Diabetes strikt behandelen is zinnig e´n noodzakelijk GEHM Rutten WJC de Grauw K Reenders Samenvatting Een belangrijke pijler onder de diabetesbehandeling is de United Kingdom Prospective Diabetes Study (UKPDS). Kritiek daarop heeft twijfel aan het nut van strikte diabetescontroles doen rijzen. In de UKPDS werden 3867 mensen (25-65 jaar) met een pas ontdekte diabetes type 2 gemiddeld 10 jaar gevolgd. Een HbA1c-percentage rond 7% bleek haalbaar als 'gemiddelde' over een periode van 10 jaar, hetgeen echter ook impliceert dat heel wat mensen een HbA1c >7% hielden. Met name de kans op microvasculaire complicaties werd kleiner door het verminderen van de hyperglykemie. Metformine had bij mensen met overgewicht een gunstiger effect op zowel micro-als macrovasculaire complicaties dan sulfonylureumderivaten of insuline. De hypertensietrial in het kader van de UKPDS (1148 patie¨nten, gemiddelde leeftijd 56 jaar, gemiddelde bloeddruk 160/94 mmHg, 9 jaar follow-up) maakte duidelijk, dat een verschil in bloeddruk tussen 154/87 mmHg en 144/82 mmHg tot snelle en grote veranderingen in morbiditeit en mortaliteit leidde. De kwaliteit van leven werd niet beı¨nvloed door een strikte diabetesbehandeling, wel door complicaties. Koste wat het kost een HbA1c <7% nastreven is geen goede geneeskunde, maar het zoveel mogelijk verlagen van het HbA1c-percentage is zinnig en noodzakelijk. Inleiding Een mooi, provocatief artikel was het, de kritiek van McCormack en Greenhalgh op de interpretatie van de resultaten van de United Kingdom Prospective Diabetes Study (UKPDS) volgens de hoofdredacteur van H&W.1 Hij verbond er de al even provocatieve uitspraak aan dat strikte diabetescontroles onzin zijn.2 Recentere kritiek op de opzet van bovengenoemd onderzoek deed Zaat zelfs verzuchten wie er nu eindelijk eens echt helderheid verschaft voor de Nederlandse huisarts over het nut van onze diabetesbehandeling.3,4 Dit artikel gaat in op de opzet en de resultaten van de UKPDS, plaatst dit baanbrekend onderzoek in het perspectief van ander diabetesonderzoek en maakt duidelijk waarom hyperglykemie behandeld moet worden. Bij inclusie lag hun leeftijd tussen 25 en 65 jaar; ze werden gedurende gemiddeld 10 jaar vervolgd. De deelnemers aan het onderzoek werden door huisartsen verwezen naar onderzoekscentra. Daar werden ze willekeurig verdeeld in een groep met intensieve behandeling (een sulfonylureumderivaat of 1-4 maal daags insuline) met aanpassing van de medicatie bij een nuchter plasmaglucose >6 mmol/l
