A case of double-orifice mitral valve (duplication of the mitral valve) in an 18-month-old baby is presented. The valve showed significant regurgitation and was replaced with a No.20 Lillehei-Kaster mitral prosthesis. Patient is kept on dipyridamole (Persantin) 10mg/Kg and aspirin 50mg/Kg daily to avoid postoperative thromboembolic complications. It is emphasized that valve replacement should be the treatment of choice in severe mitral regurgitation associated with double-orifice mitral valve.
Konjenitai lober amfizem, yenidoğanda, ilgili lobun hiperinflasyonu bunun çevredeki normal akciğer dokusuna basısı ve mediastinal shift ile karakterli bir respiratuar distres nedenidir (1,3). Nadir gö-rülen bir hastalıktır. Spontan Pnömotoraks ile karıştırılması ve ciddi olgularda uygulanan acil cerrahi müdahale ile klinik tablonun dramatik olarak düzelmesi nedeniyle önem taşır. OLGU 20 günlük kız yenidoğan; morarma, solunum sıkıntısı, huzursuzluk şikayetleriyle bir hastaneye başvurmuş. Çekilen PA akciğer frafisinde pnömotoraks olarak değerlendirilip, sol üst göğüs takılmış, hastanın solunum sıkıntısının artması nedeniyle merkezimize sevk edilmiştir.Hasta geldiğinde; siyanozu, bilateral interkostal ve sternal retraksiyonları, sağ akciğerde yaygın raileri, sol hemitoraksda havalanma azlığı, hepatomegalisi (midklavikuler hastta 4 cm) ile respiratuar distress ve dekompanse kalp yetmezliğindeydi. Hasta acilen digitalize edildi ve ikili antibiotik tedavisi başlandı. PO2 35 mmHg, PCO2 50 mmHg, Ph 7,32 idi. PA akciğer grafisinde ileri derecede sağ mediastinal shift mevcuttu (Şekil 1).
