Ο όρος εμβιομηχανική αναφέρεται σε όλους τους νόμους και τις αρχές της επιστήμης του μηχανικού που βρίσκουν εφαρμογή στη βιολογία και την ιατρική για να ερμηνεύσουν τη συμπεριφορά των ζωντανών οργανισμών. Για την εφαρμογή της εμβιομηχανικής απαιτείται η κατανόηση της δομής, της ανατομίας, της λειτουργίας, και της παθοφυσιολογίας του υπό εξέταση ιστού στη συγκεκριμένη περίπτωση του ουρητήρα. Μέχρι τώρα ο ουρητήρας έχει μελετηθεί σε πολύ μικρό βαθμό, παρά τη σημαντική εμβιομηχανική έρευνα που έχει γίνει σε άλλους βιολογικούς ιστούς και ενώ υπάρχει μεγάλο κίνητρο για την εισαγωγή των εμβιομηχανικών ιδιοτήτων του ουρητήρα σε μαθηματικά μοντέλα ανάλυσης της λειτουργίας του. Η παρούσα διατριβή έχει ως σκοπό τον προσδιορισμό των εμβιομηχανικών ιδιοτήτων των ουρητήρων του ανθρώπου, σε σχέση με την ηλικία και το φύλο, ενώ χωρίζεται σε 2 ενότητες.Στην πρώτη ενότητα χρησιμοποιήθηκαν 12 ζεύγη ανθρώπινων ουρητήρων (πτωματικό υλικό) (έξι άντρες, ηλικίας 54±9 ετών και έξι γυναίκες, ηλικίας: 55±8 ετών) για πειράματα διόγκωσης/έκτασης και για την ιστολογική ανάλυση της περιεκτικότητας του τοιχώματος του ουρητήρα σε κολλαγόνο και του πάχους αυτού (και των επιμέρους στοιβάδων του), ενώ ταυτόχρονα πραγματοποιήθηκε γεωμετρική ανάλυση των καταστάσεων μηδενικής τάσης και μηδενικής φόρτισης στα τρία τμήματα του ουρητήρα (άνω, μέσο και κάτω τμήμα). Τα πειραματικά αποτελέσματα προσομοιώθηκαν με το μαθηματικό μοντέλο τύπου Fung. Στη δεύτερη ενότητα της διατριβής χρησιμοποιήθηκαν πτωματικά δείγματα ολόκληρου του ουρητήρα από 21 άτομα. Τα δείγματα αυτά χρησιμοποιήθηκαν για τη μελέτη της αξονικής καμπυλότητας του ιστού και των παραμενουσών παραμορφώσεων στις καταστάσεις μηδενικής φόρτισης και μηδενικής τάσης σε όλο το μήκος του και συγκεκριμένα σε 15 διαφορετικές θέσεις. Συνοπτικά προκύπτει ότι: 1) Δεν υπάρχουν σημαντικές διαφορές μεταξύ του δεξιού και του αριστερού ουρητήρα σε κάποια ιδιότητα του ιστού (p>0.05). 2) Διαπιστώνεται τοπική ετερογένεια που σχετίζεται σημαντικά με την ηλικία αλλά σπάνια με το φύλο. 3) Στα νέα άτομα, οι καμπύλες αξονικής τάσης-περιφερειακής παραμόρφωσης του άνω τμήματος του ουρητήρα μετατοπίζονται σε μικρότερες τάσεις και η παράμετρος του μοντέλου Fung που αναπαριστά την αξονική δυσκαμψία είναι μικρότερη (p<0.05). 4) Το άνω τμήμα του ουρητήρα υφίσταται αύξηση της αξονικής δυσκαμψίας του σε προχωρημένη ηλικία. 5) Με τη γήρανση αυξάνεται (p<0.05) η περιεκτικότητα σε κολλαγόνο του άνω τμήματος του ουρητήρα ευνοώντας μια πιο ομοιόμορφη κατανομή κατά μήκος του. 6) Το πάχος του τοιχώματος του ουρητήρα αυξάνεται με την ηλικία, ενώ δεν είναι σταθερό κατά το μήκος του, (αυξάνεται από το άνω προς το μέσο τμήμα και ακολούθως μειώνεται προς το κάτω τμήμα), αντίθετα με ό,τι συμβαίνει με τη γωνία ανοίγματος και τις παραμένουσες παραμορφώσεις. 7) Παρατηρείται ότι η καμπυλότητα του ουρητήρα στην κατάσταση μηδενικής φόρτισης αυξάνεται κατά το πρώτο ήμισυ του ουρητήρα, και μειώνεται κατά το δεύτερο ήμισυ. 8) Τέλος τo φύλο επηρεάζει ελάχιστα (p>0.05) τη γωνία ανοίγματος και τη μορφομετρία των καταστάσεων μηδενικής φόρτισης και μηδενικής τάσης. Τα εμβιομηχανικά, μορφομετρικά και ιστολογικά δεδομένα που παρουσιάζονται στην παρούσα διατριβή προσφέρουν σημαντικές πληροφορίες αναφορικά με τις εμβιομηχανικές ιδιότητες του ουρητήρα υπό συνθήκες όχι μόνο φυσιολογικών τάσεων αλλά και στην κατάσταση άνευ τάσεων/φορτίων, επεξηγούν δε τη σχέση γήρανσης-αναδιαμόρφωσης του οργάνου. Τα δεδομένα αυτά είναι εξόχως αναγκαία σε μοντέλα πεπερασμένων στοιχείων, που θα προσομοίωσουν τη φυσιολογική και παθολογική λειτουργία του ουρητήρα.
