Общий анализ мочи является одним из наиболее часто выполняемых общеклинических лабораторных исследований на уровне первичной медицинской помощи. Изменение в анализе мочи в виде гематурии, по данным различных популяционных скринингов у детей, колеблется от 1-4 до 12-18 % случаев [1][2][3]. Уровень распространенности гематурии среди детей школьного возраста составляет 4,1-5,13 % на основании одного анализа мочи [3, 4] и 0,5-2 % -двух и бо-Íà äîïîìîãó ïåä³àòðó / To Help the Pediatrician лее анализов [2-4]. Не установлена связь гематурии с социально-экономическим статусом пациента, полом и возрастом [2, 3].Врачи первого контакта, столкнувшись с пациентом с гематурией, преимущественно делятся на две группы. Поражение желудочно-кишечного тракта является одним из основных элементов в ряду патологических изменений при ВИЧ-инфекции у детей. Такое поражение может быть вызвано прямым воздействием вируса, оппортунистическими инфекциями, длительным приемом лекарственных препаратов (антибиотики, противовирусные, противогрибковые). При прогрессировании ВИЧ-инфекции вероятность патологии пищеварительной системы возрастает. Среди оппортунистических заболеваний особого внимания заслуживает цитомегаловирусная (ЦМВ) инфекция, при которой в патологический процесс вовлекаются все органы и системы ребенка, в том числе желудочно-кишечный тракт. Эзофагит — второе по частоте после колита поражение органов пищеварения у таких больных. При ЦМВ-эзофагите у ВИЧ-инфицированных детей клинические симптомы неспецифичны. Достаточно информативным является эндоскопическое обследование. В доступной литературе имеются описания эзофагита у детей с ВИЧ-инфекцией, однако преимущественное внимание уделяется поражению пищевода, вызванному грибами. Представленный клинический случай демонстрирует развитие ЦМВ-эзофагита у подростка с ВИЧ-инфекцией. Нарушение приверженности к терапии у ребенка позволило заболеванию развиваться естественно, с прогрессирующим снижением показателей иммунного статуса, возникновением оппортунистической ЦМВ-инфекции с преимущественным поражением пищевода и развитием тяжелого ЦМВ-эзофагита. Развитие данного осложнения не давало возможности проводить коррекцию антиретровирусной терапии. Данный клинический случай иллюстрирует прогрессирующее течение ВИЧ-инфекции с развитием тяжелого ЦМВ-эзофагита у подростка на фоне отсутствия приверженности к антиретровирусной терапии.
ВИЧ-инфекция среди возможных этиологических факторов неврологической патологии пока рассматривается редко, что приводит к поздней диагностике заболевания и несвоевременному началу специфической терапии. Неврологические проявления заболевания могут возникать в связи с прямым воздействием ВИЧ на нервную систему, оппортунистическими инфекциями и злокачественными новообразованиями, нейротоксичными эффектами антиретровирусного лечения и другими системными осложнениями. ВИЧ-ассоциированные неврологические расстройства вариабельны и представлены нейрокогнитивными нарушениями, энцефалопатией, миелопатией, полинейропатией, асептическим менингитом (менингоэнцефалитом). ВИЧ-энцефалопатия у детей проявляется в форме эмоциональных, когнитивных, двигательных, речевых нарушений. У взрослых больных как возможный клинический вариант ВИЧ-энцефалопатии рассматривается синдром хореи. В доступной литературе мы не встретили сообщений о развитии хорееподобного синдрома у ВИЧ-инфицированных детей. Описанный клинический случай демонстрирует диагностику поздней стадии ВИЧ-инфекции у ребенка 13 лет с синдромом хореи. Представленное клиническое наблюдение демонстрирует сложности диагностики ВИЧ-энцефалопатии у ребенка, что было связано с редким проявлением данного заболевания в детском возрасте в виде генерализованных гиперкинезов, а также с сокрытием матерью анамнестических данных о наличии у нее ВИЧ-инфекции. Таким образом, у детей с хореей диагноз возможен только при исключении заболеваний, ее вызывающих, а именно острой ревматической лихорадки, системного заболевания соединительной ткани, демиелинизирующих заболеваний, объемно-органической патологии центральной нервной системы, болезни Вильсона — Коновалова, а также ВИЧ-инфекции.
