Депрессия и когнитивные нарушения оносятся к са-мым частым последствиям инсульта, которые наблюдаются у 50-70% пациентов [1][2][3][4][5][6][7][8]. Как показали современные ис-следования, связь депрессии и деменции намного сложнее, чем представлялось ранее: общие биохимические основы, общие звенья патогенеза и отягощающее влияние на исхо-ды инсульта послужили основанием для изучения депрес-сии в сочетании с деменцией и объединения их в так назы-ваемый синдром 2Д.Наличие постинсультной депрессии и когнитивных нарушений ассоциируется с плохим восстановлением нев-рологических функций, ухудшением прогноза и качества жизни, увеличением сроков госпитализации и повышением риска повторного инсульта и смерти [9][10][11][12][13][14].Долгое время депрессию после инсульта рассматрива-ли как нормальную реакцию на развитие неврологического заболевания. Однако установлено, что риск возникновения депрессии у больных с инсультом в 4-5 раз выше, чем у больных с ортопедической патологией при идентичной, а иногда и большей степени физической инвалидизации [15]. Инвалидизация может увеличивать частоту постин-сультной депрессии в ранние сроки, но не вызывает ее в от-даленном периоде инсульта, следовательно, влияние инва-лидизации на развитие постинсультной депрессии ограни-чено [16]. Поэтому возникло предположение, что ключе-вую роль в возникновении постинсультной депрессии иг-рают непосредственно фокальная ишемия в стратегиче-ской зоне или диффузная церебральная ишемия с наруше-нием перфузии вещества мозга и связанные с этим нейро-химические изменения, приводящие к нарушению моно-аминергической трансмиссии. Pre-dementia cognitive impairments are a significant part of a cognitive impairment range, considered the most perspective in terms of conversion into dementia prevention. At the same time pre-dementia cognitive impairment treatment presents certain difficulties in connection with the absence of precise medicinal strategies with the high evidence level. According to current clinical recommendations, the priority in cognitive pre-dementia treatment lies in the correction of modifiable risk factors of cognitive impairment progression and non-drug therapy methods. Nevertheless, there is a great need in receiving the proofs of pathogenetically justified use of drugs with a potentially effective mechanism of action. Universal and early pathogenetical mechanism of many diseases of central nervous system, leading to the development of cognitive impairments, including Alzheimer’s disease, is Ca2+ dyshomeostasis. In this connection, the use of drugs, regulating Ca2+ metabolism in neurons in treatment of patients with pre-dementia cognitive impairments, especially high brain penetrance drugs, is potentially well-grounded. The article gives the data review of modern researches, studying the efficiency and safety of nimodipin a selective Ca+ channel blocker of the 2 class. The article provides examples of the use of nimodipin in treatment and prevention of pre-dementia syndroms of various genesis: Alzheimer’s disease, chronic brain hypoperfusion damage, post-radiation and postoperative cognitive dysfunction, etc. The article presents experimental data about nimodipin use in the prevention of postoperative delirium in elderly people and in the treatment of non-cognitive neurological disorders. Justification of the perspectives of the application area extension and directions for the further research of nimodipin are as well given in the article.

Post-Cognitive Impairment, including in the young population, is an urgent medical and social problem. It is known that COVID-19, even in the absence of direct brain damage associated with covid-associated stroke or encephalitis, regardless of the severity of the infection during the acute period and the age of patients, can cause long-term and maladaptive cognitive impairment. The pathogenesis of these disorders is complex, and it is not completely clear today and continues to be actively studied. There is also no standardized diagnostic and therapeutic approach in the management of patients with post-COVID cognitive impairment. The presented clinical observation demonstrates the experience of treating cognitive impairment after COVID-19 in a young patient using the available knowledge of pathogenesis. A review of modern publications on the problem of post-COVID cognitive impairment was carried out. Based on the available literature data, the role of various pathogenetic factors in the development of post-covid cognitive impairment in the presented patient is discussed. Based on a specific clinical example, the rationale for the choice of treatment strategies and the need to further increase knowledge about the postCOVID syndrome, conduct future research in the field of pathogenesis, diagnosis and treatment of post-COVID cognitive impairment, which will create an evidence base for the development of clinical recommendations, is given.
В многочисленных работах выявлены различные фак-торы риска и механизмы, приводящие к структурным и функциональным изменениям в мозге при цереброваску-лярных заболеваниях (ЦВЗ) [1][2][3][4][5][6][7][8][9]. Однако ведущее патоге-нетическое звено в развитии клинической симптоматики этих синдромов остается неясным. В связи с частым сочета-нием деменции и депрессии у пациентов с ЦВЗ предложен термин «синдром 2Д» для упрощения их совместного обо-значения. Как известно, самой частой причиной деменции считается болезнь Альцгеймера [1]. «Чистые» формы болез-ни Альцгеймера, связанные с генетически обусловленным повышением уровня амилоида, встречаются редко, боль-шинство случаев заболевания носят спорадический характер и вызваны типичными изменениями механизмов клиренса в головном мозге и накоплением бета-амилоида [1,5,7]. Одна из предполагаемых причин этих нарушений -сердечно-со-судистые факторы риска, главным образом артериальная ги-пертензия (АГ) [2][3][4][5][6]. Понимание общности факторов риска и отдельных звеньев патогенеза нейродегенеративного про-цесса и ЦВЗ привело к появлению теории смешанной при-роды деменций, которая в настоящее время широко распро-странена и поддерживается большинством исследователей 40Неврология, нейропсихиатрия, психосоматика. 2017;9(2):40-45.
