В развитии и прогрессировании хронического панкреатита (ХП) определенную роль играет возрастание интенсивности процессов перекисного окисления липидов (ПОЛ) с угнетением ферментативного звена антиоксидантной защиты (АОЗ). Цель-изучить показатели перекисного окисления липидов и антиоксидантной защиты крови у пациентов с различными клинико-морфологическими формами ХП. Материал и методы. Обследованы четыре группы больных (n = 120). Первую группу составили 12 (10,0%) пациентов с обструктивной формой ХП, вторую-33 (27,5%) пациента с кальцифицирующей формой, третью-42 (35,0%) пациента с фиброзно-паренхиматозной формой, четвертую-33 (27,5%) пациента с ХП, осложненным псевдокистой. Состояние системы ПОЛ оценивали по концентрации малонового диальдегида (МДА) в плазме крови и эритроцитах и уровню промежуточных продуктов ПОЛ. О состоянии системы АОЗ судили по активности супероксиддисмутазы (СОД) и каталазы в гемолизате эритроцитов. В плазме крови измеряли содержание церулоплазмина (ЦП). В контрольную группу вошли 20 практически здоровых лиц. Результаты. Практически у всех больных ХП (90,0%) имела место активация процессов ПОЛ (по уровню МДА и промежуточных продуктов), чаще у пациентов первой и второй групп (р < 0,05). У 97,4% больных ХП выявлен дисбаланс между накоплением первичных продуктов ПОЛ и ускоренным распадом продуктов их трансформации в диальдегидные продукты (МДА). У 31,8% больных ХП наблюдалось значительное снижение активности фермента СОД, у 39,5%-снижение активности каталазы. Активность внеклеточного фермента ЦП у всех больных была достоверно повышена. Заключение. У пациентов с ХП зафиксирована выраженная интенсификация свободнорадикального ПОЛ на фоне напряжения ферментативной системы АОЗ. Наиболее существенный дисбаланс системы «ПОЛ-АОЗ» установлен пациентов с более длительным течением заболевания.
