατικό καρκίνο νεφρού, 18 με στρωματικό καρκίνο γαστρεντερικού και 93 ασθενείς με μεταστατικό καρκίνο παχέος εντέρου. Όλοι οι ασθενείς υποβλήθηκαν σε πλήρη κλινικοεργαστηριακό έλεγχο πριν και μετά την χημειοθεραπεία. Η αρτηριακή σκληρία εκτιμήθηκε πριν και μετά την χημειοθεραπεία με τον δείκτη ανακλώμενων κυμάτων pulse wave velocity (Complior) και τον δείκτη Augmentation Index (Sphygmocor).Αποτελέσματα: Οι ασθενείς σε όλους τους τρεις τύπους κακοήθειας είχαν σημαντικά υψηλότερες τιμές (p<0.001) των PWVc-r, PWVc-f και Aix μετά την χημειοραπεία που παρέμειναν σημαντικά υψηλές μετά από διόρθωση για την αρτηριακή πίεση και δείκτη μάζας σώματος. Ο δείκτης Aix παρουσίασε υψηλότερη μεταβολή στους ασθενείς με καρκίνο παχέος εντέρου σε σύγκριση με τους ασθενείς με καρκίνο νερφού και στρωματικό καρκίνο γαστρεντερικού (Median=3.6, 1.75 and 1.4, respectively; p<0.001). Η πολυπαραγοντική ανάλυση έδειξε ότι οι ασθενείς με υψηλότερα επίπεδα συστολικής, διαστολικής αρτηριακής πίεσης, κλάσματος εξωθήσεως και PWVc-f πριν την χημειοθεραπεία παρουσίασαν μικρότερες μεταβολές μετά την χημειοθεραπεία ενώ κάτι τέτοιο δεν παρατηρήθηκε με τον δείκτη Aix75.149Συμπεράσματα: Υπάρχει μια ξεκάθαρη επιβάρυνση στους δείκτες αρτηριακής σκληρίας σε ασθενείς που υποβάλλονται σε χημειοθεραπεία για μετατστικό καρκίνο νεφρού, παχεός και στρωματικό καρκίνο γαστρεντερικού ανεξάρτητα από τα επίπεδα αρτηριακής πίεσης και άλλους παράγοντες.
