Τα γλυκοκορτικοειδή ανήκουν στην κατηγορία των στεροειδών ορμονών και ασκούν τις δράσεις τους μέσω του υποδοχέα γλυκοκορτικοειδών (GR). Οι ορμόνες αυτές ρυθμίζουν ποικίλες βιολογικές διαδικασίες, όπως το μεταβολισμό, την απόπτωση, τις ανοσολογικές αποκρίσεις, καθώς και την κυτταρική ανάπτυξη. Ο GR δρά ως μεταγραφικός παράγοντας, και ρυθμίζει θετικά ή αρνητικά την έκφραση πολλών γονιδίων στόχων μέσω της άμεσης σύνδεσής του με ειδικές ορμονοεξαρτώμενες αλληλουχίες νουκλεοτιδίων (trans ενεργοποίηση) ή μέσω αλληλεπίδρασής του με άλλους μεταγραφικούς παράγοντες και ρύθμισης της έκφρασης των γονιδίων στόχων αυτών (trans καταστολή). Τα γλυκοκορτικοειδή διαθέτουν αντιφλεγμονώδεις ιδιότητες, οι οποίες οφείλονται κυρίως στην trans καταστολή της μεταγραφής από τον GR και γι’αυτό χρησιμοποιούνται ευρέως στην ιατρική σε διάφορες ασθένειες πχ. ρευματοειδής αρθροίτιδα. Ωστόσο, η χρόνια και υψηλής δόση χρήση τους για θεραπευτικούς σκοπούς, οδηγεί παράλληλα σε πολλές αρνητικές παρενέργειες όπως διαβήτης, απώλεια μυικής μάζας, οστεοπόρωση. Για το λόγο αυτό σημαντική είναι η εύρεση διαφόρων ενώσεων αγωνιστών-ρυθμιστών του GR (SEGRAs, SEGRMs, SEGRAMs), που θα ενεργοποιούν επιλεκτικά τους μηχανισμούς της trans καταστολής του GR, ενώ δεν θα επηρεάζουν ή θα καταστέλλουν μηχανισμούς της trans ενεργοποίησηs του GR. Επιπρόσθετα, ο GR μπορεί να εισέρχεται στα μιτοχόνδρια και να ρυθμίζει α) τη μιτοχονδριακή μεταγραφή μέσω άμεσης σύνδεσης του σε ειδικές ορμονοεξαρτώμενες αλληλουχίες, εντός της D-Loop του μιτοχονδριακού γονιδιώματος, και β) μέσω αλληλεπίδρασής του με άλλους αποπτωτικούς ή αντιαποπτωτικούς παράγοντες. Στην παρούσα διδακτορική διατριβή εξετάστηκε μια σειρά τριτερπενοειδών ενώσεων (52 στο συνολό τους) ως προς την ικανότητά τους να ενεργοποιούν τη πυρηνική μετατόπιση του GR και να δρούν ως SEGRAs. Μεταξύ των τριτερπενοειδών ενώσεων που μελετήθηκαν ήταν και τα τετρακυκλικά τριτερπένια πρωτοπαναξαδιόλη (PPD) και πρωτοπαναξατριόλη (PPT), τα παράγωγα του μποσγουελλικού οξέος [11-κετο β-μποσγουελλικό οξύ (KBA), 3-O-Ακέτυλο-11-κετο β-μποσγουελλικό οξύ (AKBA), α-μποσγουελλικό οξύ (α-BA), β-μποσγουελλικό οξύ (β-BA), Ακέτυλο α-μποσγουελλικό οξύ (α-ABA), Ακέτυλο β-μποσγουελλικό οξύ (β-ABA)], η α-αμυρίνη και τα παράγωγα της, καθώς και τα παράγωγα του μπετουλινικού οξέος. Όσον αφορά τις ενώσεις PPD και PPT, καθώς και τα παράγωγα του μποσγουελλικού οξέος, υπολογιστικές μελέτες επαγόμενης πρόσδεσης (Induced Fit Docking), ανοσοϊστοχημείας, λουσιφεράσης και ανοσοαποτύπωσης (Western Blot) έδειξαν ότι οι συγκεκριμένες ενώσεις, άλλες σε μεγαλύτερο (KBA, AKBA, α-ΒΑ, PPT) και άλλες σε μικρότερο βαθμό (β-BA, α-ABA και β-ABA, PPD), μπορούν να προσδένονται στον GR, να ενεργοποιούν την πυρηνική του μετατόπιση, και να καταστέλλουν τη μεταγραφική δραστικότητα του NFκ-B, σε κύτταρα HeLa και HEK293, που εκφράζουν ενδογενώς τον GR. Η δράση αυτή δεν παρατηρήθηκε σε κύτταρα COS.7, τα οποία εκφράζουν μηδαμινά έως ελάχιστα επίπεδα GR, υποδεικνύοντας την εμπλοκή του GR στην επιτέλεση αυτών των δράσεων. Μάλιστα πειράματα λουσιφεράσης, στα οποία πραγματοποιήθηκε υπερέκφραση του GR, καταδεικνύουν ότι οι ενώσεις KBA και AKBA ασκούν αθροιστική δράση κατά την συνχορήγησή τους με την DEX, επιβεβαιώνοντας τη συμμετοχή τους στη GR σηματοδότηση. Επίσης, όλες οι ενώσεις που προαναφέρθηκαν είτε δεν επηρεάζουν την επαγόμενη από δεξαμεθαζόνη (DEX) μεταγραφική δραστικότητα του GR είτε την καταστέλλουν. Συνεπώς, τόσο οι ενώσεις PPD και PPT, όσο και τα παράγωγα του μποσγουελλικού οξέος μπορούν να χαρακτηριστούν ως SEGRAs και να αποτελέσουν μόρια οδηγούς για την ανάπτυξη νέων αποτελεσματικότερων υποκαταστατών κορτιζόλης με μειωμένες παρενέργειες. Επιπρόσθετα, οι ενώσεις αυτές μπορούν να επάγουν απόπτωση στις κυτταρικές σειρές HeLa και HepG2. Αξιοσημείωτο είναι και το γεγονός ότι η αποπτωτική δράση ενισχύεται κατά την συγχορήγηση των ενώσεων AKBA με α-BA, με DEX, γεγονός που πιθανόν να υποδηλώνει αθροιστική δράση των ενώσεων αυτών με την DEX. Όσον αφορά την α-αμυρίνη και τα παράγωγα της, καθώς και τα παράγωγα του μπετουλινικού οξέος, δεν διέθεταν ικανότητα επαγωγής της πυρηνικής μετατόπισης του GR, αλλά ούτε και την ικανότητα καταστολής της μεταγραφικής δραστικότητας του NF-κB σε σημαντικό βαθμό. Εξ αυτών, τα παράγωγα αμυρίνης παρουσίασαν καταστολή της από DEX επαγόμενης μεταγραφικής δραστικότητας του GR. Στη συνέχεια, δεδομένου του σημαντικού ρόλου των μιτοχονδρίων στη ρύθμιση του μεταβολισμού του κυττάρου τόσο υπό φυσιολογικές όσο και παθολογικές συνθήκες, αλλά και του γεγονότος ότι ο GR μπορεί να εισέρχεται στα μιτοχόνδρια και να επιδρά στη μιτοχονδριακή μεταγραφή και απόπτωση, διερευνήθηκε ο ρόλος και οι μηχανισμοί δράσης του μιτοχονδριακού υποδοχέα γλυκοκορτικοειδών. Για την επίτευξη του στόχου αυτού μελετήθηκε η αλληλεπίδραση του GR με πρωτεΐνες που παίζουν κύριο ρόλο στο μιτοχονδριακό μεταβολισμό, τη μιτοχονδριακή μεταγραφή, απόπτωση και μηχανισμών εισόδου πρωτεϊνών στα μιτοχόνδρια. Οι πρωτεΐνες οι οποίες εντοπίστηκαν να αλληλεπιδρούν με τον υποδοχέα γλυκοκορτικοειδών είναι οι εξής: οι πρωτεΐνες θερμικού σοκ με μοριακά βάρη 70 KDa (70 Kilodalton Heat Shock Protein, HSP70) και 