Ο αγγειακός ενδοθηλιακός αυξητικός παράγοντας (VEGF) θεωρείται σημαντικός αγγειογενετικός παράγοντας στο άσθμα. Τα κυστεινικά λευκοτριένια (Cyst-LTs)εμπλέκονται στην αγγειακή διαπερατότητα στο άσθμα. Οι ανταγωνιστές των υποδοχέων των κυστεινικών λευκοτριενίων επηρεάζουν την αγγειακή διαπερατότητα μειώνοντας την έκφραση του VEGF. ΣΚΟΠΟΣ: Ο προσδιορισμός των επιπέδων του VEGF και των κυστεινικών λευκοτριενίων στο υπερκείμενο πτυέλων ασθενών με άσθμα και η πιθανή συσχέτιση μεταξύ τους καθως και με τον δείκτη αγγειακής διαπερατότητας (AVP index). ΜΕΘΟΔΟΣ: Μελετήθηκαν 121 ασθενείς με άσθμα (38 με σοβαρό ανθιστάμενο άσθμα, 41 καπνιστές) και 30 υγιείς (15 καπνιστές). Όλοι υπεβλήθησαν σε πρόκληση πτυέλων για προσδιορισμό του τύπου κυττάρων και μέτρηση VEGF και κυστεινικών λευκοτριενίων στο υπερκείμενο πτυέλων. Εκτιμήθηκε και ο δείκτης AVP. ΣΥΜΠΕΡΑΣΜΑΤΑ: Τα επίπεδα του VEGF και των Cyst-LTs (pg/ml) ήταν σημαντικά αυξημένα σε ασθενείς με άσθμα, συγκριτικά με την ομάδα ελέγχου (median, interquartile ranges 845 [487-1034] vs. 432(327-654) and 209 [171-296] vs. 92 [75-114] αντίστοιχα, p < 0.001), με σημαντική συσχέτιση μεταξύ των επιπέδων των κυστεινικών λευκοτριενίων με τα επίπεδα του VEGF στο υπερκείμενο πτυέλων και τον δείκτη AVP. Παρόμοια θετική συσχέτιση παρατηρήθηκε ανάμεσα στα επίπεδα του VEGF στο υπερκείμενο πτυέλων και του δείκτη AVP.Η παρουσία σοβαρού ανθιστάμενου στην θεραπεία άσθματος ήταν ένας σημαντικός παράγοντας και για τις δύο συσχετίσεις. ΣΥΜΠΕΡΑΣΜΑ: Τα αποτελέσματά μας δείχνουν ότι τα κυστεινικά λευκοτριένια μπορεί να τροποποιήσουν την αγγειακή διαπερατότητα μέσω αύξησης της έκφρασης του VEGF. Η παραπάνω δράση φαίνεται να επηρεάζεται από την βαρύτητα του άσθματος.
