Bedah jantung koroner merupakan jenis operasi jantung yang paling sering dilakukan pada pasien dewasa. Pada penelitian yang dilakukan tahun 2011-2014 angka mortalitas bedah jantung koroner pada pasien dengan multipel lesi pembuluh darah sebesar 27,8%.Pasien yang dipertimbangkan untuk dilakukan bedah jantung koroner memerlukan penilaian risiko operatif, pemeriksaan menyeluruh dan manajemen perioperatif yang baik oleh dokter anestesi. Pemilihan teknik dan agen anestesia pada bedah jantung koroner telah berkembang pesat dengan tersedianya obat intravena yang memiliki durasi singkat dan mudah dititrasi seperti propofol dan remifentanyl yang memberikan efek sinergis dibandingkan penggunaan opioid dosis tinggi. Oleh karena itu, teknik total intravenous anesthesia (TIVA) mulai banyak dipilih seiring dengan perkembangan obat dan tersedianya alat seperti smart-pump infusion yang memungkinkan obat intravena untuk mencapai efek klinis yang diharapkan dan memberikan angka harapan hidup dan luaran yang lebih baik hingga beberapa tahun pascabedah.
Penyakit gagal jantung merupakan suatu kondisi kelainan pada pasien dimana kemampuan pengisian dan fungsi pompa jantung mengalami gangguan. Gagal jantung sendiri dapat terjadi secara akut atau kronis serta berpotensi mengancam jiwa. Ketika kondisi tersebut memburuk maka dapat menyebabkan gangguan fungsi pernapasan dan perburukan kondisi hingga berakibat kematian. Kondisi gagal jantung dapat terjadi selama tindakan perawatan perioperatif pembedahan yang membutuhkan terapi farmakologi dan terapi non farmakologi. Salah satu modalitas terapi non farmakologi yang dapat membantu fungsi pompa jantung dalam kondisi gagal jantung adalah ventilasi mekanik. Tinjauan pustaka ini akan mengulas tentang terapi perioperatif ventilasi mekanik pada pasien dengan gagal jantung.
Latar Belakang: Regional blok sebagai ajuvan dalam operasi jantung mulai banyak digunakan sebagai terapi tambahan untuk mengurangi nyeri selama periode intraoperatif dan pascaoperasi, diantaranya blok Pekto Interkosta Fasial (PIF). Pembedahan jantung identik dengan penggunaan opioid dosis tinggi. Pada laporan kasus ini, kami melaporkan penggunaan blok PIF pada pasien dewasa yang menjalani operasi CABG.Kasus: Laki-laki, usia 58tahun, diagnosa penyakit jantung koroner, menjalani operasi CABG. Pembahasan: Kami lakukan ajuvan blok PIF pada anestesi umum dengan anestesi lokal bupivakain 0,25%+ epinephrine 1:200.000 dengan volume 20ml sisi kanan dan 20ml sisi kiri. Hemodinamik terpantau stabil tanpa penambahan opioid selama periode operasi.Kesimpulan: Penggunaan teknik PIF Blok efektif menurunkan penggunaan opioid dan menjamin pengelolaan nyeri yang adekuat pada operasi CABG. Blok ini dapat dipergunakan sebagai salah satu alternatif teknik analgesia untuk pasien sternotomi maupun kasus lain yang melibatkan rongga dada bagian depan.
Latar Belakang: Opioid dosis tinggi selama pembedahan jantung berhubungan dengan waktu intubasi yang memanjang dan mengakibatkan peningkatan lama perawatan, mobiditas dan tingkat mortalitas. Anestesi regional dan neuraxial telah digunakan untuk tatalaksana nyeri untuk mengurangi dosis opioid. Teknik terbaru seperti blok parasternal, pectoral dan erector spinae telah dikembangkan untuk meminimalisir komplikasi dari anestesi regional.Kasus: Kami melakukan blok parasternal pada gadis berusia 10 tahun yang menjalani prosedur koreksi tetrallogy of fallot. Hasil laboratorium dalam batas normal. Status fisik ASA adalah 4 dengan penyakit jantung bawaan sianotik. Opioid hanya digunakan saat induksi. Injeksi bilateral dari bupivacaine 0,25% ditambah epinephrine 1:200000 dengan total volume 40 ml diberikan masing-masing 20 ml pra insisi.Pembahasan: Hemodinamik pasien stabil selama pembedahan tanpa tanda-tanda respons terhadap nyeri. Tidak ada penambahan dosis opioid selama pembedahan. Pasien di ekstubasi pada saat pembedahan selesai.Kesimpulan: Blok parasternal efektif sebagai adjuvan pembiusan umum pada pembedahan jantung. Teknik ini memfasilitasi penurunan dosis opioid yang membantu pasien untuk bisa di ekstubasi lebih dini.
Penyakit jantung koroner merupakan penyebab kematian terbanyak di dunia. Menurut kriteria American Heart Association (AHA) tahun 2017, lebih dari 360.000 kematian di Amerika disebabkan oleh penyakit jantung koroner setiap tahunnya. Bedah pintas arteri koroner merupakan prosedur standar tatalaksana revaskularisasi pasien dengan penyakit tiga pembuluh darah arteri koroner atau pembuluh darah arteri koroner utama kiri. Sebanyak 400.000 prosedur bedah pintas arteri koroner dilakukan di Amerika setiap tahunnya. Bedah pintas arteri koroner ini mulai dikenal sejak tahun 1950. Pada tahun 1970 hampir semua prosedur bedah pintas arteri koroner menggunakan cairan kardioplegia untuk menghentikan jantung dan mesin pintas jantung paru. Pada pertengahan tahun 1990, diperkenalkan teknik bedah pintas arteri koroner off-pump. Teknik ini memungkinkan dokter bedah melakukan anastomosis arteri koroner pada jantung yang berdetak tanpa menggunakan mesin pintas jantung paru. Tujuannya adalah untuk menghindari respons inflamasi akibat penggunaan mesin pintas jantung paru serta mengurangi risiko yang timbul akibat manipulasi aorta. Pada tahun 2002, hampir 25% bedah pintas arteri koroner di Amerika dilakukan secara off-pump. Oleh karena itu, dokter anestesi memiliki peranan penting dalam menjaga stabilitas sirkulasi selama manipulasi berlangsung, membantu pemulihan dan mobilisasi dini, menurunkan morbiditas dan mortalitas, serta mampu mengurangi biaya prosedur.
