Doğuştan gelen işitme kaybının kişi üzerinde yaratacağı olumsuz etkileri azaltmak veya ortadan kaldırmak için erken tanı, cihazlanma ve eğitimin önemi kabul edilmiş durumdadır. Günümüzde erken tanı ve müdahale denildiğinde belirlenen standart bebeğin doğumdan sonraki 3 ay içinde tanı sürecinin tamamlanması; 6 aylık olduğunda da aile merkezli programlarda eğitiminin başlamasıdır. Bu amacı gerçekleştirmek üzere dünyanın birçok ülkesinde yenidoğan işitme tarama programları uygulanmaktadır. Bu derleme çalışmasında yenidoğan işitme tarama programlarına ilişkin bilgi verilerek Türkiye'deki yenidoğan işitme tarama programının bebeklerin tanılama, cihazlandırma ve aile eğitimine başlama yaşları üzerindeki etkileri özetlenmiştir. Ülkemizde bu alanda yapılan araştırmalar incelendiğinde yenidoğan işitme tarama programıyla tanı yaşının önceki yıllara göre önemli ölçüde aşağı çekildiği, ancak cihazlanma ve eğitime başlama yaşına dair bir kanıya ulaşmak için araştırma bulgularının sınırlı olduğun gözlenmektedir.
Öz. Durum çalışması olarak desenlenen bu araştırmanın amacı, bir ileri tanı merkezinde yenidoğan işitme taramasından eğitime yönlendirmeye kadar olan süreci incelemek ve bu sürece ilişkin sağlık çalışanları ile birlikte ailelerin deneyimlerini irdelemektir. Bu araştırmaya, ileri tanı merkezinden dört odyometrist, bir uzman odyolog, bir kulak burun boğaz hekimi ve çocuğu bu ileri tanı merkezinde tanılanan yedi aile katılmıştır. Bu araştırmanın verileri; aile ve sağlık çalışanları ile yapılan görüşmelerle, merkezde yapılan gözlemlerle, belgelerle, süreç ürünleriyle ve araştırmacı günlüğü yoluyla toplanmıştır. Toplanan veriler, tümevarımsal olarak analiz edilmiştir. Analizden elde edilen, bulgular, dört tema çerçevesince raporlaştırılmıştır. Araştırma sonucunda, ileri tanı merkezlerinin ve işitme kayıplı çocuklar için eğitim veren devlet kurumlarının arttırılmasına ihtiyaç duyulduğu belirlenmiştir. Ailelerin ise süreçte psikolojik ve ekonomik olarak desteklenmeye ihtiyaç duyduğu ortaya çıkmıştır. Sürecin daha etkin bir şekilde işlemesi için, sağlık çalışanlarına geri bildirim veren bir takip sistemine ve işitme kayıplı çocukların eğitimi alanında çalışanların sağlık çalışanları ile işbirliğinin arttırılmasına ihtiyaç duyulmaktadır.
Bu çalışmada koklear implant ve işitme cihazı kullanan çocukların dil ve dinleme becerilerini değerlendirmek amaçlanmıştır. Çalışmaya katılan çocukların 18'i koklear implant, 7'si işitme cihazı kullanmaktadır. Çocukların yaşları 43-84 ay arasında değişmektedir. Ortalama implant kullanma süresi 27 ay, cihaz kullanma süresi 39 aydır. Çocukların tamamı aynı eğitim kurumuna devam etmekte olup sözel eğitim almaktadırlar. Konuşmayı algılama becerilerini ölçmek için ilk konuşmayı algılama testi (İKA) ve Hacettepe fonetik dengeli iki ve tek heceli kelimeler kullanılmıştır. Dil gelişimleri "Test of Early Language Development-3" (TELD-3) Türkçe uyarlaması kullanılarak değerlendirilmiştir. Test sonuçlarına göre işitme cihazı kullanan çocuklar ile koklear implant kullanan çocukların dil performansları arasında anlamlı bir farklılık yokken, açık set kelime ayırt etme becerileri açısından koklear implantlı çocukların performansının işitme cihazı kullanan yaşıtlarından daha yüksek olduğu görülmektedir. Dil ve işitsel ayırt etme performansları ile implant kullanım süresi ve aile eğitimine başlama yaşı arasında anlamlı ilişki olduğu bulunmuştur. Her iki grupta performansı ortalamadan farklılık gösteren çocuklar bireysel olarak tartışılmıştır.Anahtar kelimeler: Dil becerisi, dinleme becerisi, işitme cihazı, işitme kayıplı çocuklar, koklear implant SUMMARY Evaluation of language and listening skills of the children with hearing loss who use cochlear implants and hearing aidsIn this study it was aimed to evaluate language and listening skills of children who were using cochlear implants and hearing aids. Of the children who participated in the study, 18 and 7 were using cochlear implants and hearing aids, respectively. Ages of the children were between 43 to 84 months. Mean durations of implant use and hearing aid use were 27 and 39 months, respectively. All the children were attending to the same education institute, and taking verbal education. Early speech perception test (ESP) and Hacettepe phonetically balanced two and single syllable word tests were used to assess speech perception skills. Language development was assessed by using Test of Early Language Development (TELD-3) Turkish version. According to the test results no significant differences were observed between the language development performances of children using hearing aid and cochlear implant, whereas performance of children with implant was better than that of children with hearing aid with respect to open set speech perception skills. A significant correlation was observed between the language and speech perception skills, and duration of implant use and the age of starting family education. Children with a performance different from the mean in each group were discussed individually.
The present study aimed to analyze the effects of newborn hearing screening program in Turkey based on the age of diagnosis, amplification and initiation of education between 2000 and 2017. The study was designed as a retrospective study. The files of 997 children registered with a research and education center for children with hearing loss were analyzed. The findings indicated that the age of diagnosis, amplification and initiation of education significantly decreased after the national newborn hearing screening program was introduced throughout the years. However, the recommended universal standards were achieved only for a minority of children in terms of timely diagnosis and early intervention. Delay in amplification mostly observed in children with mild-moderate loses and late diagnosis, was found as related to the testing in more than two different clinics. The findings on late initiation of education were discussed based on the reports in relevant literature and monitoring problems after the diagnosis and the problems observed in early education system in Turkey.
