Цель исследования -оценка эффективности вортиоксетина в коррекции депрессивной симптоматики в раннем восстановительном периоде ишемического инсульта, а также его влияния на когнитивные функции пациентов. Пациенты и методы. В исследование включено 60 пациентов с ишемическим инсультом в каротидном бассейне с симптомами легкой и умеренной депрессии по шкале депрессии Монтгомери-Асберга (Montgomery-Asberg Depression Rating Scale, MADRS). К 1-й (основной) группе отнесены 30 пациентов, получавших вортиоксетин в дозе 10 мг/сут в течение 8 нед; ко 2-й группе (сравнения) -30 пациентов, отказавшихся от приема антидепрессанта. Выраженность очагового неврологического дефицита оценивали с помощью шкалы инсульта Национального института здоровья США (National Institute of Health Stroke Scale, NIHSS), степень функционального дефицита -с помощью модифицированной шкалы Рэнкина (modified Rankin Scale, mRS), шкалы функциональной независимости (Functional Indepedence Measure, FIM). Диагноз постинсультной депрессии устанавливали согласно критериям DSM-5 (Diagnostic and Statistical Manual of mental disorders, fifth edition), выраженность депрессии анализировали с помощью госпитальной шкалы тревоги и депрессии (Hospital Anxiety and Depression Scale, HADS) и MADRS; когнитивные функции -с помощью краткой шкалы оценки психического статуса и монреальской когнитивной шкалы (Montreal Cognitive Assessment, MoCA). Для оценки нарушений высших психических функций использовали комплекс методик нейропсихологического исследования, разработанный А.Р. Лурией и его последователями, с применением классических стимульных материалов. Результаты и обсуждение. Через 8 нед наблюдения в 1-й группе отсутствовали пациенты с симптомами умеренной депрессии, симптомы легкой депрессии имелись лишь у 14 (46,7%) больных, у остальных 16 пациентов эмоциональных нарушений не отмечалось (53,3%). При этом во 2-й группе у 14 (46,7%) пациентов наблюдались признаки умеренного депрессивного расстройства, у 15 (50%) -симптомы легкой депрессии; у 1 (0,3%) пациента выявлены симптомы большого депрессивного расстройства. На фоне лечения вортиоксетином зарегистрировано значительное уменьшение выраженности депрессивной симптоматики по общему баллу MADRS (U=87,0, p<0,0001), лучшее функциональное восстановление после инсульта по шкалам FIM (U=296,0; p=0,023), NIHSS (p<0,05). При анализе результатов подтестов шкалы MoCA оказалось, что пациенты 1-й группы статистически достоверно лучше справлялись с заданиями на внимание (U=237,0; p<0,001). Заключение. The problem of ischemic stroke in the Russian Federation occupies a leading place in the structure of causes of mortality. Among all treatment methods, the systemic thrombolytic therapy with a recombinant tissue plasminogen activator is regarded as the most effective one. This review focuses on clinical studies of the safety and effectiveness of this type of treatment, as well as on prognostically unfavorable factors inducing the hemorrhagic transformation of ischemic stroke.
