Introdução: O Point-of-Care-Ultrasound (POCUS) trata-se da ultrassonografia transtorácica realizada à beira-leito do paciente. A aparelhagem, além de ser portátil, é de fácil aplicação e interpretação de seus achados. A sua utilização tem sido relacionada à melhora propedêutica e terapêutica de casos conduzidos em distintos setores intra-hospitalares. Métodos: Realizou-se uma revisão integrativa da literatura com busca no PubMed. Utilizou-se 14 artigos, selecionados a partir dos descritores “lung ultrasound”, “POCUS”, “respiratory failure” e “heart failure”, encontrados no MesH. Foram selecionados: metanálises, estudos controlados randomizados e revisões sistemáticas, em inglês, publicados entre 2012 e 2022. Resultados: As vantagens mais citadas foram: facilidade do manuseio, fornecimento de dados em tempo real e disponibilidade à beira-leito. As linhas B são o achado mais validado, apresentando em um trabalho 85% de sensibilidade e 91% de especificidade para insuficiência cardíaca (IC) e 100% de sensibilidade e de valor preditivo negativo para a exclusão do pneumotórax. A quantificação dessas consegue predizer a descompensação da IC (>15) e risco de hospitalização e morte (>30) em Centros de Terapia Intensiva. No intra operatório da cardiologia pediátrica, um estudo randomizado com 122 crianças demonstrou haver menos eventos adversos relacionados à dessaturação no grupo intervenção (55% versus 73%). Conclusões: A utilização do POCUS em ambiente hospitalar tem se mostrado de grande valia em diferentes setores. Alguns de seus achados, como as slinhas B, estão cada vez mais elucidados e demonstram alto potencial diagnóstico e propedêutico, permitindo intervenções terapêuticas mais precocemente. Mais estudos são necessários para melhorar a validação de seus achados.
INTRODUÇÃO: A tuberculose (TB) é uma doença infecciosa cujo tratamento é longo e possui, além da duração, fatores que reduzem sua adesão. Tais fatores evidenciam-se fortemente na população em situação de rua (PSR), mais vulnerável à infecção por TB. METODOLOGIA: Revisão narrativa de literatura, em inglês e português nas bases da PubMED/MEDLINE, SciELO, BVS e OPAS/PAHO utilizando os descritores "Tuberculosis", "Pulmonary", "Homeless" e "Medication Adherence". Análise de estudos pré-existentes, buscando a síntese de informações da temática. RESULTADOS: Seleção de 48 artigos, dentre os quais 27 preenchiam os critérios de inclusão e exclusão, cujas informações compuseram o artigo. DISCUSSÃO: O tempo de tratamento e seus efeitos colaterais afetam a adesão à terapêutica da TB na PSR. O uso de drogas e álcool, infecção por HIV, baixa escolaridade, falta de apoio familiar, fatores socioeconômicos e outras condições relacionadas à PSR, influenciam nessa problemática, tornando tal grupo mais vulnerável e com maiores taxas de abandono do tratamento. Almeja-se melhorar tal questão mediante políticas públicas que ainda demonstram-se insuficientes. CONCLUSÃO: Apesar da existência de políticas públicas que visem a resolução da problemática, a PSR possui maior vulnerabilidade à infecção por TB, menor adesão ao tratamento e pior prognóstico comparado a população em geral.
