We evaluated changes in relative liver enhancement (RLE) obtained by gadoxetic acid-enhanced MRI (GA-MRI) in the hepatobiliary phase and changes in splenic volume (SV) after hepatitis C virus (HCV) eradication as well as their predictive value for the development of (further) hepatic decompensation during follow-up. This retrospective study comprised 31 consecutive patients with HCV-induced advanced chronic liver disease who underwent GA-MRI before and after successful interferon-free treatment, as well as a cohort of 14 untreated chronic HCV-patients with paired GA-MRI. RLE increased by 66% (20%–94%; P < 0.001) from pre- to posttreatment, while SV decreased by −16% (−28% to −8%; P < 0.001). However, SV increased in 16% (5/31) of patients, the identical subjects who showed a decrease in RLE (GA-MRI-nonresponse). We observed an inverse correlation between the changes in RLE and SV (ρ=−0.608; P < 0.001). In the untreated patients, there was a decrease in RLE by −11% (−25% to −3%; P=0.019) and an increase in SV by 23% (7%–43%; P=0.004) (both P < 0.001 versus treated patients). Interestingly, GA-MRI-nonresponse was associated with a substantially increased risk of (further) hepatic decompensation 2 years after the end of treatment: 80% versus 8%; P < 0.001. GA-MRI might distinguish between individuals at low and high risk of (further) hepatic decompensation (GA-MRI-nonresponse) after HCV eradication. This could allow for individualized surveillance strategies.
Amaç: Osteoid osteoma, sık görülen ve benign karakterde bir kemik tümörüdür. Hastalar, rahatsız edici gece ağrıları ile ortopedi kliniğine başvurur ve çoğu tanı konulduğunda uzun süreli şikayetlere sahiptir. Tedavisinde konservatif izlem yapılabileceği gibi cerrahi tedavi akut olarak ağrının kesilmesinde ve tümörün vücuttan uzaklaştırılmasında daha etkilidir. Son 20 yıl içerisinde özellikle cerrahi ulaşım açısından riskli konumda bulunan bölgelerde perkütan ablasyon tedavisi minimum risk içeriği ile güvenle kullanılmaktadır. İşlem öncesi radyologların bir kısmı nidustan biyopsi aldıktan sonra ablasyon yaparken bir kısmı ise direkt ablasyon işlemini gerçekleştirmektedir. Çalışmanın amacı osteoid osteoma tanısı ile perkütan ablasyon yapılmasına karar verilen hastalarda biyopsi sonucunun etkinliği ve kliniğe yansımasını değerlendirmektir. Yöntemler: Retrospektif olarak planlanan çalışmaya Ocak 2013-Aralık 2016 arasında kliniğimizde osteoid osteoma tanısı ile perkütan ablasyon yapılan 10 hasta dahil edildi. Tüm hastalara girişim öncesi biyopsi yapılmıştı. İşlem öncesi biyopsi yapılmayan hastalar, takipten çıkan hastalar ve biyopsi sonucunda osteoid osteoma tanısı dışında tanı alan hastalar çalışmadan çıkarıldı. Hastaların biyopsi sonuçları ve görüntüleri retrospektif olarak elde edildi. Biyopsi sonucu pozitif ve negatif olan hastaların klinik sonuçları, nüks oranları ve maliyetleri karşılaştırıldı. Bulgular: Çalışmaya dahil edilen 10 hastanın ortalama yaşı 17,3 (4-35) idi. Tüm hastalar en az iki yıl süre ile takip edilerek nüks edip etmediği ve şikayetlerinin devam edip etmediği telefon ile sorgulandı. Yedi hastada biyopsi sonucu osteoid osteoma tanısı koyduran nidus görülürken, 3 hastada ise nidus görüntüsü görülmedi. Tüm hastalarda iki yıllık takip içerisinde nüks görülmezken, hiçbir hastada işlem sonrası sebat eden ağrı görülmedi. İstatistiksel olarak nidus görülen ve görülmeyen hastalar arasında klinik ve nüks açısından anlamlı fark görülmedi. Sonuç: Osteoid osteoma tedavisinde perkütan ablasyon etkin olarak kullanılabilecek ve yüksek oranlarda kür sağlama potansiyeli olan bir yöntemdir. İşlem öncesi yapılan biyopsinin pozitif veya negatif oluşunun, hastaların klinik gidişi ve nüks üzerinde herhangi bir etkisi yoktur. In this study, types and quantities of marine litter in Urla (İzmir) Kum Denizi Beach and Demircili Koyu Beach were determined seasonally between October 2017 and October 2018. At the beaches, marine litter larger than 2,5 centimetres were collected with 3 people along 2-meter lines, which are determined to be parallel to the sea. The collected litters were classified according to the United Nations Environment Program (UNEP, 2009), quantities were determined and the pollution status of the beaches was evaluated according to the Clean Coast Index. A total of 1,265 litter from Kum Denizi Beach and 2,099 litter from Demircili Koyu Beach were collected. In total, 3,364 pieces of litter weighing 90.81 kg were collected from the two beaches. Among the litters collected, plastic litter was found to be dominant with 2,794 pieces. This was followed by 145 metals, 141 papers, 110 fabrics, 87 foam plastics, 42 wood, 32 glass, 8 other litters and 5 rubber. Given the seasonal averages of the clean coast index values; Kum Denizi Beach was found to be clean and Demircili Koyu Beach was found to be moderately polluted.
