AIM: evaluation of the effectiveness of the enhanced recovery protocol for stoma reversal procedures.PATIENTS AND METHODS: a single-center retrospective analysis of stoma reversal surgery in 130 ostomy patients in 2012-18 was performed. From 2012 to 2015, 56 (43.1%) patients were treated before the implementation of the Enhanced Recovery After Surgery (ERAS) protocol in clinical practice, 74(56.9%) patients were treated in accordance with the principles of fast-track.RESULTS: the introduction into clinical practice of the ERAS protocol reduced postoperative complications from 8.5% to 5.4% (p=0.002) and the hospital stay from 16,3±9,4 to 11,4±4,2 days (p=0.003).CONCLUSION: the fast-track strategy is an effective way to improve the results of stoma reversal procedures.
РезюмеВ научной литературе имеются немногочисленные публикации, посвященные оценке замыкательного аппарата прямой кишки на этапах хирургической реабилитации стомированных пациентов. Цель исследования -изучение факторов, влияющих на изменение функциональной активности замыкательного аппарата прямой кишки у пациентов с кишечными стомами. Материал и методы. Проведен одноцентровой ретроспективный и проспективный анализ результатов изменения функции замыкательного аппарата прямой кишки у 83 стомированных пациентов до восстановительной операции и после нее. За период с 2016 по 2018 г. обследованы 42 (50,6 %) мужчины и 41 (49,4 %) женщина. Средний возраст в анализируемой группе составил 51,8 ± 12,6 года. Результаты и их обсуждение. У всех стомированных пациентов выявлена недостаточность анального сфинктера. Определена зависимость между степенью нарушения функциональной активности замыкательного аппарата прямой кишки, длительностью ношения стомы и возрастом больного. Заключение. Анализ полученных результатов позволяет оценить динамику восстановления функции замыкательного аппарата прямой кишки и определить оптимальные сроки выполнения восстановительного вмешательства с позиции профилактики нарушения функции замыкательного аппарата прямой кишки.Ключевые слова: стома, недостаточность анального сфинктера, восстановление непрерывности кишечника.Конфликт интересов.