INTRODUÇÃO: O carcinoma espinocelular da cavidade oral (CECO) consiste em um dos principais tipos de câncer bucal e tem como principal característica um forte padrão de disseminação para os linfonodos cervicais. O presente estudo visa analisar a segurança e aplicabilidade da biópsia do linfonodo sentinela (BLS) nos pacientes portadores de CECO em estágios iniciais e suas implicações clínicas no âmbito diagnóstico, prognóstico e terapêutico. METODOLOGIA: Este estudo compreende em uma revisão narrativa de literatura, cuja busca foi feita nas bases de dados PubMed/MEDLINE, utilizando os descritores “Sentinel Lymph Node Biopsy”, “Squamous Cell Carcinoma”, “Mouth” associados ao operador booleano AND. Foram selecionadas publicações disponíveis na íntegra entre o período de 2016 a 2021, totalizando 77 resultados. Após a leitura aprofundada e análise crítica, foram utilizados 32 artigos para a realização do presente estudo. RESULTADOS E DISCUSSÃO: A BLS tem demonstrado ser um método alternativo oncologicamente seguro e menos invasivo capaz de estadiar confiavelmente os pacientes em estágios iniciais e de identificar metástases ocultas com boa sensibilidade. Estudos revelam a superioridade da BLS com relação a mastigação, aparência e qualidade de vida em comparação ao esvaziamento cervical seletivo (ECS). A BLS também é capaz de identificar padrões atípicos de drenagem linfática e metástases saltadas, que poderiam passar despercebidas com o ECS. Por outro lado, as taxas de falsos negativos da BLS para cânceres de cabeça e pescoço podem atingir até 14%. Essa técnica possui também algumas limitações, como a dificuldade na identificação de metástases ocultas perto do tumor e local de injeção devido a alta radioatividade na região, podendo ser malsucedida em 3,4 a 7% dos casos. CONCLUSÃO: Tendo em vista as altas taxas de ECS desnecessários e a redução da sobrevida mediante a espera cautelosa, a classe médica em conjunto com a equipe multidisciplinar, deve ter ciência do crescente corpo de evidências que sugere a respeito da implementação da BLS como alternativa pertinente no manejo de casos de CECO em estágio inicial.
Introdução: A anafilaxia corresponde a uma reação imunológica sistêmica aguda que apresenta uma taxa frequente na emergência. Em muitos casos, sua evolução é imprevisível e grave, demandando um melhor conhecimento dessa enfermidade pelos profissionais. Metodologia: Realizou-se uma revisão de literatura. A coleta de dados foi efetuada no SCIELO e no PUBMED com o uso dos descritores em saúde: “Anafilaxia; Diagnóstico; Emergência; Epidemiologia; Terapêutica”. Resultados: Selecionou-se 19 artigos publicados em inglês e em português entre os anos de 2012 e 2020. A anafilaxia geralmente decorre de um mecanismo imunológico contra diferentes antígenos expostos a uma pessoa previamente sensibilizada. Sua patogênese envolve a ativação direta dos mastócitos, que promovem a liberação de mediadores inflamatórios. Tais substâncias desencadeiam o início abrupto dos sintomas. Durante as crises, os pacientes apresentam sintomas predominantes nos sistemas tegumentar e respiratório. O diagnóstico é sintomatológico. O manejo terapêutico deve ser rápido para prevenir complicações severas e o fármaco de escolha é a adrenalina. A compreensão acerca dessa condição ainda está defasada por alguns profissionais de saúde, o que embasa a condução clínica ineficaz de alguns pacientes bem como a maior ocorrência de óbito. Conclusão: A anafilaxia é uma condição clínica potencialmente fatal que deve ser manejada corretamente na emergência, para reduzir o número de óbitos. Contudo, o seu insuficiente conhecimento por alguns médicos e acesso à adrenalina no país dificultam que isso ocorra.
Background: It is estimated that by 2050, over 130 million people will have dementia. These syndromes are neuropathologies that can be diagnosed with higher accuracy with a support of complementary exams. Objectives: Review literature about de importance of complementary exams for a better management of dementia syndromes. Methods: A search was carried out in the databases PubMed/MEDLINE, LILACS and Google Scholar using the DeCS descriptors: “dementia”, “mental status and dementia tests” and “diagnosis”. Nine articles, from 2005 to 2020, in English and Portuguese, were submitted to critical analysis. Results: A clinical evaluation, biomarkers and neuroimage techniques can improve diagnosis management of dementia syndromes. Changes in the early stages include memory loss. Therefore, Mini Mental State Exam can be used. The biomarkers include ß-amiloid and tau protein in the cerebrospinal fluid. Other exams can detect the lack of vitamin B12 and folate, hypothyroidism and infectious diseases. The computed tomography (CT) is fundamental to exclude secondary causes. In magnetic resonance the brain is seen atrophied. Conclusions: This review shows studies that indicate the relevance of complementary exams for the diagnosis of dementia. It could be seen that the association of molecular analysis and neuroimage can be benefic for a better diagnosis.
