A necrólise epidérmica tóxicaé desencadeada na maior parte das vezes por medicações, sendo caracterizada por necrose extensiva da epiderme acometendo mais de 30% da superfície corpórea, levando o paciente a apresentar aspecto de grande queimado. As medicações mais comumente associadas são o alopurinol, lamotrigina, sulfassalazina, anti-inflamatórios não esteroidais e fármacos oncológicos. OBJETIVO: Neste estudo, discutimos os principais aspectos da necrólise epidérmica tóxica através de uma revisão de literatura ilustrada por um caso clínico. METODOLOGIA: Foi realizada uma revisão bibliográfica tomando-se por referência informações obtidas nas bases de dados online PubMed e SciELO. Artigos em inglês, português, francês e espanhol foram incluídos e ilustrados com um caso clínico. RELATO DO CASO: O paciente analisado foi admitido no serviço com lesões descamativo bolhosas em 40% da superfície corporal, hematúria macroscópica e taquicardia 48 horas após uso de fenitoína. O paciente foi tratado na unidade de queimados, com suspensão da medicação e curativos seriados e teve um curso favorável da doença sem sequelas. DISCUSSÃO: Apresentamos dados para orientar o tratamento de pacientes com necrólise epidérmica tóxica para cirurgiões plásticos, pediatras, intensivistas, dermatologistas e médicos de emergência. Um alto nível de suspeita é necessário para um diagnóstico e estratificação de risco adequados, e medidas e tratamento precoces de apoio devem ser realizados por uma equipe multidisciplinar treinada para minimizar os danos e a mortalidade.Palavras-chave: "necrólise epidérmica tóxica", "sobreposição de Stevens-Johnson", "necrólise epidérmica tóxica" e "síndrome Stevens-Johnson".
Intussusception is the invagination of a bowel segment into another immediately adjacent, causing obstruction, and the etiology can be benign, malignant, or idiopathic. The diagnosis may be obtained by ultrasound scans, or computed tomography imaging that can detect characteristics findings. Small bowel obstruction due to intussusception of inverted Meckel's diverticulum is a rare condition that usually constitutes a challenging diagnosis in adulthood. The surgical management in adults is often performed by en bloc resection without reduction attempts. We report a case of CASE REPORTS a middle-aged man with acute intestinal obstruction due to an ileo-ileal intussusception of inverted Meckel's diverticulum with a lipoma that was managed by laparoscopy. Worthy of note is the very scarce number of case reports on this exceeding uncommon association.
