Кафедра хирургических болезней №2 ГБОУ ВПО «Курский государственный медицинский университет» Минздрава России, Курск, Россия Цель работы-улучшение результатов лечения холедохолитиаза и стеноза большого дуоденального сосочка двенадцатиперстной кишки (БСДК) путем применения антеградной моно-и биполярной папиллосфинктеротомии (АПТ). Материал и методы. Изучены результаты диагностики и лечения 256 больных с осложненной желчнокаменной болезнью. У 132 больных разрешение холедохолитиаза проводилось с использованием эндоскопической папиллосфинктеротомии до или после лапароскопической холецистэктомии (ЛХЭ). У 124 пациентов во время ЛХЭ выполнялась антеградная (моно-или биполярная) папиллосфинктеротомия. Результаты. Антеградная моно-и биполярная папиллотомия выполнима в 99,25% случаев, позволяет малотравматично разрешить выявленную доброкачественную патологию внепеченочных желчных протоков и БСДК одноэтапно во время лапароскопической холецистэктомии и осуществима даже при стриктурах, интрадивертикулярном расположении БСДК, остром панкреатите и минимизирует возможность развития таких осложнений, как кровотечения, перфорации, панкреатита (р<0,05). Вывод. Антеградная моно-и биполярная папиллотомия выполнима практически в 100% случаев, позволяет малотравматично и безопасно разрешить выявленную доброкачественную патологию внепеченочных желчных протоков и большого сосочка двенадцатиперстной кишки сразу во время ЛХЭ, избежав при этом многоэтапности и неоправданного перехода на открытую операцию (конверсию, которая по данным литературы, колеблется от 5 до 10%). Она осуществима даже в тех случаях, когда невыполнима или противопоказана эндоскопическая папиллотомия: стриктура, интрадивертикулярное расположение большого сосочка двенадцатиперстной кишки, острый панкреатит.