90 KDa (90 Kilodalton Heat Shock Protein, HSP90), ο μεταγραφικός παράγοντας που αποτελεί υποδοχέα που ενεργοποιείται από παράγοντες που επάγουν τον πολλαπλασιασμό των υπεροξεισωμάτων (Peroxisome proliferator-activated receptor alpha, PPARα), η αφυδρογονάση του πυροσταφυλικού οξέος (Pyruvate dehydrogenase, PDH), ο τασοεξαρτώμενος δίαυλος ανιόντων (Voltage dependent anion channel, VDAC), ο μιτοχονδριακός μεταγραφικός παράγοντας MtfA, καθώς και η αντιαποπτωτική πρωτεΐνη BCl2. Μεταξύ αυτών ισχυρή αλληλεπίδραση παρουσιάζουν οι πρωτεΐνες PDH, HSP70 και PPARα. Λόγω του κομβικού ρόλου της PDH στη ρύθμιση του ενεργειακού μεταβολισμού, η αλληλεπίδραση του GR με την PDH αξιολογήθηκε ιδιαίτερα σημαντική και επαληθεύτηκε περαιτέρω τόσο με χρήση διαφορετικών αντισωμάτων ανοσοκατακρήμνισης έναντι του GR όσο και με χρήση αντισωμάτων έναντι της PDH. Η μελέτη πραγματοποιήθηκε τόσο υπό συνθήκες υπερέκφρασης του GR όσο και υπό συνθήκες ύπαρξης ενδογενών επιπέδων έκφρασης του mtGR. Η αλληλεπίδραση του μιτοχονδριακού GR με την αφυδρογονάση του πυροσταφυλικού οξέος (PDH), ένα ένζυμο κλειδί στον καρκίνο, καθώς είναι υπεύθυνη για τον επαναπρογραμματισμό του μεταβολισμού των καρκινικών κυττάρων, μας ώθησε στη διερεύνηση του ρόλου του mtGR στην καρκινογένεση. Για την επίτευξη του σκοπού αυτού πραγματοποιήθηκαν in vivo πειράματα καρκινογένεσης σε ανοσοκατεσταλμένους μύες NOD-SCID οι οποίοι είχαν έλλειψη B- και T- λεμφοκυττάρων, καθώς και σε μύες NSG, οι οποίοι είχαν έλλειψη των Β-, T- και των NK (Natural Killer cells)-κυττάρων. Οι μύες εμβολιάστηκαν με κύτταρα από τις σταθερές κυτταρικές σειρές HepG2mtGFP ή HepG2mtGFPGR, οι οποίες υπερεκφράζουν σταθερά την πράσινη φθορίζουσα πρωτεΐνη (GFP) ή την χιμαιρική GFPGR πρωτεΐνη, αντίστοιχα, με μιτοχονδριακή στόχευση. Η χρονική στιγμή εμφάνισης των όγκων στους μύες που είχαν εμβολιαστεί με τα κύτταρα HepG2mtGFPGR προηγούνταν αυτής σε πειραματόζωα που είχαν εμβολιαστεί με HepG2mtGFP. Επιπρόσθετα, τόσο ο ρυθμός ανάπτυξης όσο και το μέγεθος των όγκων, το οποίο μετρήθηκε όταν οι όγκοι αφαιρέθηκαν χειρουργικά από τους μύες, ήταν πολύ μεγαλύτεροι στους όγκους των κυττάρων HepG2mtGFPGR, σε σύγκριση με αυτούς των κυττάρων αναφοράς HepG2mtGFP. Με σκοπό τη διερεύνηση των μηχανισμών μέσω του οποίου ο mtGR οδηγεί στην ανάπτυξη όγκων πραγματοποιήθηκαν περαιτέρω μελέτες σε ολικά και μιτοχονδριακά κλάσματα από τους προαναφερθέντες όγκους, ελέγχοντας τα πρωτεϊνικά επίπεδα μορίων που συμμετέχουν στην απόπτωση, την αυτοφαγία, αναπνευστική αλυσίδα-οξειδωτική φωσφορυλίωση, τον κύκλο του κιτρικού οξέος τη γλυκονεογένεση, καθώς