The objective: to evaluate the psycho-emotional state, lifestyle changes, clinical symptoms and treatment measures in women with genital endometriosis after COVID-19 disease. Materials and methods. The study involved 120 women with genital endometriosis. The main group included 62 women with genital endometriosis after COVID-19 disease, the control group consisted of 58 patients with genital endometriosis, no symptoms of COVID-19 and negative PCR test at the time of examination. Impact of Covid-19 on lifestyle changes, symptoms and treatment of endometriosis was measured with a questionnaire. Pain assessment was performed by a visually analog scale (VAS). The psychoemotional state of women was analyzed by a Peritraumatic Distress Inventory. Statistical analyses based on Microsoft Excel statistical analysis package and “Statistica 7.0”(StatSoft Inc., USA). Results. The scientific work demonstrated high level of peritraumatic stress, lifestyle changes (decreased physical activity, increased sleepiness), worsening of clinical manifestations (increased fatigue, severe menstrual bleeding, dysmenorrhea, increased pelvic pain) and changes in treatment measures (increased analgesics and non-steroidal anti-inflammatory drugs using, discontinuation of hormonal therapy) in women with genital endometriosis after COVID-19 disease compared with group of women with genital endometriosis and without symptoms of coronavirus infection. Conclusions. The obtained results are necessary to optimize the management of patients with genital endometriosis and COVID-19.
Актуальність. Вивчення ендометріозу визначаються значним поширенням даної патології, прогресуючим перебігом, розвитком хронічного тазового болю, диспареунії, непліддя, що порушує якість життя жінки. Мета дослідження. Дослідили асоціацію рівня лептину в сироватці крові жінок з ендометріозом зі стадією тяжкості патологічного процесу. Матеріали та методи. У дослідження залучено 69 пацієнток з діагнозом ендометріозу (основна група) і 47 жінок без гінекологічної патології (контрольна група), які проходили обстеження і оперативне лікування у міському клінічному перинатальному центрі, м. Івано-Франківськ, Україна. Ступінь тяжкості ендометріозу встановлювали згідно наказу МОЗ України № 319 від 6.04.2016 року. Кількісне визначення лептину в сироватці крові проводили методом імуноферментного аналізу (ELISA Kit № CAN-L-4260, Canada). Результати статистично аналізували за допомогою Statistica 12.0 (StatSoft Inc., США) та пакету статистичного аналізу Microsoft Excel. Результати. Пацієнтки обох груп вірогідно не різнилися за індексом маси тіла (р˃0,05). Середній показник лептину у жінок основної групи у 3,2 рази перевищував рівень гормону у контрольній групі (28,94±7,20 нг/мл проти 12,11±4,16 нг/мл) (р<0,05). 55,1 % жінок з ендометріозом мали показник лептину від 20,00 до 29,99 нг/мл, що у 6,5 разів перевищувало частоту жінок контролю (р<0,05). Встановлено збільшення показника лептину при прогресуванні стадії тяжкості ендометріозу: при І стадіїї середня різниця становила 8,82±2,71 нг/мл (р<0,05), при ІІ – 14,86±2,63 нг/мл (р<0,001), при ІІІ – 18,12±3,69 нг/мл (р<0,001), при ІV – 26,99±3,52 нг/мл (р<0,001) порівняно з показником лептину групи контролю (12,11±4,16 нг/мл). Отже, зростання рівня лептину у 1,7 разів збільшує шанси ендометріозу І стадії тяжкості (OR=4,43; 95% CІ: 2,01-9,78; р<0,001), у 2,2 рази – ІІ стадії (OR=4,09; 95% CІ: 1,29-12,94; р<0,05), збільшення гормону у 2,4 рази підвищує шанси ІІІ стадії (OR=3,51; 95% CІ: 1,10-11,23; р<0,05), а у 3,2 рази – ІV стадії тяжкості (OR=3,79; 95% CІ: 1,19-12,07; р<0,05). Висновки. Лептин можна вважати прогностичним маркером встановлення ендометріозу та діагностики ступеня тяжкості.
