Özet: Osteoporoz, toplumda yaygın görünen, kemik mikro mimarisinde bozulma ve düşük kemik kütlesi ile karakterize sistemik bir hastalıktır. Çalışmamızın amacı paraoksonaz (PON1) geninin 584A>G(Gln192Arg) ve 172T>A(Leu55Met) polimorfizmleri ile postmenopozal osteoporoz ile ilişkisinin olup olmadığını saptamaktır. Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Fiziksel Tıp ve Rehabilitasyon Anabilim Dalı Kliniği'nde takipli 143 postmenopozal osteoporozu olan ve 102 sağlıklı kadın çalışmaya dahil edildi. Çalışma retrospektif olarak planlandı. Genotipler alel-spesifik DNA primeri ve floresan özellikli DNA probu kullanılarak qPCR cihazında (The Applied Biosystems® StepOne™) belirlendi. PON1 584A>G polimorfizminin genotip ve alel frekanslarında gruplar arasında istatistiksel olarak anlamlı bir fark bulunmamıştır (p=0.362 ve p=0.318). PON1 172T>A polimorfizminin genotip ve alel frekansları karşılaştırıldığında ise, sağlıklı bireylere göre postmenopozal osteoporozu olanlarda T alelinin istatistiksel olarak anlamlı derecede yüksek bulunduğu (sırasıyla p<0.001, p<0.001) gözlendi. Çalışmamızda PON1 172T>A(Leu55Met) gen polimorfizmi ile postmenopozal osteoporoz arasında ilişki olduğu gösterildi.
Özet: Osteoporoz dünyada yaygın görülen önemli bir sağlık sorunudur. Osteoporoza bağlı kalça kırıkları önemli bir mortalite ve morbidite sebebidir. Bu çalışmanın amacı; kliniğimizde takipli osteoporoz hastalarındaki kalça kırığı sıklığını belirlemektir. Ocak 2009-Aralık 2010 tarihleri arasında polikliniğimize başvuran osteoporoz tanısı ile takipli hastaların dosyaları retrospektif olarak tarandı. Aralık 2018'e kadar geçen sürede kalça kırığı geçiren hastaların sosyodemografik özellikleri (yaşı, cinsiyeti), tanıları (postmenopozal osteoporoz, premenopozal osteoporoz, erkek osteoporozu, sekonder osteoporoz), kalça kırığı dışında başka kırık geçirip geçirmedikleri dosya bilgilerinden ve telefonla sorgulanarak elde edildi. Osteoporotik kalça kırığı ile ilişkili ölümler değerlendirildi. Osteoporoz tanısı olan 286 hasta dosyası incelendi. Hastaların 147'si çalışmaya dâhil edildi. Hastaların % 8,84 (n=13)'ünde kalça kırığı tespit edildi. Osteoporoz hastalarında kalça kırığına bağlı ölüm oranı % 2,04 (n=3) 'dü. On üç hastanın 10'unda kalça kırığı haricinde farklı bölgede (vertebra, kol, ön kol) kırık mevcuttu. Takipli osteoporoz hastalarındaki kalça kırığı sıklığı ve buna bağlı ölümlerin oranı belirlenmiştir. Sonuçlarımızın mevcut veri tabanına katkı sağlayacağı ve farkındalığı arttıracağı düşünülmüştür.
Çalışmamızda el-el bileği yaralanmalarından sonra gelişen kompleks bölgesel ağrı sendromunun (KBAS) değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Hastaların sosyodemografik özellikleri, iş kazası varlığı, el-el bileği yaralanma tipi (kemik, tendon, sinir, mikst), Modifiye El Yaralanması Ciddiyet Skoru (MEYCS) (minör, orta, ciddi, majör) ve KBAS varlığı hastane bilgi sisteminden elde edildi. Bulgular: Çalışmaya 311 hasta dosyası dahil edildi. Hastaların %23.8'inde KBAS geliştiği görüldü. Hastaların %49.2'si mikst tip yaralanmaya sahipti. Yaralanmanın lezyon tipi ile KBAS gelişimi arasında mikst tip yaralanma lehine ilişki vardı (p=0.015). Sinir hasarının eşlik ettiği yaralanmalar KBAS gelişimi ile ilişkili bulundu (p=0.001). MEYCS ile KBAS gelişimi arasında majör yaralanma lehine anlamlı fark tespit edildi (p=0.003). Sonuç: El-el bileği yaralanmalı hastalarda MEYCS yüksekliği ve/veya sinir yaralanması varlığının KBAS gelişimi yönünden risk faktörü olabileceği sonucuna varılmıştır.
