Busca-se neste trabalho discutir a questão das mediações técnicas que, hoje, no campo da saúde/doença, interpõe-se entre o homem e seu corpo e mente. Tratase de um trabalho de natureza essencialmente ensaís-tica no qual a questão é analisada considerando que as formações sociais não são entidades auto evidentes, necessitando, portanto, de modelos teóricos para serem entendidas. Assim, adotam-se, como referência teórico-metodológica, os princípios que regem a pós-modernidade, especialmente aquele que diz respeito à "separação entre meios e fins". Ainda no plano metodológico, tais princípios são ilustrados utilizando-se como referência dissertação de mestrado defendida em 2006, que explora as possibilidades do uso da internet como instrumento de ação empoderadora. A principal proposta apresentada levanta a necessidade de uma ação empoderadora de controle social sobre as instâncias mediadoras, visando enfrentar o fenômeno da hipertrofia das mediações, que faz com que elas se tornem autônomas, deixando com isso de estar a serviço do homem na sua relação com seu corpo e mente. BACKGROUND AND OBJECTIVES: Back pain is one of the most common pain in humans. It impacts the health and quality of life and can be disabling. Diseases detected in adolescence and poorly managed may get worse in adulthood. The objective of this study is to estimate the prevalence, the associated factors and the characteristics of back pain in adolescents living in the city of São Paulo. METHODS: A cross-sectional population-based study-Health Survey in São Paulo (2015) with 539 adolescents of both genders between 15 and 19 years old was used. The information was collected through home interviews and the participants were selected by probabilistic sampling. Frequencies, Chi-square test, and logistic regression analysis were used in this analysis. The level of significance was 5%. RESULTS: The estimated prevalence of back pain in adolescents in the city of São Paulo was 22.4%. Back pain in adolescents had the following associated factors: dizziness (OR 3.1), common mental disorder (OR 2.4), insomnia (OR 2.6) and perform household chores (OR 1.8). To relieve the pain, 46.6% of adolescents do nothing, 17.3% use self-medication and 8.9% use prescribed medication. CONCLUSION: Acknowledging back pain as a public health problem requires strategies that allow us to learn the origins, associated factors and coping strategies that may influence new ways of prioritizing and organizing healthcare.
RESUMO: Introdução: A cefaleia é uma das queixas somáticas relacionadas a problemas de saúde mais comuns entre crianças e adolescentes. Objetivo: Estimar a prevalência de cefaleia em adolescentes da cidade de São Paulo, Brasil, e fatores associados. Métodos: Trata-se de estudo transversal de base populacional, realizado em 2015, com 539 adolescentes de ambos os sexos, entre 15 e 19 anos de idade. Coletaram-se as informações por inquérito domiciliar, e selecionaram-se os participantes com base em amostragem probabilística. Utilizaram-se na análise frequências, teste χ2 e análise de regressão logística. Adotou-se nível de significância de 5%. Resultados: A prevalência estimada de cefaleia é de 38,2% (intervalo de confiança - IC95% 33,8 - 42,7), de dor de cabeça simples 32,8% (IC95% 28,8 - 37,2) e de enxaqueca 7,8% (IC95% 5,6 - 10,7). Os fatores associados encontrados com cefaleia foram: ser do sexo feminino (razão de chances/odds ratio - OR = 2,2; IC95% 1,4 - 3,4), transtorno mental comum (TMC) (OR = 2,8; IC95% 1,7 - 4,9), problema de visão (OR = 2,6; IC95% 1,6 - 4,2), dor nas costas (OR = 2,2; IC95% 1,3 - 3,5), sinusite (OR = 2,0; IC95% 1,2 - 3,4) e ter escolaridade com ensino fundamental II incompleto (OR = 3,0; IC95% 1,6 - 5,6). Conclusão: A prevalência de cefaleia em adolescentes da cidade de São Paulo em 2015 representou mais de 1/3 (um terço) dessa população. Os principais fatores associados foram sexo, baixa escolaridade e as comorbidades TMC e problema de visão.
