Background: Vascular endothelial growth factor (VEGF) is an angiopoetic factor; its variability in circulating levels is mediated by expression of specific VEGF-A gene variants. The aim of this study was to investigate the predictive role of VEGF-A gene polymorphism in clinical outcomes of STelevation myocardial infarction (STEMI) patients. Methods: For the study, 135 patients with acute STEMI and 30 healthy volunteers were enrolled. The G634C polymorphism in VEGF-A gene was performed by real-time polymerase chain reaction at baseline. The 6-month combined clinical endpoint was then determined. Design: The study was an open prospective single-center cohort study. Results: The entire patient population was distributed into two groups based on the G634G-genotype (n = 70) and combination of G634C and C634C-genotypes (n = 65). Unadjusted multivariate regressive logistic analysis showed peak troponin I levels at admission, Killip class of heart failure > 2, GC/CC polymorphisms in VEGF-A gene, and dynamic increase of NT-pro brain natriuretic peptide (BNP) and VEGF-A levels for 6 months, which were independent predictors for the combined clinical endpoint. After adjustment for dynamic changes of NT-proBNP and VEGF-A levels, we found that GC/CC polymorphisms in the VEGF-A gene was an independent predictor of clinical outcome. Kaplan-Meier curves demonstrated that STEMI patients with GG VEGF-A genotype had a lower frequency of clinical combined endpoint accumulation when compared to those who had GC/CC VEGF-A genotypes (Log-rank p = 0.02). Conclusion: The G634C polymorphism in the VEGF-A gene was found to be an independent predictor for 6-month clinical combined endpoint in STEMI patients.

Myeloperoxidase as an important immune factor can be linked to atrial fibrillation (AF) development. Previous studies indicated myeloperoxidase (MPO) as a significant mortality predictor in ST-segment elevation myocardial infarction (STEMI). The MPO role in prognosis for adverse clinical outcomes, as atrial fibrillations in STEMI patients remains not fully understood. The purpose of the investigation was to examine the MPO in prognosis of adverse outcomes presented by atrial fibrillations in patients with STEMI. Methods The study included 233 STEMI patients, 112 males, 121 females, and average age 67.87±7.11 years. Follow-up period was 14 days. Adverse outcome was observed and included atrial fibrillations. Plasma MPO levels were determined by ELISA, MPO was collected on admission. Results Patients with indicated adverse effects showed significantly increased MPO levels compared to those without ones, 169.91 IQR [89.45; 201.98] ng/ml vs 81.4 [33.91;181.29] ng/ml; p < 0.001. A ROC analysis determined a MPO cut-off value for arrhythmia (AF) development of 176.53 ng/ml with a sensitivity of 71.1% and specificity of 77.6%, AUC 0.732; 95 % CI 0.677-0.782; p <0.001. Elevated MPO concentration was a significant predictor for AF (OR 1.115; 95% CI: 1.007-1.841; p<0.001). Even adjusted for age and sex MPO remained significantly predictive for arrhythmia development. Conclusions The study suggests that increased levels of MPO are associated with the higher risk of atrial fibrillation. MPO could be a possible significant factor for therapy optimization in high-risk STEMI patients.

Малої НАМНУ», Харків, Україна kutyainna@rambler.ruМета дослідженнявивчити асоціації між однонуклеотидним поліморфізмом G634C гена ВЕФР-A (rs 2010963) з факторами серцево-судинного ризику, ступенем коронарного ушкодження, характером структурно-функціональних змін міокарда, перебігом госпітального та 6 місячного періодів спостереженням у пацієнтів, що перенесли гострий інфаркт міокарда з елевацією сегмента ST.Обстежено 135 пацієнтів з гострий інфаркт міокарда з елевацією сегмента ST, 109 (80,7%) чоловіків та 26 (19,3%) жінок, у середньому віці (59,21±8,92) років. Рівень ВЕФР-А визначали імуноферментним методом з використанням набору реактивів IBLINTERNATIONAL GMBH, (Німеччина). Дослідження алельного поліморфізму G634C гена ВЕФР-A (rs 2010963) проводили методом полімеразної ланцюгової реакції в реальному часі.Розподіл генотипів гена ВЕФР-А (rs 2010963) у хворих на гострий інфаркт міокарда з елевацією сегмента ST показав наступну частоту: GG, GC та СС відповідно -51,9%, 47,4%, та 0,7%. Подальший аналіз проводився у двох групаху пацієнтів з GGгенотипом (n=70) та носіїв GС+СС-генотипів (n=65). В носіїв генотипу GC+СС частіше ушкоджувалась передня стінка лівого шлуночка (р=0,022). Виявлено достовірні відмінності у виникненні комбінованої кінцевої точки через 6 місяців спостереженняїї частота була достовірно вища у генотипів GC+CC гена ВЕФР-А (р=0,020). Уніваріантний регресійний логістичний аналіз показав, що рівні ВЕФР-А в поєднанні з його генотипами GC+CC, спадковість, ускладнений перебіг гострого періоду гострий інфаркт міокарда та передня локалізація можуть бути предикторами несприятливого перебігу захворювання протягом 6 місяців спостереження (Р < 0,0001).Встановлено, що рівень ВЕФР-А при наявності GC+СС генотипу та ускладнений перебіг в гострому періоді являються чутливими предикторами несприятливих подій протягом 6 місяців спостереження після перенесеного гострий інфаркт міокарда з елевацією сегмента ST.Ключові слова: гострий інфаркт міокарда з елевацією сегмента ST, поліморфізм G634C гена ВЕФР-A (rs 2010963), васкулоендотеліальний фактор росту-А.Зв'язок роботи з науковими програмами, планами, темами.
