У статті висвітлено проблему анестезіологічного забезпечення фізіологічного розродження у вагітних із прееклампсією. Прееклампсія є специфічним мультиорганним розладом, що виникає з другого/третього триместру вагітності та супроводжується такими критичними станами, як еклампсія, HELLP-синдром, гостре ураження нирок, набряк мозку. Основними ускладненнями для плода є обмеження росту, недоношеність та мертвонароджуваність. У післяпологовому періоді у таких жінок підвищується ризик виникнення артеріальної гіпертензії, цукрового діабету та інвалідизації. До цього часу залишається актуальною проблемою анестезіологічне забезпечення процесу розродження пацієнток із прееклампсією. Нами висвітлено досвід проведення анестезіологічного забезпечення пологів у вагітної з прееклампсією середнього ступеня тяжкості за допомогою епідуральної аналгезії низькоконцентрованими місцевими анестетиками (10,0 мл 0,125% розчину ропівакаїну). Під час проведення анестезіологічного забезпечення пологів у вагітної з прееклампсією контролювалися показники гемодинаміки та рівня болю до пологів, через 24 години та на третю добу після розродження. За результатами досліджень можна стверджувати, що епідуральна аналгезія допустимо знижує гемодинамічні показники, водночас негативно не впливає на процес фізіологічних пологів та може бути методом вибору анестезіологічного забезпечення розродження у цієї категорії вагітних.
