ΣΚΟΠΟΣ: Σκοπός της παρούσας μελέτης αποτελεί η δευτεροπαθής μεταβολή της έκφρασης της δυσφερλίνης σε ασθενείς με μυϊκή δυστροφία Duchenne (DMD) και η αναζήτηση του πρωτοπαθούς πρωτεϊνικού ελλείμματος σε ασθενείς με ζωνιαία μυϊκή δυστροφία (LGMD). ΜΕΘΟΔΟΙ: Για τη μελέτη της δευτεροπαθούς μεταβολής της έκφρασης της δυσφερλίνης στην DMD χρησιμοποιήθηκαν 20 βιοψίες μυός ενώ για τη μελέτη της πρωτοπαθούς μεταβολής της έκφρασης πρωτεϊνών σε ασθενείς με LGMD χρησιμοποιήθηκαν 87 βιοψίες μυός. Η μελέτη της έκφρασης των πρωτεϊνών πραγματοποιήθηκε με ανοσοϊστοχημικές μεθόδους και κάθετη ηλεκτροφόρηση. ΑΠΟΤΕΛΕΣΜΑΤΑ: Η ανοσοϊστοχημική έκφραση της δυσφερλίνης στο σαρκείλημμα ήταν φυσιολογική, μειωμένη ή αυξημένη σε σχέση με τους μάρτυρες ενώ σε όλες τις βιοψίες ανεδείχθη διαφορετικού βαθμού ενδοκυττάρια εντόπιση της πρωτεΐνης, κυμαινόμενη από ήπια έως έντονη. Σύμφωνα με τα αποτελέσματα της ανοσοηλεκτροφόρησης, η δυσφερλίνη βρέθηκε να έχει αυξημένη έκφραση στις 15 από τις 20 βιοψίες που μελετήθηκαν. Επιπλέον, η ποσότητα της δυσφερλίνης ήταν ανάλογη του αριθμού των ινών με αυξημένη σαρκειλημματική ανάδειξη της πρωτεΐνης. Το πρωτοπαθές πρωτεϊνικό έλλειμμα προσδιορίστηκε σε 42 ασθενείς με LGMD (48%). Η σχετική συχνότητα των μορφών LGMD στην Ελλάδα είναι: καλπαϊνοπάθεια (LGMD2A) 26%, γ-σαρκογλυκανοπάθεια (LGMD2C) 8%, καβεολινοπάθεια (LGMD1C) 6%, δυσφερλινοπάθεια (LGMD2B) 6%, α- σαρκογλυκανοπάθεια (LGM2D) 1% και β-σαρκογλυκανοπάθεια (LGMD2E) 1%. ΣΥΜΠΕΡΑΣΜΑΤΑ: Τα δεδομένα αυτά υποδεικνύουν ότι η έκφραση της δυσφερλίνης μεταβάλλεται στη μυϊκή δυστροφία Duchenne, αν και δεν αποτελεί μέρος του συμπλέγματος δυστροφίνης- γλυκοπρωτεϊνών. Η καλπαϊνοπάθεια (LGMD2A) αποτελεί τη συχνότερη μορφή ζωνιαίας μυϊκής δυστροφίας στον Ελληνικό πληθυσμό και θα πρέπει να συμπεριλαμβάνεται στα αρχικά στάδια της διαγνωστικής προσέγγισης ενός ασθενούς με ζωνιαία μυϊκή δυστροφία.
