Η Στένωση του Ισθμού της Αορτής αποτελεί μια συγγενή αγγειακήνόσο που αντιπροσωπεύει το 5-10% των συγγενών καρδιοπαθειών. Μετά από αρκετά χρόνια εφαρμογής διαφόρων τεχνικών, σήμερα η άμεσηχειρουργικήαντιμετώπισηαποτελείτηθεμέλιαλίθογιατηναποκατάσταση της ανωμαλίας αυτής.Έχει αποδειχθεί ότι η συγκεκριμένη νόσος έχει σοβαρά επακόλουθα.Η παραμένουσα υπέρταση, η επαναστένωση και οι αγγειακές διαταραχέςπουεκφράζονταιμετηνεμφάνισηκαρδιαγγειακώνσυμβαμάτωναποτελούν συχνά ευρήματα ακόμα και μετά από μια επιτυχή διόρθωση τηςβλάβης και χωρίς παραμένουσα υπέρταση.Το συγκεκριμένο φαινόμενο φαίνεται να αποδίδεται σε μηχανικέςκαι φλεγμονώδεις διαταραχές και αλλοιώσεις των αγγείων εγγύς της βλάβης, οι οποίες δεν φαίνεται να υποχωρούν ούτε μετά από μια επιτυχήεπέμβαση.Στοερευνητικόμέροςτηςπαρούσαςδιατριβήςαυτόεπιβεβαιώθηκε τόσο με τη μέτρηση φλεγμονωδών δεικτών όσο και με την χρήση πληθυσμογραφίας ροής και υπερήχων με τα οποία αποδείχθηκε η μηχανική διαταραχή των αγγείων αυτών.Ο κοινός αρτηριακός κορμός από την άλλη πλευρά αποτελεί μιαασυνήθη διαταραχήαφού αποτελεί κάτω από 3% των συγγενώνκαρδιοπαθειών.Καιεδώ ημακροπρόθεσμηπαρακολούθησηείναιεξέχουσας σημασίας,αφού η χρήση του ιδανικότερου μοσχεύματος γιατοποθέτηση στον χώρο εξώθησηςτης δεξιάς κοιλίας αποτελεί ένααντικείμενο που δεν έχει μελετηθεί σε βάθος χρόνου.Στοερευνητικότμήματηςδιατριβήςμελετήθηκανταμακροπρόθεσμα αποτελέσματα διόρθωσης κοινού αρτηριακού κορμού με τοποθέτηση ομοιμοσχευμάτων, τόσο αορτικών όσο και πνευμονικών. Το κομμάτιαυτότηςδιατριβήςεκπονήθηκεσεσυνεργασίαμετηνΚαρδιοχειρουργική κλινική του Southampton University Hospital.Παράλληλα γίνεται μια ανασκόπηση τηςσχετικής βιβλιογραφίαςπου αποδεικνύουν συνολικά την αναγκαιότητα της μακροπρόθεσμηςπαρακολούθησης των ασθενών αυτών.