και μόρια με τα οποία αλληλεπιδρά ο υποδοχέας γλυκοκορτικοειδών. Από τα πειράματα που εφαρμόστηκαν, εξήχθη το συμπέρασμα ότι ο μιτοχονδριακός υποδοχέας γλυκοκορτικοειδών τόσο μέσω της αλληλεπίδρασής του με την PDH όσο και μέσω της εμπλοκής του στη ρύθμιση της μιτοχονδριακής μεταγραφής καταστέλλει την έκφραση των υπομονάδων ενζύμων της αναπνευστικής αλυσίδας-οξειδωτικής φωσφορυλίωσης και προκαλεί μεταβολές στα επίπεδα ενζύμων του κύκλου του κιτρικού οξέος, συντελώντας έτσι στον επαναπρογραμματισμό του μεταβολισμού των καρκινικών κυττάρων, ενισχύοντας τη γλυκόλυση και μειώνοντας τη γλυκονεογένεση. Σημαντικό ρόλο στην όλη διαδικασία διαδραματίζει και η αυτοφαγία, η οποία ενεργοποιείται ενισχύοντας έτσι την παραγωγή πρόδρομων μορίων, απαραίτητων για την ανάπτυξη των νεοσυντιθέμενων καρκινικών κυττάρων. Επίσης, παρουσία του mtGR, δεν ενεργοποιήθηκαν μηχανισμοί επαγωγής μιτοχονδριακά εξαρτώμενης απόπτωσης, σε συμφωνία με την ανταγωνιστική δράση απόπτωσης και αυτοφαγίας κατά την καρκινογένεση. Επιπρόσθετα, στην παρούσα διδακτορική διατριβή, με σκοπό την περαιτέρω διερεύνηση του ρόλου του μιτοχονδριακού GR υπό συνθήκες διαθεσιμότητας οξυγόνου, μελετήθηκε η διαφορική ορμονοεξαρτώμενη έκφραση πρωτεινών που εμπλέκονται στη ρύθμιση της μιτοχονδριακής λειτουργίας σε κύτταρα HepG2, HepG2mtGFP και HepG2mtGFPGR. Τα αποτελέσματα της μελέτης αυτής έδειξαν ότι η επίδραση του mtGR στη ρύθμιση της μιτοχονδριακής μεταγραφής και βιοσύνθεση OXPHOS εξαρτάται από την παρουσία της ορμόνης και το χρόνο έκθεσης σε αυτήν. Η αρχικά παρατηρούμενη ορμονοεξαρτώμενη επαγωγή της σύνθεσης της υπομονάδας II της οξειδάσης του κυτοχρώματος c αντιστρέφεται με το χρόνο, ενώ η κατασταλτική δράση του mtGR στα επίπεδα της PEPCK λαμβάνει χώρα ανεξαρτήτων συνθηκών. Επίσης, δεδομένου ότι δεν παρατηρήθηκαν αλλαγές σε αυτοφαγικούς παράγοντες, υποδεικνύεται ότι η παρουσία του mtGR στα μιτοχόνδρια δεν αποτελεί ικανή και αναγκαία συνθήκη για την επαγωγή αυτοφαγίας. Με βάση όλα τα παραπάνω, συμπεραίνουμε ότι η αλληλεπίδραση του mtGR με την PDH φαίνεται να παίζει κυρίαρχο ρόλο στη ρύθμιση του μιτοχονδριακού μεταβολισμού. Η αλληλεπίδραση αυτή πιθανόν να οδηγεί σε καταστολή της δραστικότητας του ενζύμου, καταστολή της γλυκονεογένεσης και επαγωγή του μεταβολικού επαναπρογραμματισμού όπως περιγράφεται στο φαινόμενο Warburg. Eνισχυτικά σε αυτήν την δράση δρά και η ρύθμιση της μιτοχονδριακής μεταγραφής από τον mtGR. Τα αποτελέσματα μας συνεισφέρουν στην κατανόηση του ρόλου του mtGR και στην ανάδειξη νέων φαρμακευτικών στόχων για την αντιμετώπιση του καρκίνου.
